Two employees were welding wear parts onto an excavator bucket. The injured employee received flash burn to his eyes from the welding arc reflecting into the back of his welding hood. The employees had failed to recognize the hazard and did not place any kind of screen for protection.

Mostrando los 45While removing carriage bolts to replace cutting edges on bobcat skid loader employee used his finger to stop the bolt from turning while using an impact wrench to remove the nut. The carriage bolt had developed a razor share edge which lacerated the employees finger and required six sutures and bandages.
Employee removed a guard from a tail pulley area to replace a grease canister while the belt was in operation. The fins on the tail pulley caught the employee's glove, pulling his left hand into the fins, resulting in lacerations and abrasions; which required stitches and a skin graft.
A mechanic was holding a drill steel with the deck bushing raised up on the drill steel and suspended from the jib line. The jib line broke causing the deck bushing to slide down the drill steel striking the employee's hand. The employee's left thumb was pinched causing a laceration needing ten stitches.
Technician lifted cutting edge with bar to put crane sling under it. Cutting edge slipped crushed three fingers on technician's left hand.
While installing top pin on a 785 haul truck the bar holding the strut slipped out causing the strut to fall. The strut knocked the employee off the ladder and to the ground. The fall was 3 feet.
Employee was grinding on a horn in an impactor when a piece of debris got in his eye under his safety glasses.
Employee was riding on bus back to office at the end of his shift when bus hit a rough spot in road. The bumps caused the employee to jar his lower back. Employee felt a sharp pain in lower back but felt no medical attention was needed. On June 26th, Employee decided to visit doctor where he was examined and released to full duty.
Operator was driving haul truck down ramp when he hit a soft spot on the ramp. The soft spot caused the EE;'s seat to bottom out. This caused the EE pain in his lower back.
Employee was driving haul truck up an in pit ramp. The front of the truck lifted and returned to the ground violently injuring the employee's lower back. He was taken to the local hospital ER, examined, treated, and released to restricted duty. He is currently working with restrictions for an undetermined amount of time and is under Dr's care.
Empoyee parked haul truck inproperly causing ladder to be higher from ground. EE got off haul truck he felt a pull in his shoulder. He did not feel anything was wrong and did not want to see a dr. He went to dr. 12/12/06 for unrated reason & told dr about shoulder. X-rayed found nothing wrong. still bother him return to dr. if to going for surgery. 4/18/07 he went for medical
Employee was in the process to change out steel silica fines screen. The old screen was hung-up on a mounting bolt. Employee was using a hand shovel & was attempting to brake the old screen away from the bolt. His hand slipped on the shovel handle & his finger was caught between the shovel handle & the screen edge. His finger was cut, requiring sutures to close it at the E.R.
Contractor employee was walking along the perimeter fence line of mine property. While walking through the tall grass, employee stepped into animal burrow hole. Employee strained right hip muscle.
The employee reported to fellow workers that he was having problems & suspected he was having a heart attack. The project emergency response was activated with EMT's responding with rescue equipment and vehicle. The employee was transported to local emergency hospital, then on to advanced cardiac care in SLC.

The employee, maintenance electrican opened a remotely located electrical centrol panel. A wasp nest (flying insect) was disturbed. A wasp flew straight at the electrican's face, stinging him on left eye lid. Eye lid was swollen, employee was taken to local hospital ER for evaluation, first aid. Two days later employee received cortazone shot to counter facial swelling.
EMPLOYEE IS THE CRUSHER PLANT MECHANIC. HE WAS IN PROCESS OF ADJUSTING CRUSHER CONE WHEN HE CAUGHT RIGHT RING FINGER BETWEEN CYLINDER BODY AND ADJUSTING WEDGE. LACERATED FINGER TIP REQUIRING STITCHES TO FINGER TIP
EE WAS USING A CABLE COME-ALONG TO LIFT A TRANSMISSION. THE CABLE CRIMP (CLAMP) FAILED, ALLOWING THE LOAD AND CABLE TO SLIP. THE SUDDEN FAILURE CAUSED COME-ALONG FRAME TO STRIKE THE EE IN THE FACE, LACERATING CHIN. MEDICAL TREATMENT FOR LACERATION, SUTURES TO FACE AT CHIN. EE WAS RELEASED FOR FULL DUTY AT TIME OF TREATMENT.
Two employees were changing curtain liners in the impacter (crusher). They each lifted one end of the curtain liner. Ee said he strained something. He reported pain & swelling in the groin area on 26 May 2004. He was seen by dr. on 26 May 2004. There was no medical treatment or medication @ that time but surgery was planned to repair hernia. Ee was allowed to continue to work.
EE WAS OPERATING HITACHI BACKHOE, WORKING ON ORE FACE. A HYDRAULIC HOSE 1.5 ID. RUPTURED NEXT TO RIGHT SIDE OF OPERATOR'S CAB. RIGHT CAB WINDOW WAS SHATTERED WITH GLASS AND OIL STRIKING EMPLOYEE. EE RECEIVED BLAST TYPE INJURIES WITH LACERATIONS TO RIGHT SIDE SHOULDER, NECK & FACE. INJURIES WERE CLEANED, SUTURED AND DRESSED.
EE WAS MEMBER OF 'PM' CREW. EE WAS WASHING A D-10 DOZER W/ FIRE HOSE. HE WAS ATTEMPTING TO CLIMB ONTO THE DOZER RIPPER TABLE BY CLIMBING UP THE REAR DOZER TRACK. AS EE ASCENDED THE DOZER TRACK, HIS LEFT FOREHEAD STRUCK A SHARP CORNER, LACERATING HIS FOREHEAD WHICH REQUIRED SUTURES. EE, LATER THAT DAY, COMPLAINED OF WEAKNESS IN BOTH ARMS. DR. GAVE MEDS & RESTRICTED WORK.
EE WAS LIFTING TANK TO BE REPAIRED WITH OVERHEAD CRANE IN MINE REPAIR SHOP. AS TANK STARTED TO LIFT, IT MOVED ALONG SHOP FLOOR, BECAUSE RIGGING WAS NOT CENTERED CORRECTLY. EE'S RIGHT FOOT WAS STRUCK BY TANK. EE WENT TO DR FOR TREATMENT ON 19 DEC. 2003, FOR INFECTION TO GREAT TOE. EE RECEIVED RX FOR INFECTION & ANTI-INFLAMMATORY MEDS.
NOISE INDUCED HEARING LOSS AS PER REPEAT AUDIOGRAM THAT CONFIRMS 'STS' OF 17 SEPT. 2003. A CONTRIBUTING FACTOR IS THAT EE SHOOTS HIGH CALIBER WEAPONS ON A REGULAR BASIS AS A HOBBY.
EE WAS A MEMBER OF MAINTENANCE CREW, PERFORMING PREVENTIVE MAINTENANCE ON A PRODUCTION HAUL TRUCK. THE EE'S JOB WAS TO REMOVE MUD FROM THE UNDERSIDE OF TRUCK, WITH HIGH PRESSURE WATER HOSE. EE WAS CARRYING OUT HIS NORMAL DUTIES; WHEN HE EXITED, FROM UNDER TRUCK, HE NOTICED HIS LOWER BACK WAS IN PAIN. HE HAD TWISTED HIS LOWER BACK.
EE STATES HE HAD BEEN WELDING FOR SEVERAL SHIFTSWITHOUT PROPER VENTILATION OR RESPIRATOR. EXPERIENCED TIGHTNESS IN CHEST AND TROUBLE BREATHING. SEEN BY DR. AND PRESCRIBED MEDICATION FOR LUNG IRRITATION AND INFLAMATION.
CUTTING A HOSE, WITH A RETRACTABLE HOOK KNIFE - KNIFE SLIPPED AND CUT INTO UPPER PART OF LEG.
EE WAS REMOVING REAR DIFFERENTIAL PLUG. WHEN IT CAME LOOSE, THE BREAKER BAR CAME DOWN, HITTING EE UNDER LEFT EYE.
EE WAS GRINDING ON A METAL WHEN A PIECE OF METAL WENT INTO HIS RIGHT EYE.
EE WAS LOADING DRILL HOLES, HE POSITIONED THE AUGER ON THE DRILL TRUCK OVER HOLE. IT SWUNG BACK STRIKING EE RIGHT ABOVE THE LEFT KNEE. THE JOYSTICK TO THE AUGER CONTROL MECHANICALLY FAILED CAUSING THE ACCIDENT.
TWO MECHANICS WERE INSTALLING BEARING WHEN ONE STRUCK IT WITH A HAMMER. A PIECE OF METAL FLEW OFF STRIKING THE OTHER EMPLOYEE NEXT TO HIS NOSE. HE WAS RUSHED TO THE E.R. & METAL WAS REMOVED WI TH TWEEZERS. ON 1/31/02 HIS FACE BECAME BRUISED & SWOLLEN. HE WENT TO HIS DOCTOR WHO FOUND ANOTHER PIECE OF METAL. DOCTOR HAD TO MAKE INCISION TO REMOVE METAL; 5 STITCHES TO CLOSE INCISION.
EMPLOYEE WAS DOING EQUIPMENT WALK AROUND INSPECTION. HE LIFTED CHALK BLOCK ON HOLDING PIN TO SEE IF IT WAS FREE OF ICE. HE THEN ALLOWED CHALK BLOCK TO FALL BACK INTO NORMAL POSITION. EMPLOYEE' S FINGER WAS POSITIONED WHERE IT WAS CAUGHT BETWEEN CHALK BLOCK AND PIN.
EMPLOYEE WAS LOADING SALT INTO SALT SPREADER. LIFTING 50 LB BAGS OF SALT AND DUMPING INTO SPREADER. HE SAID HIS NECK STARTED HURTING A COUPLE OF HOURS LATER.
EE WAS HOOKING UP A WELDER IN A MECHANIC TRK. THE JACK FAILED CAUSING WELDER TO SLIP AND STRUCK EE IN THE RT. LOWER LEG.
EE WAS HELPING DRILLER CHANGE A DRILL BIT. THE CHAIN WRENCH BROKE AND A PIECE OF IT STRUCK EE IN THE CHIN.
EMPLOYEE WAS SHOVELING SPILLED MATERIAL FROM A PILE NEAR A CONVEYOR TAIL PULLEY. EMPLOYEE DID NOT TURE ENTIRE BODY WHILE SHOVELING AND NOTICEDA SLIGHT DISCOMFORT IN LOWER BACK.
EE WAS USING A HAMMER CRIMPING TOOL TO CRIMP LEAD ONTO BATTERY CABLE. EE MISSED THE END OF THE TOOL AND HIT FINGER.
EE STEPPED OFF OF DOZER ARM WHILE DISMOUTING CAT D9 DOZER EE STEPPED ON ROCK AND TWISTED RIGHT ANKLE RESULTING IN A 4 CLASS 4 SPRAIN.
EE WAS LIFTING GRATING IN PREPARATION TO RIG UP CRANE. EE PULLED RIGHT SHOULDER
EMPLOYEE WAS DISMOUNTING DOZER AFTER CHECKING COOLANT LEVEL HE SLIPPED ON THE DOZER TRACK AND HIS LEG WENT BETWEEN THE TRACK AND FRAME OF DOZER, TWISTING HIS KNEE.
EMPLOYEE DROVE THROUGH DUSTY AREA ON HAUL ROAD. EMPLOYEE SWERVED AND PULLED MUSCLE ALONG RIBS.
EE REPORTED THAT ON THE WEEKEND AT HOME HE HAD INJURED HIS KNEE. LATER HIS DR. SAID THAT IT WAS WORK RELATED. WORKERS COMPENSATION HAS DETERMINED THAT IT WAS NOT WORK RELATED AND WE ARE REPORT ING THIS TO SATISFY LEGAL REQUIREMENTS.
EE WAS HAMMERING WHILE REPLACE SCREEN CLOTH AND HIT RIGHT ELBOW ON SCREEN FRAME.
EMPLOYEE RECEIVED MINOR BURN TO EYE, WHEN HE WASREPAIRING FRONT STRUT ON HAULAGE TRUCK. ANOTHEREMPLOYEE OVERFILLED A BATTERY ON THE SAME TRUCK AND WATER WITH POSSIBLY SOME ACID SPLASHED INTO THE EYE OF THE INJURED.
EMPLOYEE WAS INSPECTING AIR LINE. HE LIFTED ENGINE HOOD ON CAT 777C HAUL TRUCK TO INSPECT LINES.WHILE HE WAS BENT OVER WITH UPPER BODY EXTENDED INTO ENGINE COMPARTMENT THE ENGINE HOOD FELL STRIKING EMPLOYEE ACROSS UPPER BACK AND SHOULDERS. EMPLOYEE SUFFERRED BRUISES TO SHOULDERS, NECK, BACK.
EE LIFTED WHEEL CHOCKS FROM BEHIND TIRE OF HAUL TRUCK WITH RIGHT HAND. EE FELT A PULLING TEAR IN HIS ARM ABOVE THE ELBOW.
