Employee was cutting a water with a utility knife and the knife slipped and cut his right forearm.

Strain to the left arm and neck.

Employee was using a hammer to hit the metal bucket of the dozer, he was replacing the dozers bucket tip and a piece of metal chipped off and hit his left forearm.

Employee was steaming cleaning a drill, stepped onto a ladder and slipped and fell twisting his left knee.

He was stepping out of his service truck heard pop under right foot

Employee was working on a piece of equipment and a port power bolt broke and came back and hit his hand.

Coupling hose from an emulsion truck tanker was being detached, the driver did not allow time for the air to release from the hose when he uncoupled the connection thus being under pressure the coupling struck the driver in the side of the head, as a result the driver sustained a concussion.

Employee was climbing off of a dozer and slipped on the track and grabbed a handle to keep from falling and pulled his left shoulder.

Employee had just finished setting a water pump and was walking up a hill and felt something in his knee popped.

Employee was loading and unloading water pumps and hoses and pulled the muscle in his lower back.

Employee was shoveling coal from conveyer belt and slipped hitting the crusher, resulting in a contusion to the left chest wall.

Contract Mechanic was changing the track on a D-11 dozer and the track flipped over mashing his left index finger.

While loading blasting agent onto the blast hole the blasting agent caught fire, detonated and caused the detonation of additional primers that had been placed around the blast hole area. Fire was caused by Blaster smoking near blast hole. See attached MSHA Citation #8521062.

Employee's foot slipped as he was climbing off of the loader causing him to fall, hitting his right elbow.

Employee was putting a roller on a D10R when the jack slipped letting the roller fall and smash his left ring finger.

Fire to a D10 N Bulldozer. No (person) was injured or involved in the fire to the bulldozer.

Employee was backing over a rock with the bulldozer and the dozer caught on the rock pulling, jerked the emplyee pulling his left shoulder.

Employee was pulling a pump hose and the hose slipped out of his hand pulling his right shoulder.

Employee was operating a rock truck when the wheel broke off the right rear axle causing the rear of the truck to hit the ground.

Employee was working on the door of a D10 dozer, he removed the door & it slipped pinching his finger, he jerked his hand out from under the door cutting his right pinky finger.

Employee was preparing to weld a C-clamp, he sat the clap on a table to measure it and the clamp slipped mashing his right finger.

Employee was sliding a 4x8 screen and lifted up on it and pulled a muscle in his chest wall.

The injuried was using a grinder to smooth rough edges of a shaft and several small pieces of metal came off and flew under his safety glasses and got lodged under his right eye lid.

He was driving an equalizer pin into an equalizer bar; a piece of the pin chipped off and went into the thumb on his left hand. He had to have the chipped piece removed from his finger.

Rock truck operator backed 100 ton rock truck off dump, over-turned rock truck, was flown to University of Alabama Hospital where he was treated and released with a hairline crack in L1 vertebrate.

Employee was lifting 5 gallon can of antifreeze.

EE was under the D-10 Dozer welding. Bogie arm strap came loose from the side of the machine causing the bogie arm to swing in a downward position hitting him on the head. Head was cut.

EMPLOYEE SHUT THE DOOR OF THE DRILL ON HIS FINGER & MASHED HIS FINGER. LOST THE FINGERNAIL.

He was pulling on dragline flywheel and pulled a muscle in his left shoulder.

Employee was running dozer, while ripping hard rock. The retainer holding the pin in the ripper tooth broke. He was unaware the tooth was hot; he attempted to drive the pin back into the ripper tooth and when his hand came in contact with the tooth, he burned his left hand.

He was shoveling dirt into blasting hole; strained disk in back and neck.

Climbing on dozer to fuel; slipped and fell to the ground.

HIT HEAD ON TOP OF CAB OF DOZER.

STEPPED ON A ROCK. SPRAINED & BRUISED ANKLE.

LEG WAS CAUGHT BETWEEN DRILL HEAD & TOP OF MAST

CABLE ON HEAD SWIVEL ON CORDRILL BROKE: BRAKES ON HEAD SWIVEL FELLED TO HOLD. EE FELL ON DRILL, BROKE LEFT LEG ABOVE ANKLE.

THE COME-A-LONG ON A JACK SLIPPED AND DROPPED A PLATE ON HIS FINGERS.

EMPLOYEE WAS INSTALLING A SEED GRADER ON A FARM TRACTOR. AS HE WAS CONNECTING A SHAFT, THE POWER TAKE OFF ON THE TRACTOR ENGAGED AND CAUGHT HIS GLOVE, TWISTING HIS RIGHT HAND AND ARM.

EE WAS ATTEMPTING TO HOLD A 3-1/4" NUT WITH A WRENCH WHEN THE NUT TURNED CUTTING HIS LEFT THUMB.

EE WAS ADJUSTING CONVEYOR BELT ON COAL CRUSHER USING A PAIR OF COMEALONGS TO PULL BELT TOGETHER. WHEN HE STARTED TO RELEASE THE COMEALONGS HIS HAND SLIPPED OFF THE HANDLE. THE HANDLE HIT EE AB OVE RT EYE REQUIRING 7 STITCHES.

HE WAS LOADING AN IDLER FROM THE P&H DRAGLINE ONTO A TRUCK; THE IDLER SLIPPED AND CAUGHT HIS FINGER UNDER IT.

HE WAS TRYING TO REMOVE A BEARING WITH A HAMMER AND CHISEL. HE MISSED THE CHISEL WITH THE HAMMER AND HIT HIS LEFT HAND.
