As employee pulled the bows back the rope broke and employee fell on backside. The employee complained of back pain and was taken to the hospital and diagnosed with a broken lower back.
EE was passing air tank to other EE on dredge and 1/4 inch piece of metal lodged into EE's left pinky finger, requiring ER to remove metal and 2 stitches.
At end of day, EE was exiting dozer, stepped off cab platform onto track, walked down to low end of track, stepped down on the c-frame, then stepped down to ground level. It was while stepping to ground level EE said EE had sharp pain in EE's left knee. Initial prognosis is torn meniscus, waiting on MRI to confirm.

Employee was using a wooden block to adjust a discharge pipe underneath a screen deck. Pipe came apart and employee's finger was wedged between the pipe and wooden block causing a laceration that required 8 stitches.
Employee was measuring the sheave sizes on the gearbox end of a Eagle 44" fine material washer. The guard that surrounds the sheaves had been open to obtain the measurements. As the wind blew the guard back to the closed position, the employee's left thumb was cut and received 10 stitches.
A right handed employee was using a pocket knife to cut the tail off of an electrical tie. As the knife cut the tie, the blade cut the wrist of the employee, requiring 4 stitches.
Office Manager was walking in the yard of the scale house to communicate with a truck driver and step on an unlevel area of the lot and spranined her ankle. She landed on her knee and received 4 stitches in her knee.
The miner was using a oxygen/acetyline torch to remove a bolt. The "slag" from the process ignited the right pants leg of the miner. The miner received second degree burns on his right leg.**Mgt. notified & miner decided Oct 16 to receive treatment. Miner was released to work Oct 26 with restrictions but did not return until Oct 19.**
The employee was assisting the plant manager in replacing bucket teeth on an excavator. The plant manager was using a hammer to remove the teeth when a piece became airborne and hit the employee below the left eye. The employee was standing approximately 10 feet away and was not wearing safety glasses. No stitches were required.
EE fell after slipping on ice after an ice storm. He struck his knee on the ground and caused a contusion. EE was performing startup operations on the plant when the injury occurred and was on the ground level.
Sediment pit over-filled due to pump failure. water ran over containment dam into dewatering area. Incident occurred on Sunday afternoon and no one was working in the area at the time.
THE INJURY TO EE'S FOREHEAD WAS A DIRECT RESULT OF LOW OXYGEN LEVELS IN THE BLOOD STREAM. HE RECEIVED A PACE MAKER THE FOLLOWING DAY TO CORRECT THE HEALTH RELATED PROBLEM. EE BEGAN TO FELL FAI NT WHILE OPERATING THE LOADER & HE ATTEMPTED TO EXIT THE CAB. WHILE CLIMBING DOWN THE STEPS, HE PASSED-OUT & STRUCK HIS FOREHEAD ON THE STEP. HE RECEIVED 10 STITCHES TO TREAT THE LACERATION.
THE INJURY OCCURRED WHILE WASHING MATERIAL FROM UNDER PLANT. HE WAS LEANING AGAINST 10" PIPE AND LOST HIS BALANCE AND FELL ON HAND.
