While grinding and wearing safety glasses got a piece of metal in eye

While using a hammer smacked the back of left hand with hammer,

Jumped off bulldozer, landed on rock, twisted ankle

Missed bottom step exiting front end loader, fell and hit back of head

Trying to remove a tub liner from the Impactor and hit left thumb with a 4 lb hammer.

Miners were attempting to lift a rock out of jaw crusher with a chain attached to 980 loader bucket, when operator was told to lower bucket, employee placed his hand on the jaw hopper frame, under bucket and pinched his fingers between bucket and hopper frame, resulting in two smashed fingers.

Rock backed up in jaw, employee was trying to remove it, rocks broke free and smashed his thumb.

The lid on the VSI was opened at the end of the shift to do maintenance. It was left open overnight to finish the next day. When they started changing lid liners, they didn't know the Hyd pressure had bled off but the lid was stuck in the raised position. EE stood on the lid & it dropped apprx 2 ft causing him to loose balance & fall off. He was not wearing fall protection.

EE WAS SHOVELING MUD FROM THE ROLLERS HIS RIGHT ARM GOT CAUGHT UP FRACTURED FOREARM.

CLEANING BELT-DID NOT HEAR WHEN ALL CLEAR CALLED-BELT TURNED ON-RIGHT ARM PULLED INTO TAIL ROLL.

MUCKING UNDER 2ND SCREEN-HAND GOT CAUGHT IN MIDDLE IDLER & PULLED ARM IN ALL WAY.
