THE EMPLOYEE'SGRIP WAS OVERCOME WHEN THE WEIGHT OF HTE ROTOR 107 LBS 2 WAS TRANSFERRED FROM THE MOUNTING SHAFT. THE ROTOR SUPRESSED THE FIRST KNUCKLE OF THE EMPLOYEE'S MIDDLE FINGER BETWEEN THE ROTOR AND THE TIRE VALVE STEM MOUNTING BRACKET.

EMPLOYEE WAS RETURNING TO THE CAB OF THE DRILL FROM THE DRILL DECK AND ATTEMPTED TO CLOSE THE SLIDING DOOR. THE DOOR STUCK IN THE TRACK REQURING A HARD PUSH ON THE LATCH MECHANISM WITH BOTH HA NDS. THE DOOR SLID SUDDENLY RESULTING IN THE EMPLOYEE'S LEFT LITTLE FINGER BEING CAUGHT BETWEEN THE LATCH MECHANISM AND THE DOOR LATCH MOUNTED ON THE DOOR FRAME.
EMPL WAS DRIVING STEEL PIN BACK INTO BUSHING WHERE SHOVEL STICK ATTACHES TO SHOVEL BUCKET. PIN RETAINING RING HD BROKEN ALLOWING PIN TO BACK OUT PART WAY. AS EMPLOYEE WAS DRIVING PIN WITH SLED GE HAMMER PIECE FROM PIN STEEL SPLINTER FROM FLANG CHIPPED OFF & FLEW THROUGH CLOTHERING & INTO EMPLS LEG.
EMPLOYEE WAS REMOVING 480-VOLT ELECTRICAL LEADS FROM ONE OF CONTACTORS MOUNTED ON A PANEL INSIDE A ELECTRIC HEATING UNIT.AS THE PHASE LEADS WERE LOOSENED AND PULLED FREE,TWO LEADS TOUCHED RESU LTING IN AN ELECTRICAL ARC WHICH BRUISED THE EYES OF THE EMPLOYEE.THE EMPLOYEE HAD FAILED TO FOLLOW LOCKOUT PROCEDURES.
HYDRAULIC BRAKE LINE RUPTURES SPRAYING EXHAUST MANIFOLD & TURBO CHARGER AREA OF HAUL TRUCK ENGINE. HYDRAULIC OIL CAUGHT FIRE & HEAT CAUSED OTHER HOSES TO FAIL WHICH FURTHER FUELED THE FIRE FR OM STEERING OIL ACCUMULATOR. FIRE DAMAGED ENGINE & DESTROYED ELECTRICAL & HYDRAULIC SYSTEM ON TRUCK & CAB.
EMPLOYEE WAS REMOVING SMOLDERING OPEN GEAR LUBRICANT FROM THE INTERIOR OF THE SHOVEL HOUSE ROLLERS BY REACHING THROUGH AN OPENING IN THE FRAME OF THE SHOVEL. THE CUTTING OF A BOLT WITH A TORC H CAUSED THE SMOLDERING. IN THE PROCESS, HIS HAND CAME CLOSE TO THE HIGH VOLTAGE COLLECTOR RING AND HE DREW AN ARC TO HIS LEFT HAND FROM THE RING. BURNS RESULTED TO HIS HAND AND ARM.
I NAME WAS CLIM6ING STEPS UP THE FRONT OF THE REAR TRAILER. HE HAD A SHOVEL IN HIS HAND. AS HE REACHED THE THIRD STEP HE SLIPPED FALLING BACK WARDS HITTING ON THE TRAILER TOUNQE CAUSING A LOWE R BACK STRAIN. 3

AS HE APPLIED POWER TO THE WRENCH, THE SIDE HAN DLE STRUCK HIS 0OREARM CAUSING HIM TO LOOSE HIS GRIP ON THE CLAMP. THE RAPID ADVANCE OF THE LUG NUT BY AIR WRENCH RESULTED IN THE EMPLOYEES THUMB BEING PINCHED BETWEEN THE LUG NUT AND CLAMP.

EMPL EMPTYING 55 GAL TRASH DRUM INTO DUMPSTER.EMPL PLACED LEFG HAND ON EDGE OF DUMPSTER AND ROTATED DRUM HELD IN DRUM HANDLER MOUNTED ON FORKLIFT TO DUMP CONTENTS WITH RIGHT HAND.EMPL RELEASED DRUM -ROTATED BACK TO UPRIGHT POSITION BOTTOM EDGE OF DRUM CAUGHT EMPL FINGER AND CRUSHED AGAINST DUMPSTER.

THE EMPLOYEE WAS ATTEMPTING TO REMOVE A FAULTY DOZER CONTROL VALVE BY STANDING ON THE TRACK OF THE FIAT-ALLIS HD31 (RIGHT SIDE) AND REACHING INTO A COMPARTMENT WHERE THE VALVE WAS LOCATED WITH WITH A COMBINATION WRENCH (BOX END)THE WRENCH SLIPPED OFF THE NUT SECURING THE VALVE CAUSING THE EMPLOYEE TO LOSE BALANCE AND FALL TO FLOOR.
THE EMPLOYEE WAS INSTALLING HANDRAILS ON A NEW DECK WHICH HAD BEEN INSTALLED ADJACENT TO AND AT THE SAME LEVEL AS AN EXISTING CATWALK. HE CLIMBED OVER THE EXISTING HANDRAIL TO GAIN ACCESS TO H IS WORK AREA AND FELL TO THE GRATING WHEN HE ATTEMPTED TO USE A LOOSE UNWELDED RAILING AS A HANDHOLD. A 4-STITCH AVULSION RESULTED.
