A piece of framework was being removed with a cutting torch. When the framework was cut free it tipped in a manner which struck employee in the head. Employee was wearing a hard hat which was knocked off. They received approximately a 2 in laceration to the top of head. Employee was observed for approximately 15 min before they drove themselves to the Hospital.

Lost balance on a flat surface and fell on hand
Tracking cable on to yarder spool. Clamp came loose causing cable to come out and hit employee on arm and leg.
Employee stated has worked for multiple companies air arcing and other noisy work over EE's work experience and hasn't always worn protection. Protection is provided in the shop and EE has had numerous training on hearing conservation. EE stated to MSHA that 80-90% of EE's work is fixing over the road trucks and construction equipment.
