While running a miner, a rock fell and struck his left leg.
While pillaring #11 entry, #7 lift, the roof fell in covering up the continuous miner with approx. 6' of rock. Cover of approx. 300'. Area bolted with 6' fully grouted resin bolts & header boards. Sub main sect. (19 belt). #11 heading outby spad #4409.
While removing a wheel unit from shuttle car, employee had a breaker bar on a wrench. He stood on the breaker bar which slipped off the wrench and struck him on the leg.
Injured twisted knee while running roof bolter. Normal work while bolting. Not sure how knee became injured. Right knee has ligiments torn. MRI scheduled 3/25/08
A roof fall occurred at #20 belt head on E-panel submains 1-North. Laminated shale rock fell from 6'-10'-12'. Area bolted with 6' resin bolts & header boards & 12' cable bolts with monster mats. Cover 415' sect. was moved outby affected area. Cribbed & dangered off. Called hotline.
Piece of rock approx 1 1/2 feet long 3in thick. Fell from between roof bolts in fully bolted area and hit injured in the back.
While injured was tramming bolter from one work place to the next work place, he put his arm outside the tram canopy deck area and trammed bolter into rib causing arm to get caught between rib and bolt machine.

x5-4-6 bld, along 19 felt on sub main-fell in approx. 6' high, 16 w x 30' l area bolted with 6' fully grouted resin bolts and 4' crib set in inter sent. along belt-top laminated 8 hole and had water in top. cribbed off and dangered off a will not be cleaned up. Called hotline-cover approx. 275.
N. Main set. #9 inter. 2 way-fell in approx. 6-12' high-16' l wide, 25' l. bolted with 6' fully grouted resin bolts and 6" x12" header boards. chen was spot bolted with 12' lb cable tenain bolts. Cover 300' mining on top of pillared pits seam. top laminated shale in the streaks of sandstone chea dangered off cribbed off will not be cleaned up called hot line 54pd.
A roof fall occurred in 4 way inter. Fall approx 5'-6'h, 17'w x 40'l. Area bolted with 6' resin bolts & 6" x 12" hole boards. Prior fall area was supported with 4' wooden cribs. Fall is at edge of pittsburgh piller & solid coal area. Called hotline. Over 240' top shale laminated. Sub main #3 1 brk 2blocks outby spad #419. Fall observed
Roof fall approx. 6' high, 12'L x 18'W. Area mined in 8-14-07. Cover approx. 350'. Area bolted with 6' resin bolts. Area dangered off & rebolted with 12' cable bolts & 6' resin bolts. All int. cribbed on 8-22. Ventilation not affected. No injuries.
While employee was driving a rubber tired 2-3 man ride, employee hit rib with front tire causing steering knob to strike right hand.
Using hydraulic tension equipment to put tension on cable type roof bolts, he twisted his back.

Injured person was operating 4 wheel rubber tired ride pulling a portable rock dusted while dusting #16 belt line, front tire hit rib making steering wheel jerk back in opposite direction hitting small finger on rt hand breaking finger and joint on rt hand (5th).
While traveling underground toward section, employees noticed top breaking and dripping and could not get stopped in time to avoid roof fall. Equip. operator stopped ride & started backing up when fall occurred, striking employee. #2 Main Intake 18 Bl., inby spad #34.
4 way Intersection bolted with 4' fully grouted resin bolts. Bolted in 1996. Fall approx. 6' high - 20' long - 17' wide. Cover of approx. 107'. Fall located inby spad #34, #2 main Intake #18 Bl. inter.
Injured was turning to walk away from roof bolter when he got his foot wedged between two rocks and twisted his right inside leg near knee.
On 2-South #4 hdg - 7 bl fall approx 6'-7' high x 17' w x 40'-60' in length. Cover approx 250' area bolted with 5' resin bolts. The hdg was mined between a full, partial pillared block & solid block of Pittsburgh coal causing roof condition problems in seam. Affected area has been rebolted with 10' & 12' cable bolts & 6' 3/4" resin bolts & boards. Hdg width narrowed down also.
While EE was installing a cable bit. His drill steel got stuck in the roof while trying to remove drill empty, he cut his finger between drill steel and bolt chuck.
While operating operator's side of bolter, drill head went up higher than the canopy and got wedged onto canopy. Employee used left hand to separate head and canopy causing hand laceration.
Roof fall in on Main dtake newtal hdg-just inby 3 head aber dev 1997 and bolted with 4' 3/4" resin bolts. Thsi area hdg has a roof fall 20-30' inby this fall which has fallen in has a60. we pooted rib prior to fall since fall has installed 10' cable bolt in this area fall approx. 20' & 17' wide cover 150'. fall is 4' high.
On 2rt. #1 Panel x7-8 fell in at 6 bldg-apporx. 6 1/2-7' high by 17' wide top was shale and laminated div on 12-2-05, pouted off and day will not be cleanely area bolter with 6'-3/5" fully grouted resin bolts-cover of 3500 fall was in area of bolt top ans posted off a d.o. before it fell.

POST WERE SET ALONG THE RIB ALSO SOME CEMENT PINS. 16'-17' WIDE AT APPROXIMATELY. DEVELOPED FEB 05. FALL IS POSTED OFF & D.O. & WILL NOT BE CLEANED UP. FALL IS OUTBY SPAD #2569.
While doing weekly exam located a roof fall at 92 bld old 1 rt. panel #4 bldg. which is common with 3 other bldg. 1 rt is not a active section. Fall approx. 6-9' high 16-17' wide x approx. 25-30' long cover of approx. 3000'. developed 10-04 area bolted with 6' resin bolt and 6'-9' install ii bolts. top gray laminated shale. fall will not be cleaned up.
EMPLOYEE STATED WHILE HE WAS SHOVELING #2 BELT, HIS LEG GOT STUCK IN MUD & HURT HIS KNEE.
While blocking up shuttle car to repair wheel unit, shuttle car was being picked up with scoop bucket. Shuttle car slid out of bucket while employee was installing cribs under shuttle car to block it up, smashing left thumb between car frame and crib blocks.
ON 2 RT #6 HDG. SPAD #2821, A ROOF FALL OCCURRED. APPROX 6' HIGH, 16'-17' WIDE & 20'-25' LONG. AREA BOLTED WITH 5/8" RESIN BOLTS, 6X6 PLATES & HOLE BOARDS. DEVELOPED APPROX APRIL '05. COVER OF 350'-400'. FALL IS POSTED OFF & DANGERED OFF & WILL NOT BE CLEANED UP. TOP WAS GRAY SHALE.
ON 2 RT SECT. AT 16 BL. #9 HDG. AT A 3-WAY INTER. APPROX. 7' HIGH X 18' LONG X 15 1/2' WIDE. TOP WAS GRAY SHALE. AREA BOLTED WITH 6' FULLY-GROUTED RESIN BOLTS, ALONG WITH 2" X 6" X 12" HOLE BOARDS. AREA DEVELOPED APPROX. 5/30/05. COVER OF 500' AREA WAS SUPPORTED WITH 4' WOOD CRIB INBY & OUTBY OF INTER. AREA IS APPROX. 800' OUTBY FACE. WILL NOT BE CLEANED UP.
EE moved miner from X 8-9 to #8 inter. He stopped the miner to hook up rope to miner cable handler to drag miner slack to face of #8. When he bent down to get rope, felt miner up against him, causing cable handler to puncture his R. thigh. EE was not tramming the miner. Miner was checked & left-side IVS changed. Also, small roll in bottom observed in #8 hdg.
While employee was on 2 right sect., #6 heading, 9 block a piece of slate 1" thick x 2' wide x 1 1/2' long fell from between 6' resin bolts with 2"T x 6"W X 12"L hole boards, striking employee on his right hand. Employee hasn't called mine to inform us of his injury. Company has tried several times to reach him.

DEVELOPED 2/05, 4WAY INTERSECTION OF #6 HEADING. FELL IN XCUT 5-6 & 6-7 ALSO FELL. #7 & #5 INTERSECTIONS HAD CEMENT PIERS BUILT IN THEM & NEVER FELL. FALL WAS APPROX 17'W X 4'H X 70'L.
ON OLD 1 RT PANEL (9 BELT) #3 INTER. SPAD #2541. FELL IN APPROX 6' HIGH, 16' W X 16' L IN A FOUR WAY INTERSECTION. AREA BOLTED WITH 6' RESIN BOLTS. COVER APPROX. 300'-350'. FALL WILL BE POSTED OFF & DANGERED OFF & WILL NOT BE CLEANED UP. AREA DEVELOPED 1-05.
On old 1 rt panel (9 belt) #4 by inter fell in, 1 block in by spad #2513. Area developed 12-04, bolted with 6' resin bolts and T-3 channel. it was a 4 way inter, cover approx 300-350'. Fall was approx 6' high 16' l 16' w. Fall will not be cleaned up. Posted and dangered off.
While installing belt structure on a belt move - Employee tripped over structure rail & fell on his rt. knee causing injury.
A fall occurred in the old works 35' in by spad #2429 to 50' out by spad #2467. Fall was approx 4-6' high and approx 16' wide, for the distance listed above. Cover approx 300-350'. Fall will not be cleaned up, it will be posted and dangered off.
In by spad #2517 1 rt old panel (9 belt) Top fell approx 6' high 60-70' length and 16' wide. Cover 350'. Area bolted with 6' resin bolts 8 by 8 plates and #12 had T-channels installed. Fall started 2' in by #11. Will not be cleaned up. Found in weekly exam. Posted and dangered off area.
WHILE EE WAS GOING THROUGH A MANDOOR, HE FELT PAIN IN THE LEFT SIDE OF HIS STOMACH. DIAGNOSED AS HERNIA - SURGICALLY REPAIRED ON 1/28/05.
Intersection 3-way fall in 18 block, #7 heading, 1 right section. Top gray laminated shale and sandstone. Area bolted with 6' resin bolts. Over 350' - 400'. Fall will not be cleaned up.
Intersection fell in at 19 block, #7 heading, 1 right section. Top gray laminated shale & little streaks of sandstone intermixed. Cover of 370' - 400'. 3-way intersection. Area bolted with 6' bolts & supplemented with 9' install II bolts. Fall will not be cleaned up. Moved sect. from this area.
Top fell for approx 30' in length, 6' high between 60-61 bl. on 1 rt belt. cover approx. 350' I called and reported this fall, but forgot to do the report.
INTERSECTION FELL IN, APPROX. 6-8'HIGH. TOP WAS GRAY LAMINATED SHALE. AREA BOLTED WITH 6'RESIN WITH 6 X 16 PLATES. CEMENT PIERS ALSO INSTALLED IN INTERSECTION TO NARROW WIDTH OF INTERSECTION T. 13"WIDE. COVER 300 - 350'HIGH, 16 - 16 1/2' WIDE. AREA MINED APPROX. 6/22/04.
