Employee was changing a bearing on a tail pulley that snapped forward and cut left pinky finger.

Employee placed his left hand in door jam of loader, door wasn't securely latched which allowed the door to slam shut breaking his finger.

Employee hurt wrist & forearm while shoveling.

Shop area - employee was carrying part of a steel blocking when his right knee popped.

Employee was tightening a bolt on excavator bucket with 3/4" impact wrench. His finger got caught between bolt and h-link causing a gash on left middle finger. Employee was taken to the nearest hospital where his finger was stitched.

After following proper lock out tag out procedures, EE went to complete the task of dislodging a rock from the under belt conveyor of the Minyu jaw. While not visible from walkway approaches, a few pieces of material had worked to the edge of the feeder. While attempting to remove the rock, a rock from above struck hardhat of EE causing him to fall to the ground.

Twisted ankle on a rock

Employee was hit in the chin by a cable when he attempted to jump over it, but instead stepped on the cable as it was being tightened. Employee went to the doctor for some stitches and then went back to work for the rest of the day.

Two employees were placing a backstop for a ramp to a feeder hopper. The backstop, 5' x 15' x 1/2" steel plate, was lifed vertical to lean against feeder, the groundman pushed up on the steel plate by hand to over center when he did that the chain came unhooked from the plate which caused the steel plate to fall toward him pinning his leg against the ground.

Employee was putting the guardrail back on a loader. The rail slipped and cut the employee's left index finger.

Employee was repairing roller belts. Hand was drawn in and pressed between roller and conveyor belt.

Cutting on El-Jay screener - a piece of slag fell down into left top of boot causing a burn to the ankle area. Removed boot, administered first aid and employee was taken to the doctor.

Employee was cutting a screen with a grinder. The grinder slipped and cut his finger.

Employee was cleaning up floor pit with a loader. He hit a rock which stopped the loader - seat belt came lose and the employee hit his head on the windshield and his stomach hit the steering wheel causing some bruising.

Employee was replacing O-ring of steering cylinder with 14mm wrench and it slipped off bolt, right hand jerked back hitting against the corner edge of metal frame on the loader. Employee could not open his hand afterward and was told to go to the doctor.

Material in a cone discharge conveyor tailpully caused the belt to stop, the plant operator radioed ee to check out the belt. He climbed up on the plant grabed the drive belts to turn the conveyor backword by hand to clean the tailpulley out. As he did the plant operator started the conveyor.His thumb went arond the Vee Belt Pulley severing the tip of his thumb.

Employee was shoveling around a crusher feed belt, which was feed by another belt. He noticed some material had built up in the rock box and was spilling over onto the ground. As he attempted to clear the material with a shovel, he placed his hand on the rock box and was struck on the left hand index finger tip by a rock.

Employee was driving a loader and hit a pothole and claims his back hurts. He went to the doctor but has continued working full time.

Employee sliped and fell off the Screen Deck landed wrong on his foot which resulted in a possible sprained ankle

EE WAS ATTEMPTING TO CLEAN UP A PISTON ON THE WIRE WHEEL SIDE OF THE SHOP BENCH GRINDER. THE TOOL REST WAS BROKEN OFF & ONE WAS ON ORDER. THE GRINDER SHOULD NOT HAVE BEEN USED. EE WAS HOLDIN G PISTON BETWEEN HIS 2 HANDS & PISTON GOT CAUGHTIN THE WIRE BRUSH & CAUGHT THE TIPS OF HIS FINGERS

TURNING A LOADER WHEEL OVER- FLANGE OF WHEEL SLIPPED - FINGER GOT CAUGHT BETWEEN FLANGE AND WHEEL AND RIGHT FINGER WAS LACERATED.

OPERATOR USED HIS FOOT TO TRY TO MOVE A STUCK PIECE OF ASPHALT AND IT KICKED BACK AT HIM PINCHING HIS ANKLE EITHER BETWEEN TWO PIECES OF ASPHALT OR BETWEEN THE SIDE OF THE FEEDER AND THE ASPHA LT.

THE WELDER WAS USING ELECTRIC GRINDER AND A PIECE OF FOREIGN MATERIAL GOT INTO HIS LEFT EYE.

EE STEPPED OUT OF HIS TRUCK HE TOOK A COUPLE OF STEPS AND WENT DOWN AT THE TIME HE WENT DOWN HE COMPLAINED OF PAIN SHOOTING DOWN HIS LEG LIKE HIS HIP WENT OUT OR HIS LOWER BACK WENT OUT.

ADJUSTING A TAIL PULLEY AND WRENCH CAME OFF AND GOT ON TAIL PULLEY THEN HTI MY HAND.

MOVING STEEL BEAMS, TURNED AND SEEMED TO TWIST LEFT LOWER BACK.

A ROCK HE WAS MOVING SLIPPED AND SQUEEZED HIS FINGER BETWEEN ANOTHER ROCK.

CHAIN SADDLE ON BUCKET OF LOADER PULLED ON CHAIN TO ALIGN SADDLE W/PICK POINT BUCKET APPROX 10' OFF GROUND WHEN CHAIN SADDLE PEEPLED OFF BUCKET EDGE AND FELL TOWARDS HEAD AREA AND IN THE PROCE SS OF WARDING OFF THE BLOW W/RIGHT ARM ELBOW WAS IMPACTED RECEIVING THE DAMAGE.

LIFTING AN DINSTALLING HYD MOTORS ON 980 C WHEEL LOADER IN TIGHT CONFINED AREA.
