Employee was welding when a spark/slag became airborne and landed in EE's ear. No internal damage found during clinical observation. Employee was given antibiotics as a precautionary measure.

EE reports gradual onset of lower back/sacral discomfort while operating Front End Loader. No specific incident, but they report that Loader was "bouncing around" a lot during operation. Seat was verified to be in proper operational condition.

After getting down from a loader, employee tripped over a rock in the mud and sustained injuries to EE's ankle.

While using the porta potty restroom employee stung on the right butt cheek due to bee being on toilet seat.

Subcontracted haul truck driver found unresponsive, crew, medics and emt's were undable to revive driver. Driver was a subcontract employee of another company so I dont have all the victim information available.

IE and another EE (Transco NW welders) were removing a channel frame from the crusher frame. IE cut the cross brace 99% & other EE held the other side. When EE pushed the opposite side while IE pulled IE's side free, the cross brace came free & EE lost EE's grip. The brace rotated 180 degrees and the bottom corner struck IE on the right side of nose.

The injured employee was using a Dingo TX 525 to remove accumulated material beneath a structure. While operating the equipment in reverse, the bucket contacted a vertical steel member. The sudden motion crushed EE's left wrist between the Dingo controls and the diagonal structural steel member. The diagnosis was a strain and contusion of the left wrist; no broken bones.

While sitting in the loader, a wasp flew into the cab and stung operator on the right arm near EE's bicep. EE did not have any abnormal reactions to the sting and at this point no first aid has been administered. Intercompany nurse hotline, WorkCare was called and the incident has been captured.

Looking at base of spill shed to see if stable to move, got stung on upper chest and below right eye.

Lifting 60 lb. rock samples repetitively into bed of pickup truck, which created a repetitive motion injury.

A Lead Hand and another miner were replacing a roller on a cross over belt for the screen plant. In order to remove the bolts from the back side of the roller, the Lead Hand knelt down on the under screen belt and instantly felt pain in his right knee. The Lead Hand was taken to the local ER, where he was diagnosed with knee pain and placed in a splint to immobilize his knee.

Employee was hanging the upper guard at the tail wheel section of the uphill conveyor. Another employee had one end of the guard and the IE had the other end. The guard was sitting on the top edge of the lower guard when the IE moved it to put a bold in it when it slipped off the top edge. His finger was in one of the holes and it was cut as the guards slid past each other.

Contractor was in loader operator's blind spot when he was struck in the right thigh below his buttocks by the loader bucket. Both parties failed to communicate their intended actions to the other. The contact caused some deep bruising but no cuts or abrasions to the skin. Precautionary x-ray results were negative.

Employee was grinding inside a hopper and the grinder caught something and was torn from hands and the grinder hit the employee in the face, requiring stiches.

Employee was pulling strut using his pry bar, pry bar slipped and came back and hit him in the mouth.

Employee threw a chain over a beam to hang a come-a-long and the chain impacted his hand and broke his finger

While opening the back door to the MCC, employee pulled muscle in back.

Stepped on rock and turned ankle

After manually lifting 10-20 pieces of steel weighing about 200 lbs, worker felt pain in his lower back.

WHILE LIFTING & THEN MOVING A STEEL BEAM IT SLIPPED & PINCHED HIS FINGER.
