The miner was working on removing a bearing from a motor mount assembly. A small pry bar was being used to remove the bearing. The pry bar was under load when the miner struck the shaft with a hammer. The pry bar released and struck the miner in right cheek near the nose. The impact caused a laceration which required stitches and CT scans.

Mostrando los 12The miner was working to repair a piece of metal by beating it flat with a hammer. While striking the metal, the hammer missed its target and the miner's right index finger was struck by the hammer. The miner was wearing gloves and all other required PPE at the time. The strike made a laceration in the finger that required 5 stitches at the local emergency room.
EE was using a utility knife to trim a hole in a piece of rubber belting. The EE was attempting to widen the hole to hang the rubber, when the knife slipped, cutting the EE's hand causing a laceration to their left thumb.
EE and crusher foreman were doing maintenance on crusher CR-27. They were changing blow bars on the crusher head when the wrench that EE was holding slipped off the bolt and EE pinched the tip of finger, cutting the finger. EE was wearing gloves. EE was taken to emergency room for stitches. EE is on restricted duty and will follow up with doctor.
Employee doing crusher maintenance work, fabricating rubber belt cover when he took a box cutter knife to cut rubber, kneeling on 3'x4' rubber knife slipped and he cut his leg. Supervisor took him to clinic where they cleaned wound and applied sutures. When back to work with no restrictions.
Parked the haul truck end of shift, climbed out proceeding down 3 point ladder. Stepped off ladder slipped on wet surface landing on her left elbow and twisted her right ankle. Time of fall she did not notice any pain. She reported to supervisor at 7:00PM of fall. She has been working everyday. Now has ankle swelling and a bruise elbow. She was sent to doctor 04/30/15 Thursday.
Operator changing teeth on loader bucket. Hammer glanced off the segment striking the edge of the tooth. Teeth fragments came off, hitting him in the left forearm. This produced several lacerations, and foreign body retention. Taken to hospital to remove the metalic fragment, and suture of lacerations. Was returned to his normal duties the same day.
Operated excavator in rough terrain and aggravated arthritic condition in neck.
Employee stepped off of the ladders of a front-end loader and felt pain in right hip similar to a cramp. Pain did not susbside as a cramp would. Extreme pain after shift was reported to supervisor and medical attention was approved. No problems were observed with the ladder, seat or loader.
NO EVENT TOOK PLACE. THE EMPLOYEE HAD SORE TESTICLES AND CHOSE TO GO TO HIS OWN DOCTOR. THE DOCTOR SAID IT WAS "OFF THE JOB". HE FOLLOWED UP WITH THE SAME CLINIC AND A DIFFERENT DOCTOR SAID IT WAS "ON THE JOB". WE ARE SENDING HIM FOR AN INDEPENDENT UROLOGIST OPINION. EMPLOYEE WAS RUNNING A PIECE OF EQUIPMENT WITH AN AIR SEAT.
EE WAS CLEANING A TRUCK WINDSHIELD AND HE FELL HE TURNED HIS ANKLE WHEN HE LANDED. THE LACERATION WAS AT THE CONCRTE PLANT.
THE WIND WAS BLOWING TIN PANELS OFF OF THE SHOP DOOR ISECURED ONE PIECE UP AND WAS IN THE PROCESS OF SECURING ANOTHER PIECE, WHEN THE WIND GUST HIT THE PIECE AND TORE IT OFF HITTING ME ON THE SIDE OF THE HEAD CUTTING MY EAR.
