Operator drove the excavator into a sump hole. He was in an area that he should not have been. The operator was trapped inside the cab for approxomately 1.5 hours. The operator was extracted and had no injuries.

EE failed to adjust seat spring tension on seat at beginning of shift to except his weight 240 lbs when truck crossed rough road the seat bottomed out causing EE to hit very hard on floor causing injury.
CURRENT WORK CONDITIONS HAVE AGGRAVATED PRIOR BACK INJURY. OPERATIN BACKHOE AT SHEPPARDS WHERE THERE IS A HARD LAYER OF SHELL ROCK OVER DIRT HE IS DIGGING.
DRILLER AND DRILL HELPER WERE DRILLING FOR CLAY. DRILL HELPER COUPLED 1 1/4" HIGH PRESSURE WATERHOSE TO CORE BARRELL TO DISCHARGE CLAY SAMPLE. WHEN COUPLED, PRESSURE WAS APPLIED TO THE HOSE CA USING HOSE TO COME LOOSE AND STRIKING DRILL HELPER ON RIGHT SIDE OF FACE CAUSING LACERATION REQUIRING TWENTY TWO STITCHES.
