Lid of storage box on loader fell onto employee's right hand ring finger, lacerating skin on right side of fingernail. Originally did not think this was reportable since there was no lost time and restrictions did not inhibit employee from doing his regular job. Now it has been determined that the rx given makes this a reportable incident since it was more than a single dose.

Mostrando los 6Employee was lying on the ground underneath loader trying to tighten bolts on center pin using a ratchet and breaker bar. The bolt snapped off and the employee jerked himself out of the way from the falling cheater bar. At this time he felt some pain in his shoulder and on up into his neck. Pain increased as the day progressed.
Employee was working on screens putting punch plates in place. He and another employee (Paul Kunisch) were guiding plates into position and his right hand got caught between the side of the screen and the punch plate.
THE WIND BLEW A 2'-3' PIECE OF PLYWOOD INTO EE BACK WHILE HE WAS WORKING IN A HOPPER. EE WENT TO HOSPITAL WHERE THEY PUT HIM ON LIGHT DUTY, NOLIFTING OVER 10 LBS. HE HAD A FOLLOW UP ON 2-17- 03 AND WAS RELEASED TO WORK WITH NO RESTRICTIONS
EMPLOYEE WAS WELDING ON TRUCK BOX. WHEN HE CHIPPED HOT SLAG OFF OF WELD, A CHIP FLEW INTO HIS MOUTH.
