EE was cleaning shot in pit when he heard a crackling noise and noticed rock falling from high wall. One of the rocks struck the ledge of wall and exploded, the exploded rock was no bigger than the inside of a hard hat. EE was about 30 foot away and began running to avoid being struck but one rock did hit him from behind on his back right side-mid/upper shoulder.
Employee was very quickly heading towards the dumpster located near the shop right before quitting time. He was rapidly walking/jogging and while doing so his knee gave out. When his knee gave out he fell onto the rock and cut his knee open. He immediately turned pale and felt nauseas so he was taken to the emergency room where he received stitches.
Miner was assisting in the replacement of conveyor belt- was holding a guard while fellow miner removed it. He planned to let the guard drop straight down but it was angled in towards the conveyor and it pinched his hand between the guard and the conveyor. It smashed his pinky and lacerated (1-in.) his index finger on the right hand.
Miner was switching out screens in Deister screen plant. He was moving a 50lb screen up a 12% incline when he stumbled, rushed to put the screen down so he wouldn't fall on it, and felt a "pinch" on the middle/rt. side of his back, although there was no pain at that time. Accident was reported to his supervisor 2 hrs later, after stabbing pain developed while in wheel loader.
Miner at crusher asked excavator operator to bring breaker over, due to hung rock. Miner motioned with hands for operator to swing breaker to left. When operator did, miner reports that he was hit in chest with breaker. Miner was bent back over crusher handrail. No witnesses to breaker hitting miner, although the operator saw miner bent back over catwalk handrail.
Quarry truck was at the stockpile on the far east end of the property starting in forward motion, when the customer truck pulled in the lot on the opposite side of the stockpiles. Both trucks were traveling the same direction when the customer truck turned to the right, into the path of the truck. Quarry truck swerved to the right when customer truck hit back tire of truck.
Employee was trying to flip gates on the screen deck when he lost his balance and jumped from the deck 10-12ft landing on his feet.
EE was traveling down ramp, he was unable to maintain control and sideswiped the highwall at the bottom of the ramp.
Adjusting a bolt on the Crusher when he stumbled on the platform and broke his left foot.
