Employee states the accident was on July 22, 2019. However, EE did not miss any work until 9/15/2020. We were told to make a report once EE missed work. EE stated EE was removing the battery off the Cone Crusher and pulled EE's shoulder.

EMPLOYEE WAS ADJUSTING A BELT WITH A WRENCH. THE WRENCH SLIPPED AND CUT HIS FINGERS. WAS NEVER OFF WORK.
THE EE WAS HOLDING A BOLT WITH HIS LEFT HAND AND ACCIDENTLY CUT HIS LEFT HAND WHILE REMOVING THE BOLT WITH THE AIR DRILL.
EMPLOYEE WAS TRYING TO LOOSEN A NUT BY PULLING HARD ON THE PIPE WRENCH. AS HE DID THIS, HIS RIGHT HAND SLIPPED OFF THE WRENCH AND SWUNG BACK, STRIKING A PIECE OF STEEL BELOW HIM.
EMPLOYEE WAS DOING ROUTINE MAINTENANCE IN THE CRUSHING AREA. ROCKS WERE OVERHEAD. WHEN THE EMPLOYEE WAS POUNDING IN THE AREA, THE VIBRATION CAUSED THE ROCKS TO "WALK" TO THE EDGE AND FALL OVER .
