While working in a man-basket; EE had reached for a drill and while doing such EE struck his mouth on the gate that had rotated out of postion because the top hinge pin had come out of the hole that it was mounted through. EE did not go to doctor until either 3/14 or 3/17. EE did see his family dentist for care of the tooth during the evening of 3/11/2008.

EE was attempting to reposition a metal section of the vent tubing so that the tube mated better with the section already mounted in the back. EE was wearing belt & lanyard but when attempted to reach a clamp he loosen his lanyard & he stepped on the rail to reach the clamp; when he reached the clamp & cable, the tubing gave away & fell. The EE fell on vent tubing and then ground.

While moving a 2" airline, EE stepped on a 4" discharge line that caused his fall and broke leg.

When cleaning product chute for maintenance/ repair frozen material slid down and hit left knee. Follow up doctor visit (6-17-04) EE stated that knee still hurt.

HE WAS DRIVING TO GET RELOADED AND HIT A HOLE INTHE GROUND AND HIS BACK GOT HURT.

WHILE CUTTING A TIE STRAP THE KNIFE SLIPPED AND CUT HAND.

EMP WAS TIGHTENING A LOCK NUT ON A BEARING WITH A HAMMER WHEN A PIECE OF METAL CAME OFF AND WENTINTO THE EYE.

THE EMPLOYEE PUT A LADDER ON THE SIDE OF THE CONVEYOR AND ACCIDENTALLY GRABBED THE ELECTRIC MOTOR SHEAVE AND CAUGHT HIS HAND BETWEEN THE DRIVE BELTS AND SHEAVE CAUSING AN OPEN FRACTURE OF THE RIGHT LONG FINGER DISTAL.

WHILE SPLICING CONVEYOR BELT, EE CUT LEG WITH UTILITY KNIFE, CAUSING A LACERATION WHICH REQUIRED SUTURES.

SLIPPED AND FELL ON THE ICE WHILE MOVING AND SETTING UP EQUIPMENT. THE IMPRESSIONS/DIAGNOSIS WAS A FRACTURE OF THE 9TH RIB.

BLASTER WAS LOADING SHOT, CARRYIN G50# OF ANFO, STEPPED ON ROCK. HURT LEFT KNEE.

OPERATING ENGINEER WAS WORKING IN MUDDY CONDITIONS, THERE WAS A CRACK UNDERNEATH MUD. HE WENT TO TURN AND TURNE DLEFT ANKLE FOOT STARTED TO SWELL UP.

EMPLOYEE SLIPPED ON A WET CONVEYOR BELT AND LANDED ON LEFT SIDE OF HIS BACK.

WHILE IN THE PROCESS OF MOVING A 3" GAS POWERED PUMP HE SMASHED HIS LITTLE FINGER OF HIS LEFT HAND BETWEEN THE TRAILER TONGUE AND THE STIFF LEG.

EMPLOYEE BENT OVER TO CHECK WATER TRUCK AND COULN'T STRAIGHTEN UP.

HEATING NUTS ON PC OF EQUIPEMENT SO THAT THEY COULD BE REMOVED ANOTHER OPER USING IMPACT WRENCH TO REMOVE THE LIP WAS CUT AND FRONT TOOTH CHIPPED. STITCHED UP AND HAD ROOT CANAL

HE WAS GOING TO SHUT OFF THE CONVEYOR AND SLIPPED ON SOME ICE/SNOW, FELL, AND FRACTURED HIS LEFT LEG.

EMPLOYEE SLIPPED ON ICY EQUIPMENT AND CUT RIGHT ELBOW REQUIRING THREE STITCHES.
