Mostrando los 15Associate was walking up stairway and the top stair tread gave way causing him to jar his left leg and land on his buttocks on the landing. On September 6th, he went to All Cities Medicines and was diagnosed with a sprain achilles tendon and given restricted duty. Associate had the same achilles injured on 08/30/2010 and was a LTA with surgery.
Associate was helping maintenance install a guard in an elevated area. While standing with his foot on a step he let his heel get lower than his toes and felt a sharp pain in his leg.
Employee was performing maintenance on the dredge. He was tightening loose bolts on the frame structure of the dredge ladder. He was holding wrench on the bolt when it slipped off pinching & cutting the end of his first finger right hand between the wrench and angle structure.
Employee was looking at object in screen waste containment area. He bent down to look at object when wind blew foreign object from containment into left eye.
Employee was exiting the cab of Caterpillar 980H loader slipped and twisted Back.
Employee was performing repairs to plant feed hopper (#HO-06) walls. While patching wall employee stated the wind blew sand or rust around his safety glass's into his left eye. This feed hopper is located in the stockpile area, outside in the weather.
While cleaning under a hopper, small amounts of rust from a pipe fell into the face of the employee.
EE WAS MANUALLY PULLING A CABLE & WHILE PULLING HE FELT HIS BACK MUSCLES START TO HURT.
EMPLOYEE WAS LIFTING A 5 GALLON BUCKET WEIGHING APPROX 15 LBS AND WHEN EMPLOYEE LIFTED PAIL FELT PAIN IN HER RIGHT SHOULDER AND ARM.
EE WAS ENTERING BOBCAT SKIDLOADER WHEN HE SLIPPED AND HIT HIS LEFT KNEE ON THE BUCKET.
WALKING UP STAIRS TO OPERATORS SHED WHEN RIGHT KNEE GAVE OUT FELL ON RIGHT KNEE.
EE WAS DRYING A BEAKER WHEN THE BEAKER BROKE CUTTING HER RIGHT RING FINGER.
EE WAS USING A GAS TORCH TO CUT BOLTS OFF REMAINING FASTENERS TO REMOVE LOOSE CUTTING EDGE FROM BUCKET OF PAYLOADER. BUCKET WAS APPROX. 1-2" GROUND AND EE WAS STANDING IN FRONT OF IT. BUCKET F ELL STRIKING RIGHT FOOT (BIG TOE AND 2 SMALL TOES).
EMPLOYEE WAS GETTING OUT OF THE BOBCAT WHEN HIS FOOT SLIPPED ON THE PLATE OF THE BOBCAT. HE HAD A PAIN IN HIS CALF.
