Excavator operator was excavating smoldering material. MSHA inspector stated EE momentarily saw flames. All personnel were withdrawn from the mine immediately. There was no damage or impairment to operators or equipment.

Employee tripped on a rock when returning to truck upon completion of specific task
Worker was using a belt polisher for approximately 1 hour on 9/10/20. Worker told a co-worker that worker's back was hurting after task was complete and continued working for the shift. On 9/11/20 worker was sitting in the service truck for about 10 minutes when right leg turned numb and stated worker could not walk.
Employee was in the jaw rock crusher control room when EE went to step up on the platform at the control desk. Employee's foot slipped causing EE to fall. EE attempted to catch self and while doing so EE's right hand went through the glass of a window.
Replacing wishbone on rear side of 772 truck pulled something in back
Employee was changing side liner in the impact crusher the plate slipped hitting the ring finger on the right hand causing a small laceration.
Repairing the rock box wall under the impact crusher aprons. They were using a skid steer to lift the steel plate to the conveyor under the crusher, below the wall. This is a confined area and caused them to be bent over slightly. The size of the plate and the confined area caused them to shift the plate to one side, with claimant taking the extra weight causing the injury.
Dumped rock out of truck. Was holding tailgate to clean rock off spreader pan. The scraper slipped and caught hand. Lacerated left ring finger.
Dozer operator got the dozer stuck while trimming a muck pile. It was determined that the operator was safer in the cab rather then trying to climb out. So it was determined the operator was "in effect trapped" in the cab for approximately 75 min while equipment was mobilized to assist in safely securing the dozer.
Employee was holding carriage bolt up so that a nut could be removed with an impact wrench. As the nut was being removed the bolt dropped down and was spinning. The head of the bolt was worn down and had a sharp edge. The spinning bolt cut through the employee's glove causing a laceration to his left index finger requiring 16 stitches.
