Employee was operating a skidsteer when it lost power and began to slide. Employee jumped for the equipment causing a sprain in employee's knee.

Employee ran equipment while it was overheating.

Employee was cleaning chute when pressure washer hose got loose and smashed employee's finger against the rail, causing a laceration

Injured worker was driving down into the mine shaft and the brakes went out in the truck EE was driving. EE hit a wall to avoid hitting the vehicle in front of EE and the truck flipped upside down.

Employee suffered a laceration to their arm, hand scaling a heading.

Employee dropped a piece of flat steel on ankle causing a contusion to the right lower leg/ankle.

Employee was hand scaling and loose rock fell and struck EE's hand, creating small laceration on left hand.

Employee states the controls on the Powder Monkey basket malfunctioned and pinned employee against the ceiling bruising their ribs.

Night shift employee was lifting a metal plate out of the third level dump hopper. As EE was attempting to pick the plate up EE felt a sharp pain in the lower back. EE then reported the incident to the supervisor. EE was taken to a nearby medical center where EE was treated for lower back muscle strain.

Employee was kneeling on conveyor belt and slipped, EE rolled off into the frame work where EE's leg got caught.

Employee was using hammer to remove steel coupler from drill and a piece of the coupler splintered off and lodged in employees right index finger.

Employee was using a pry bar to trip the door on a transfer chute located at level 2 crushing plant. As he was prying in a downward motion, the pry bar slipped causing him to fall injuring his neck. Note: Employee was working on the ground level near the chute.

After checking the coolant and air filter box for the pre-shift inspection of the Jumbo Drill, she slipped and fell to the ground when dismounting the drill. The employee landed on the her left side and injured her left lower back.

Employee hit a bump while driving the haul truck and felt a sharp pain in the right side of neck.

The employee bent to pick up the guard to replace on tail pulley after cleaning. When he lifted the guard from the ground he strained his back.

Employee lost footing exiting the rear of the service truck while starting the air compressor mounted on the back of the truck. The employee injured left knee.

Still under investigation. ** The miner was operating a loaded 40 ton articulated haul truck down a slope when the truck went out of control and hit a rib. The cab section overturned. The victim was ejected from the truck. **

Mechanic was grinding on steel when personal grinder exploded, came apart and cut upper right thigh.

VICTIM WAS ON A BELT CONVEYOR TO CUT OFF A LOOSE PIECE OF BELT. HE FELL APPROXIMATELY 20 FEET TO THE GROUND BELOW. HE DIED ON JANUARY 11, 2011. HARD FROST THE MORNING OF THE ACCIDENT. FAILED TO WEAR COMPANY SUPPLIED SAFETY HARNESS.

Brake failed on WA 500 loader and rolled back 75' - 100' on slope hitting fuel truck.

EMPLOYEE WAS OPERATING HIS PERSONAL VEHICLE ON THE MINE PROPERTY & LOST CONTROL. EE SLID INTO A BERM & ROLLED THE TRUCK. HE WAS NOT ON DUTY AT THIS TIME & HE WAS NOT OPERATING A COMPANY VEHICLE. EE WAS DRIVING IN AN AREA THAT HE SHOULD NOT HAVE BEEN IN.

EMPLOYEE ALLEGES HE WAS GETTING READY TO LOAD EXPLOSIVES AND ROCK FELL FROM FACE AREA HITTING HIM IN THE BACK.

CENTER PIN ON DRILL BOOM BROKE CAUSING BOOM ON DRILL TO LET GO WHILE RAISING STEEL OUT OF HOLE.

WHILE CLEANING OUT @ PREPARATION TO PRIME A HOLE, ROCK SLIDE FROM ABOVE THE HOLE CUTTING HIS RT.HAND, REQUIRING SUTURES.

EMPLOYEE STEPPED (ALLEGES) BACKWARD OFF DUMP TRUCK WHILE CHANGING OUT WINDSHIELD WIPER, LANDING ON HEELS OF FEET.

EE WAS SCALING THE ROOF AT HEADING WHEN HOSE DEVELOPED A LEAK. IT FLOWED DOWN ARM CAUSING EQ TO ARC/SPARK. EE REACTED BY JUMPING OFF OF EQUIP AND FALLING, INJURING HIS L. ELBOW AND L. CALF MUS CLE.

ROOF FALL IN INTERSECTION GOING TO SHOP AREA.

EE WAS ATTEMPTING TO REMOVE A 16' DRILL STEEL WHICH WAS STUCK IN THE HEADING WHICH RESULTED IN LOWER BACK LUMBAR STRAIN. ORIG DR REPORT STATED EE WAS TO RETURN TO WORK 8-13-01 ON 8-15-01. HE R ETURNED TO DR AND WAS TOLD TO BE OFF WORK UNTIL 8-22-01.

EMPLOYEE WAS MARKING OFF FACE HEADING N-16 FOR DRILL HOLES TO BE DRILLED. ROCK FELL FROM FACE AREA AND STRUCK HIM IN THE RIGHT SIDE OF HEAD.

CLIMBING DOWN FROM LADDER - STEPPED ON ROCK THAT MOVED CAUSING FALL - BROKE ANKLE.
