EE WAS OPERATING JACK HAMMER WHEN HE SLIPPED ON A LOOSE PIECE OF SHELL. THE EE & THE JACK HAMMER SLIPPED. THE JACK HAMMER FELL ON EE'S LEFT ARM CAUSING THE SMALL BONE IN LEFT ARM TO BREAK.

PULLING A SMALL PIECE OF GRANITE WITH CHAIN. CHAIN BROKE, DOGS STARTED SWINGING, HIT FOREHEAD. SMALL CUT, 2 STITCHES REQUIRED. NO TIME LOST FROMWORK.
