EE was helping to install a tilt cylinder on a Komatsu loader. As he was trying to align the cylinder with the mounting bracket it was moved by the boom truck operator and he stated he was jolted, causing pain in his lower back. EE continued to work until 3-22-11.
At approximately 12:30 am several large pieces of the high wall dislodged due to the recent freezing and thawing. The rock struck the top of the belt entry canopy,crushing it and damaging the belt structure under it. A plan to remove the damaged canopy and belt structure was put in place and approved by MSHA and the OMSL. There were no injuries in conjunction with this incident.

A non-injury roof fall accident has occured in the no.1 entry of the left return on panel 32 off the 4th northwest mains. The fall wa discovered during the weekly exam of the left return airway. The fall is located approximately 50 feet inby survey station no,1098 in the intersection. the fall is approximately 40 feet in length and approximately 6 feet thick.
EE walked behind the continuous miner to check his centerline. While walking back to his position to load another sc, he tripped over the miners waterline and fell on his left knee. He finished his shift, and worked the remainder of the week, but his knee continued to bother him. Went to his doctor 10-25-2010 and was diagnosed with a sprained meniscus, taken off work for a week.
