Employee says he was prying on a drive sprocket and felt a slight pull in groin area. This was not reported until 2/28/07. He says he did not report it as he thought it amounted to nothing. His Dr wanted him to cease activity for 2 weeks as he had a previous hernia repair in the proximity of his soreness.
Employee was servicing a dozer when he stepped on a cobble and twisted left knee. *THIS EMPLOYEE DID NOT REPORT HIS INJURY TO THE COMPANY UNTIL 3/18/05*
EE was removing his overalls at the end of work shift. He noticed his right middle finger was hanging at the first joint. Does not know how it occurred.
EMPLOYEE WAS HELPING TO REMOVE A METAL PLATE FROM A BARMAC CRUSHER. ANOTHER EMPLOYEE WAS HITTING THE PLATE WITH A PRY-BAR. EE REACHED DOWN TO GET THE PLATE WHILE THE OTHER EMPLOYEE WAS HIT IT AGAIN STRIKING HIS HAND.
EMPLOYEE WAS PICKING A PIECE OF METAL OFF A CONVEYOR THAT HAD COME THROUGH A CRUSHER AND BURNED HIS RIGHT HAND ON THE METAL.
EE WAS OPERATING LOADER TRAVELING OVER ROADWAY AND HIT LARGE RUTS IN ROADWAY WHICH CAUSED PAIN IN HIS RIGHT SIDE OF NECK, LOWER AND UPPER BACK.
EMPLOYEE WAS PUSHING TIMBER INTO LOCATION AND FELT A STRAIN IN HIS LEFT ARM. THIS ACCIDENT WAS NOT REPORTED TO THE EMPLOYER UNTIL 12/4/02.

ANOTHER EMPLOYEE WAS BARRING (LIFTING) MOTOR WHILE EE PLACED SHIM UNDER MOTOR AND THE BAR SLIPPED OFF MOTOR FRAME CUTTING EE'S FINGER (LEFT INDEX).
LOADER RAN INTO THE FRONT OF WATERTRUCK CRUSHING IT IN. WATERTRUCK DRIVER WAS ABLE TO GET HIMSELF OUT OF WATERTRUCK. EE WAS TAKEN TO EMERGENCY CENTER AND CHECKED BY DR, DR RELEASED EE TO RETUR N TO WORK AS HE FOUND NO INJURIES. NO X RAYS OR PRESCRIPTIONS WERE ISSUED. 3 WEEK 6-18-01 AFTER THE INCIDENT THE EE STARTED EXPERIENCING PAIN AND RETURNED TO THE DR FOR A FOLLOW UP. DR ORDERE
EE LISTING 25 LB. BOXES OF ASPHALT CONCRETE FOR TESTING.
