EMPLOYEE CALLED THE MINE ONWEDNESDAY EVENING STATING THAT HE HURT HIS BACK IN #6 ENTRY IN THEFACE ON 09-05-095 WHILE MOVING MINER CABLE OUT OF THE ROAD
SHUTTLE CAAR OPERATOR WAS TRAVELIN THE #4 ENTRY WHIEL PASSING THROUGH THE CHECK CURTAIN LOCATED OUTBY TH ELAT OPEN CROSSCUT. THE CURTAIN CUT THE OPERATOR UNDER THE LEFT EYE. IT APPEARED TO B E ONLY A SLGIHT SCRATCH
MINE OPERATOR WAS TRAMMIGN MINE RTO FACE WHEN HE WAS STRUCK ON LEG AND HIP WITH THE RETRAINING CLAMP AND MIEN CABLE.
WHILE BENDING ROOF BOLT HAND SLIPPED, CAUSING BOLT TO CUT FINGER.
EMPLOYEE WAS OPERATING A SCOOP WHEN THE WHEEL OF THE SCOOP TRAMMED OVER A ROCK IN THE ROADWAY. THIS ACTION CAUSED THE OPERATOR TO BE THROWN UPWARD WHICH INJURED HIS LOWER BACK. HE EXPERIENCED PAIN IN THE LEFT LEG WITH BURNING SENSATION.
VICTIM COMPLAINED OF SHOULDER & CHEST PAINS CO-WORKER BROUGHT VICTIM TO SURFACE & OPEN VICTIMS REQUEST TRANSPORTED HIM TO HOSPITAL
FALL IS ADJOINING PREVIOUS FALL IN AN AREA THAT WAS NOT USED. AREA HAD BEEN DANGERED OFF PRIOR TO OCCURENCE OF FALL.
HE HA DFOR SOME TIEM EXPERIENCED SWELLING IN HIS LEFT KNEE. MEDICAL EVALUATION SHOWS A CYST IN THE JOINT, REQUIRING ON-GOING MEDICAL TREATMENT, I.E. DRAINING WIHT A NEEDLE. THE DR ATTRIBUTE S THIS CONDITION TO WORKING ON HIS KNEES IN LOW COAL OVER A PERIOD OF YEARS.

ADDITIONAL SUPPORT WAS PUT THERE AND IN AJACENT AREAS.
4" RYDER SEAM CAME DOWN TO WITHIN 4' OF COAL SEAM.
ROOF FALL OUTBY FACE 50' FROM FACE, 30' LONG, 16' WIDE, 0-6' THICK, SPAD NO. 4733.
EMPLOYEE WAS OPERATING THE FLETCHER SINGLE HEAD ROOF BOLTER IN #6 ENTRY WHEN A HYDRAULIC HOSE RUPTURED SPRAYING HYDRAULIC OIL OVER HIS FACE AND IN HIS EYES, CAUSING IRRITATION & BURNING TO THE EYES AND FACE. THE HOT OIL DID BLISTER A SPOT OVER THE LEFT EYE BROW. HE RECEIVED PROMPT MEDICAL ATTENTION FROM EMT AND FINISHED HIS SHIFT.
SHOVELING ON BELTLINE BOTTOM BELT CAUGHT SHOVEL CARRYING BACK THROUGH TAILPIECE BETWEEN BOTTOM AND BOTTOM BELT. SHOVELBLADE WAS STICKING OUT FROM UNDER TAILPIECE GUARD. STARTED TO REACH FOR SHOVEL SPLICE CAM THROUGH & KNOCKED SHOVEL BLADEINTO RIGHT CHEEK.
HE WAS PLASTERING BRATTICES & MOVED THE BUCKET OF B-BOND SEALANT TO ONE SIDE & THE LIQUID SPLASHED INTO HIS LEFT EYE.
EE WAS GETTING OUT OF TRAM AT END OF SHIFT WHEN TRAM STARTED TO ROLL AFTER LOCKING BRAKE WAS SET THE FOREMAN OPERATING TRAM THEN APPLIED THE FOOT BRAKE & THE STOPPING ACTION CAUSED EE TO FALL AGAINST A RAISED METAL DOOR AT THE DRIFT MOLUTH CAUSING PAIN TO HIS BACK

A SECTION OF ROOF APPROX. 40' LONG BY 6' TO 10' THICK AND 16' WIDE, FELL IN INTERSECTION INBY SPAD #2385 IN THE RETURN AIR COURSE, #4 ENTRY, ON #3 BELT AT 2ND WEST MAINS.
A SECTION OF ROOF APPROX 35' LONG BY 6 TO 8 FT. THICK & 16' WIDE FELL ON BELT & TRACK IN #3 BELT ENTRY OF THE 2ND WEST MAINS. THE FALL WAS BETWEEN SPADS #2331 & #2337 & IS CONTIGUOUS TO AN EA RLIER FALL IN THE SAME AREA. THE FALL WAS ALONGTHE LENGTH OF THE ENTRY CROSSCUT WAS NOT INVOLVED.
COUPLING UP RAM CAR TO 270 TRACTOR. CAR BEGAN TO ROLL BACKWARDS HE GRABBED IT WITH HIS HAND TO TRY TO KEEP IT FROM ROLLING PULLED HIS LEFT SHOULDER OUT OF SOCKET.
HE WAS LOADING CRIB BLOCKS ONTO CART TO BLOCK UP TRACTOR FOR REPAIR. WHILE LIFTING CRIB BLOCK AND TURNING, HE SUFFERED SEVER PAIN IN LOWER BACK AND NUMBNESS IN HIS LEG.
THIS REPORT IS BEING REPORTED PURSANT TO MSHA CITATION 4005230. FOLLOWING ORDER OF AMINSTRATIVE LAW JUDGE ON ABOVE DATE CO DENIGS TO CLAIM. SAYS HE INJURED KNEE WHILE MOVING BELT.
HE SAYS HE WAS FLOATING OUT THE 270 TRACTOR OPERATOR DURING LUNCH; HE HIT A ROUGH PLACE, AND WAS BOUNCED AGAINST AN OVERHANG IN THE MINE ROOF. HE FELT PAIN AND STIFFNESS IN HIS NECK.
HE WAS ATTEMPTING TO REMOVE ROCK FROM TAILPIECE PULLING/TWISTING MOTION CAUSED SEVERE PAIN IN THE LOWER BACK
HE WAS OPERATING THE 270 TRACTOR TOO CLOSE TO THE COAL RIB AND WAS CAUGHT BETWEEN THE RIB AND THE MACHINE.THEY WERE MOVING THE SECTION TO START LEFT PANEL OFF MAINS.
HE WAS SPLCING BATTERY CHARGER CABLE. WHEN HE STRUCK THE CRIMPING TOOL, A PIECE OF METAL BROKE OFF THE CRIMPING TOOL, AND LODGED IN THE FLESH BETWEEN THUMB & FIRST FINGER OF LEFT HAND.
AT BEGINNING OF SHIFT HE WAS OPERATING 270 TRACTOR HAULING COAL. HE HIT BUMP IN ROAD WHICH JARRED HIM CAUSING HIM TO FEEL SHARP PAIN IN BACK. HE WENT FROM THERE TO REGULAR JOB AS ROOF BOLTER. PAIN BECAME INCREASING WORSE. FINALLY WHEN HE COULD NOT TOLLERATE IT ANY LONGER HE HAD TO BE TAKEN OUT OF MINE & TO HOSPITAL.
A DEFECTIVE AREA OF ROOF GAVE WAY APPROX.30FT LONG BY 50FT WIDE BY 8FT TO 10FT THICH APPROVED ROOF CONTROL PLAN WAS BEING FOLLOWED & ADDITIONAL SUPPORTS HAD BEEN USED IN THIS AREA
HE WAS INSTALLING 3/4 X 48 REBAR BOLTS IN #001 SECTION. WHILE BENDING A BOLT PRIOR TO INSTALLATION, HE FELT SEVERE PAIN IN HIS LOWER BACK.
HE WAS INSTALLING 3/4 X 48 REBUR BOLTS IN 001 SECTION. WHILE BENDING BOLT TO PUSH IT UP INTO THE HOLE, HE FELT A TEARING IN HIS ABODOMEN.
HE WAS HELPING TO CHANGE OUT BELTHEAD #4. HE HEARD A POPPING SOUND IN HIS RIGHT KNEE, AND FELT PAIN. HE WAS ABLE TO FINISH OUT THE SHIFT AND WORKED 4 HOURS THE FOLLOWING DAY. MEDICAL REPORT SH OWS A SPRAIN OF THE RIGHT KNEE.

HE WAS CHANGING BITS ON THE 14-4 JOY MINER. WHILE TURNING THE RIPPER DRUM, HE FELT A SHARP PAIN IN LOWER LEFT ABDOMEN. CONTINUED TO WORK THROUGH 1-29-88. SURGERY 2-6-88.
HE WAS HELPING 8ET BITS ON THE MINER. HE TURNED TO REACH FOR THE BIT WRENCH & FELT HIS KNEE POP & FELT PAIN IN THE KNEE JOINT.
HE WAS HELPING 7HE REPAIRMAN WELD BRACKETS ON THE CUTTER BOOM OF THE 14-4CM JOY MINER HE WAS USING A GRINDER TO PREPARE THE METAL SURFACE FOR WELDING, WHEN T HE GRINDER KICKED BACK, CUTTING HI M BEHIND THE LE0T EAR.
THE EMPLOYEE WA8 WORKING AT HIS REGULAR JOB AS ROOF BOLTER HELPER. WHILE BENDING BOLT, HE SUFFED SHARP PAIN IN HIS LOWER BACK.
BELT DRIVE ROLL9R ON #2 BELT HEAD HAD BROKEN AND HE WAS HELPING INSTALL NEW ROLLER.LIFTING NEW ROLLER INTO PLACE FELT SOMETHING GIVE IN HIS BAK. PAIN BECAME PROGRESSIVELY WORSE DURING SHIFT.WO RKED 3-3 BUT WA8 IN SO MUCH PAIN TUES HE WAS UNABLE TO WORK.
HE STATES THAT HE WAS DOING HIS REGULAR JOB OPERATING 270S&S TRACTOR HE HIT A BUMP OR HOLE IN HAULWAY AND WAS BOUNCED OFF DECK OF TRACTOR WHEN HE LANDED ON DECK SOMETHING IN HIS BACK POPPED AN D HE EXPERIENCED SEVERE PAIN HE HAS A HISTORY OF PREVIOU BACK INJURY AND STATED THAT HIS BACK HAD BEEN HURTING HIM FOR ABOUT A MONTH
HE WAS HAULING COAL WITH THE 270 SS TRACTOR & RAMCAR, IT WAS VERY ROUGH RIDING AND HE WAS BOUNCED ABOUT QUITE A BIT AT DINNER TIME, WHEN HE HOT OFF THE TRACTOR HIS LEGS FELL OUT FROM UNDER HIM AND HE EXPERINECED NUMBNESS IN HIS MIDSECTION.
THE MINE HAD BEEN IDLE SINCE ROOF FALLS OF 020884 THIS FALL WAS DISCOVERED BY FOREMAN FREDDIE RUNYON WHILE FIREBOSSING PRIOR TO RESUMPTION OF OPERATIONS IN ANOTHER AREA OF THE MINE APPARENT CO NTRIBUTING FACTOR COAL STREAKS ABOVE THE ROOF BOLT ANCHOR ZONE THE APPROVED ROOF CONTROL PLAN HAD BEEN COMPLIED WITH IN THIS AREA
