Employee received a small avulsion fracture while changing V-belts on an impact crusher.
Employee was pulling the slack out of new conveyor belting when his hand slipped. Employee lost his balance and fell backward hitting back of head on the ground.

This caused the employee's left hand to be caught between the shaft and conveyor frame. Employee received a crushing injury to his middle, ring and little finger on his left hand.
While greasing belt on wash plant, ladder gave way and ee fell to the ground hitting his elbow.
Employee was changing the screens on a 5 x 16 shaker, when he lifted a screen and strained his lower back.
Employee was tied off inside the primary crusher attempting to unplug the crusher. He was moving a 25 to 30 lb rock when his left elbow go t caught in his lanyard causing the weight to the rock to shift to his left hand. When the rock shifted, he lost control of it and smashed his left hand in between two rocks.
The employee was pulling electrical wire and felt a pain in his right shoulder.

Approx 3:15am, witness called police reported they had witnesses a vehicle traveling at high rate of speed, ran a stop sign at 46th & 129th Ave ran through gate. Police responded but unable to locate vehicle. Approx 8:30am EE's reported a vehicle had crashed into the pit floor.
Employee using a cable come along to realign a tail pulley when he felt a pain in his right groin region.
EE was resetting a steering cylinder in a Caterpillar 773A haul truck when the cylinder's weight shifted and smashed his finger in between the cylinder and the frame of the truck, causing a small laceration on his ring finger on his left hand.
EE was climing into 773A Haul truck when he attempted to open the door and lost his hand hold. He fell backwards off the catwalk approximately 8 feet to the ground and landed on his left wrist.
Employee was turning around on level ground to walk to the restroom and felt a pain in his right knee.
EE was using an air chisel to hammer out a metal pin on the front of a haul truck when a piece of metal fell off and stuck in the tip of his nose causing a laceration.
Employee was lifting motor frame with a ratchet cable puller when the hook slipped off the motor frame and the motor frame struck the employee on the left knee.
EMPLOYEE WAS CLEANING ROCK OUT FROM AROUND A HEAD PULLEY ON A CONVEYOR AND HE CAUGHT HIS FINGER BETWEEN A ROCK HE WAS LIFTING AND AN ELECTRIC MOTOR'S FRAME WHICH LACERATED HIS FINGER.
EE WAS CLIMBING INTO HAUL UNIT (#423-866) WHEN HE MISSED A HAND HOLD AND FELL OFF LADDER.
EE ALLEGES HE WAS WALKING DOWN STAIRS ON SECOND LEVEL OF WASH TOWER & SOMEONE LEFT A CROWBAR ON STAIRS. HE DID NOT SEE IT BECAUSE IT WAS COVERED W/DUST. HE STEPPED ON IT & ROLLED CAUSING HIS FEET TO SLIP OUT FROM UNDER HIM WHICH CAUSED HIM TO FALL. EE ALLEGES HE FELL ONTO HIS BUTT & LANDED IN A SITTING POSITION ON THE STEP ABOVE WHERE THE CROWBAR WAS LOCATED. HE ALLEGES A STRAIN.
EE TRIED TO MOVE ROCK THAT HAD FALLEN OFF OF A HAUL UNIT OUT FROM IN FRONT OF THE 209 CRUSHING PLANT.
EE PULLED HARD ON A GREASE HOSE THAT HAD BECOME LODGED AT THE 8 X 20 SCREENING DECK AND HE FELT A SHARP PAIN IN LOWER STOMACH AND GROIN AREA.
EE WAS OPERATING A LOADER WITH THE WINDOWS OPEN & DUST/DIRT FLEW INTO HIS RT. EYE. EE WASHED EYE WHEN INCIDENT HAPPENED BUT HIS EYE WAS SWOLLEN & SORE THE NEXT MORNING & HE WANTED TO SEEK MEDI CAL ATTENTION.
EE WAS CUTTING BELT ON CONVEYOR WHEN KNIFE SLIPPED AND CUT HIS LEFT KNEE. STITCHES WERE NOT INCLUDED ON DR.'S REPORT BUT AFTER QUESTIONING EE FURTHER ON 3/25/03, APAC FOUND OUT THE EE HAD RECI EVED 2 STITCHES IN KNEE.
EE WAS CHECKING OIL & HYDRAULIC FLUIDS ON HAUL TRUCKS & WAS WALKING BETWEEN THEM WHEN THE WIND BLEW DUST IN HIS EYES. EE WENT TO ER LATER THAT NIGHT BECAUSE THEY HURT. CORNEAL ABRASIONS, CLEAN ED, PRESCRIPTION MEDICATION.
EE WAS RIPPING SURFACE ROCK W/D8 DOZER WHEN RIPPER CAUGHT THEN RELEASED CAUSING DOZER TO BUCK, SPRAINING EE'S UPPER BACK. EE DID NOT REPORT UNTIL 9-3-02 & WAS SENT TO DOCTOR. DR EXAMINED, X-RA Y NEGATIVE, PRESCRIPTION MEDICATION, RESTRICTED, RELEASED TO REGULAR DUTY TODAY.
EE IMPROPERLY BENT OVER TO LIFT AN 8" DISCHARGE HOSE AND STRAINED HIS LOWER BACK. SENT TO DR., X-RAY NEGATIVE, PRESCRIPTION MEDICATION, RESTRICTED DUTY, DIAGNOSED AS L-S STRAIN.
EMPLOYEE WAS SWINGING A PICK WHILE SHOVELING AND LOWER BACK BECAME SORE. REPORTED WHEN STILL SORE ON 4-24-02. SENT TO DOCTOR. X-RAY NEGATIVE PRESCRIPTION MEDICATION, RESTRICTED DUTY.
EE RAN A FRONT END LOADER ALL DAY AND BACK BECAME SORE THEY NEXT MORNING SAT EE REPORTED MONDAY 4-8-02. SENT TO DR EXAMINED, LS STRAIN, PRESCRIPTIONAL MEDICATION, RESTRICTED DUTY. X RAYS NEGAT IVE.
EE CAME INTO OFFICE WITH WIFE. HE HAD BEEN OFF FOR A PERIOD OF TIME. HE SAID HE HAD AN ALLERGIC REACTION TO SOMETHING, BROKE OUT IN HIVES, THEN HAD PROBLEMS BREATHING. HE SAID WHEN HE WENT IN FOR HIS FINAL CHECK THE DOCTOR RELATED TO HIM THAT HE HAD ACUTE SILICOSIS. WE HAD HIM SIGN A MEDICAL RELEASE & SENT FOR HIS RECORDS. (THIS IS ANALLEGED ILLNESS/CLAIM UNTIL FURTHER INFORMATION)
EMPLOYEE WAS STRIKING THE RACE OF A LARGE BEARING TO DISLODGE IT WHEN A SMALL PIECE OF THE HAMMER HEAD FLEW INTO HIS INDEX FINGER, RIGHT HAND. X RAY NEGATIVE, EXAMINED, SLIVER OF STEEL TO BE REMOVED BY HAND SPECIALIST.
EMPLOYEE WAS CLIMBING DOWN A LADDER AND STEPPED WRONG ON TO UNLEVEL GROUND TWISTING RIGHT ANKLE. DOCTOR EXAMINED, X-RAYS NEGATIVE, PRESCRIPTION MEDICATION, RESTRICTED DUTY.
EMPLOYEE STEPPED DOWN FROM LAST STEP ON LOADER TO GROUND. HIS RIGHT FOOT HIT A RUT AND HE TWISTED HIS ANKLE. CHECKED, X-RAYED, PRESCRIPTION MEDICATION, BRACE, RESTRICTED DUTY.
EE STEPPED ON A ROCK WITH HIS LEFT FOOT AND TWISTED HIS LEFT ANKLE. XRAY NEGATIVE, PRESCRIPTION MEDICATION, RESTRICTED DUTY.
EE WAS HOLDING A NUT WITH A WRENCH WHILE ANOTHER EE LOOSENED THE BOLT. THE NUT BROKE LOOSE AND THE EE S MIDDLE FINGER ON HIS RIGHT HAND GOT MASHED BETWEEN THE WRENCH AND STEEL CAUSING A LACERA TION. X RAY NEGATIVE, STITCHES, CLEANED AND BUNDAGED.
EMPLOYEE WAS GETTING DOWN FROM HAUL UNIT AND MISSED HAND GRIP. HE FELL TO GROUND, HITTING ON HIS FEET AND ROLLING ONTO LEFT SIDE. WE SENT HIM TO DOCTOR TO INSURE HE WAS OK; X-RAYS NEGATIVE, CONTUSIONS (BRUISES) TO LEFT SIDE (HIP, LEG, ETC.) PRESCRIPTION MEDICATION, RESTRICTED DUTY. RECHECK ON 8/14/00 A.M. RELEASED TO REGULAR DUTY.
EMPLOYEE WAS MOVING AN ELECTRIC MOTOR WITH A PRYBAR AND STRAINED HIS LOWER BACK. DOCTOR PRESCRIBED MEDICATION & RESTRICTED DUTY.
