Employee was attempting to clean a return roller on conveyor under the HP 300 crusher, against company policy, without utilizing lock out tag out training. EE was using a bar to scrape mud off the return roller. The bar was caught between the roller and moving conveyor belt, pulling the EE's left arm into the pinch point and resulting in broken bones and a dislocation.

APPROX 30' OF RIB COAL LET LOOSE ON THE SLOPE PILLAR INBY SIDE BETWEEN SLOPE & COAL #1 CHUTE BLOCKING GONGWAY.

TWO MINERS WERE ASSIGNED WORK IN THE MONKEY HEADING OF THE 2ND LEVEL EAST GANGWAY WORKING. THEY APPARENTLY WERE PREPARING A HITCH FOR ROOF SUPPORT IN THE CYCLE OF MINING, AND AS THEY FRED THE HITCH, A POCKET OF METHANE GAS WAS IGNITED CAUSIING AN EXPLOSION TO OCCUR. THE RESUTLING EXPLOSION CAUSED FATAL INJURIES TO ONE AND SERIOUS INJURIES TO OTHER.
WORKING ON 75 DEGREE PITCH. WET CONDITIONS, FATIGUE. EMPLOYEE FELL APPROX. 100 FEET DOWN THE PITCH, SUBSTAINING BROKEN JAW, A LACERATION ON THE SCALP, COLLAPSED LUNG.
