Employee was resetting the motor starter for the cone when it is assumed they came in conduct with an energized conductor. Employee woke up on the ground and was found by coworker at which point EMS and MSHA was called. Employee was released from hospital on 04/07/2022.

Mostrando los 3Employee was removing the hydraulic pump of an excavator. EE told me that oil had spilled on the tracks and they had fallen. EE told me they were fine. When I left EE was working on a truck tailgate. The next morning, EE called in sick. On 9/10/18 EE was seen at St. Charles ER and diagnosed with broken ribs. EE was cleared to return to work on 9/13/18.
Employee was holding onto a piece of 3" tubing while another employee was striking it with a sledge hammer. The sledge hammer glanced off a bolt head and hit employee holding the tubing in the wrist. The incident resulted in a broken wrist.
