AFTER EVERYTHING WAS BOLTED, TWO WORKERS WENT BACK TO INSTALL BELT HANGERS. THEY SET THE BOLTER UP TO INSTALL THE TIGHT SIDE BELT HANGER - WHILE CO-WORKER WAS INSTALLING THE HANGER, EE WENT IN BETWEEN THE RIGHT SIDE DRILL BOOM AND THE "T" BAR. WHILE LOOKING FOR THE NEXT BELT HANGER MARK - HE PUT HIS HAND ON THE DRILL BOOM. HIS 2 FINGERS WERE CAUGHT 2 MOVING PARTS & CUT OFF.

A ROOF FALL OCCURRED IN THE INTERSECTION JUST INBY SPAD 1551 OF THE 6 LEFT SECTION: NO VISIABLE SIGNS OF CUTTERS OR CLAY BEINS. AREA WAS BOLTED WITH 42" #6 REBAR TENSION REBAR BOLTS. INITIAL C AVE WAS APX 5'HIGH, 18'WIDE, 18'LONG. AT 6:30 AM CAVE EXTENDED OUTBY APX 100' - 18'WIDE, 7'HIGH.
WHILE MAKING THE SUNDAY MINE RUN EE FOUND THE CAVE IN 1 WEST SECTION 42+10 IT CAVED FROM #2 TO #7 APX 8' OF ROCK FELL ENTRY WIDTH 19 TO 20' 42" TENSION REBAR BOLTS - 6 ACROSS - A BOOM HOLE WAS CUT FROM 4 TO 2.
SLIPPED ON ICE WHILE AT SURFACE CHARGING STATION, TAKING SCOOP OFF CHARGE. MINER INJURED RIGHT SHOULDER, WENT TO EMERGENCY ROOM AT INDIANA HOSP., AFTER WORKING HIS NORMAL SHIFT. DIAGNOSED AS TORN ROTATOR CUFF. NO LOST TIME OR RESTRICTED DUTY FROM DATE OF ACCIDENT UNTIL DATE OF SURGERY. LOST TIME FROM DATE OF SURGERY TO RETURN TO REGULAR JOB AT FULL CAPACITY ON 11-18-03.
WHILE SERVICING THE MINER, EE REMOVED THE RUB RAIL FROM THE MINER TO HOSE AROUND THE SCRUBBER MOTOR. WHEN REPLACING THE RUB RAIL HE FELT SOMETHING PULL IN HIS LOWER BACK.
EE WAS TAKING SHOWER ON CEMENT FLOOR WHEN HE SLIPPED & FRACTURED HIS LEFT ANKLE
OPERATOR WAS TWISTING HIS BODY TO EXIT FROM THE OPERATORS COMPARTMENT WHEN HE STRAINED A MUSCLE IN HIS RIGHT LUMBAR.
A CAVE WAS FOUND IN OUR COMMON ENTRIES, 1 XCUT OUTBY SPAD #195 AREA IS INBY WHERE AN OTHER CAVE HAD FALLED ON 7/20/01. THE AREA THAT HAD FALLED WAS BOLTED WITH 6'POINT ANCHOR BOLTS.
A X CUT BETWEEN 36 AND 37 ROOM WAS JUST CUT THROUGH APX A 15' CUT. WHILE MOVING THE MINER, THE 15' CUT STARTED WORKING AND FELL IN ABOUT 2' TO 3' HIGH, AT THIS TIME THE ENTIRE X CUT 36 TO 37 S TARTED WORKING AND EVENTUALLY CAVED.
EE & FOREMAN WERE SITTING @ FEEDER DISCUSSING WHAT NEEDED TO BE DONE DURING SHIFT. #2 CAR PULLED ONTO THE FEEDER WHILE THEY WERE TALKING. EE GOT UP, WALKED PAST #2 CAR- OPPOSITE THE OPERATOR- GOING TOWARD HIS MANTRIP. #2 CAR PULLED OFF FEEDER & WAS MAKING A LEFT TURN WHEN THE BOOM OF THE CAR STRUCK EE KNOCKING HIM TO THE BOTTOM. SHUTTLE CAR OPERATOR DID NOT KNOW HE WAS THERE.
WHILE RUNNING SHUTTLE CAR HE HAD A DISCOMFORT INHIS CHEST. WHILE HELPING TO MOVE THE MINER HE BECAME SHORT OF BREATH. HE WAS THEN BROUGHT OUT AND TAKEN TO INDIANA HOSPITAL.
INTERSECTING SLIPS IN THE ROOF AT AN INTERSECTION.
WORKER WAS REACHING OVER A PIECE OF EQUIPMENT WHEN IT CAME IN CONTACT WITH THE MINE FLOOR CRUSHING HIS FOOT.
OPERATOR WAS STANDING IN WATER WITH HOLES IN HIS BOOTS, WHEN HE PICKED UP A CABLE WHILE MAKING CONTACT WITH HIS SHUTTLE CAR THAT HAD A FAULTY GROUND IN A VULCANIZED SPLICE. HE RECEIVED AN ELEC TRICAL SHOCK. HE WAS SENT TO HOSPITAL FOR OBSERVATION. HE WAS RELEASED AND SENT BACK TO WORK, MISSING ONE DAY.
ROOF FALL IN THE #5 ENTRY OF WEST MAINS AT 4+70 APPROX 20' LONG, 10' WIDE AND ABOVE THE ANCHORAGE (USING 6' DOUBLE LOC BOLTS) WITH ROCK DOWN ALONG THE RIGHT RIB IN #5. THE AREA THAT FELL WAS M OSTLY ONE LARGE PIECE.
THE OPERATOR SETS FULLY UPRIGHT IN THIS COMPARTMENT. THE SEAT IN WHICH HE WAS RIDING ON BECAME DISENGAGED CAUSING HIM TO FALL ONTO HIS HAND, FRACTURING A BONE.
WHILE MOVING THE MINER, EE POSITIONED HIMSELF IN THE MIDDLE OF THE MACHINE, WHILE TRAMMING FORWARD, THE CABLE TENDING ARM CAUGHT HIM FROM BEHIND, WHEN THE ARM CAME IN CONTACT WITH EE, EE HIT T HE WRONG TROM LEAVER, WHICH PINCHED HIS LEG BETWEEN THE RIB AND CABLE TENDING ARM.
CAVED IN THE XCUT #1 TO #2 OF WEST MAINS, APX. 140' INBY STATION #125 OF NORTH MAINS ( THE CAVE IS 140' LEFT OF SPAD 125). THE CAVE IS APX. 50' LONG, 17' WIDE AND 8-9' HIGH. THE XCUT WAS BOLTE D WITH 5' TENSION REBAR BOLTS. THIS CAVE WAS FOUND WHEN MAKING A WEEKLY EXAM OF THE RETURN AIR COURSE.
TURNED A CROSS CUT INTO TWO CLAY VEINS THAT RAN PARALLEL IN THE CROSS CUT AND WAS BOLTED WITH TOO SHORT OF BOLTS. NO IMPAIRMENT HAPPENED DUE TO THE FOREMAN SEEING IT WAS GOING TO CAVE AND G OT THE MEN AND EQUIPMENT TO A SAFE AREA.
A ROOF FALL OCCURRED AT APX 11 PM IN THE #2 ENTRY OF NORTH MAINS STATION #73(9+10). THE CAVE IS APX 30 LONG 20' WIDE 15-20 HIGH. #2 ENTRY WAS BOLTED WITH 6' DOUBLE LOCK BOLTS UP TO WITHIN 10 F T OF THE CAVE. THIS AREA DRILL GOOD WHEN THE REBOLTING WAS DONE. THIS AREA WAS BOLTED WITH 4' FULLY GROUTED BOLTS.
A ROOF FALL OCCURRED PRIOR TO THE 0600 PRE-SHIFT. THE FALL WAS FOUND AT 0628. THE LOCATION OF THE FALL WAS IN #1 ENTYR OF NORTH MAINS, STARTING AT SPAD #65 (8H0) GOING OUTBY FOR 60'. THE #1 EN TRY IS A RETURN ENTRY.
2 CLAY VEINS RUNNING DIAGONAL IN NO.2 HEADING FOR ABOUT 60 FEET DROPPED OUT. STOPPED AT INTERSECTION WHERE 6 FEET BOLTS WERE USED.
TWISTED KNEE
EMPLOYEE WAS OPERATING OR MOVING ROOF BOLTER AND SLIPPED ON THE MUDDY BOTTOM AND THE BOLTER SQUEEZED HIM AGAINST THE COAL RIB.
WE MINE 8 FT. OF COAL. THE MAN WAS HANGING A RUN THRU CHECK WHICH REQUIRES REACHING THE ROOF. HE CLIMBED ON THE END OF A SHUTTLE CAR AND SLIPPED BETWEEN THE SUPPORT ARM & FRAME, TWISTING HIS LEG.
EMPLOYEE WAS WAITING TO START BOLTING A PIECE OF ROCK FELL BETWEEN THE BOLTS AND STRUCK HIS LEFT FOOT.
