Employee was laying on the shop floor underneath the #10 Wagner LST 25X removing the bolts from the axle support plate. As the last bolt was removed, the plate fell to the ground and smashed EE's left index and middle fingers. 7 stitches were required on the left index finger and 6 stitches were required on the left middle finger.

Employee was exiting the porta potty at the mine site headed back to her truck that was parked 15 feet away, when she fell forward hitting the ground. She was taken to the Hospital where x-rays showed her right wrist to be broken. She returned that day on light duty.

The employee was at the #5 drive area at #38 X-cut. He was removing a drive. He was on the in by side of the 25-X. The 25-X oper was backing up, EE reached for the winch hook that was on the tip of the fork. The fork was also on top of the drive arm. He didn't realize the forks were on top of the arm. The forks fell and hit him on top of the right foot on the steel toe

Person was removing an inner race off of a Pickup truck #120 Isuzu. He was removing the bearing race with a heel bar and a hammer. As he struck the heel-bar a piece of the race broke off and it penetrated his outer left index finger between the hand and the first knuckle. After the doctor removed the small piece of metal, it resulting in 4 stitches.

Two employees were in the process of setting a simplex jack to support roof in the belt line. When they applied pressure to the jack a piece of the roof broke and struck the employee in the upper left part of the back. Resulting in a partial collapsed lung.

The employee was preparing to help make a belt splice. He grabbed the template for the splice. He had it resting on the splice board when it slipped the pin slid through the template and caught his finger. Resulting in a laceration to his left ring finger on the tip.

The employee was applying whip checks to the hoses at the head-gate area of the LW. When he pulled the staple the back pressure popped the #13 valve out of the connection and struck him on the left ring finger. Resulting in a fracture to that finger.

Employee was using a 3/4 ton come-a-long to remove a 40' roll of 60" mine belt from a 975. The belt was lifted high enough for clearance, and then was being lower to the ground when the come-a-long released & free wheeled until the belt hit the ground. EE was holding onto the slack end of the come-a-long and his arm was jerked upward thus staining his left shoulder.

The employee was attempting to remove a plastic plug from a valve bank he was replacing. He grabbed his razor knife and tried to pry the cap out. The knife blade cut through the cap and also cut his left thumb.

The employee was letting down a track jack that was holding up a take-up truck. As he applied pressure to the jack handle it slipped hitting him in the left side of the head resulting in a small laceration.

The employee was dropping miner cable from the mine roof, the miner cable had tension on it. As it came down it bent the employee over backwards and to the side. Resulting in a strain in his mid to lower back on the left side.

The employee was removing cables from the #1 shield in prep. for extended push. He was cutting down zip ties and as he was performing this duty he cut through his left arm just above his wrist. Received 5 stitches

While shoveling silt onto the #2 main line belt, employee's left shoulder started hurting. After approximately another hour shoveling and the pain worsening, EE was brought outside and transported to the clinic where he was diagnosed with an acute muscle stain in left shoulder. He was given a cortisone shot and a prescription for a muscle relaxer & released to return to work.

Dropping a Kennedy overcast panel down. The first course of block had been removed. They were attempting to move the panel when it dislodged a piece of the wall(approx. 4 block) that fell striking employee on the right hand. Resulting in a fracture to the little finger and a laceration to the right wrist.

The employee was preparing to swap base lift hoses. He was waiting for the shearer to pass and he had his hand on top of the base lift. When the shearer came passed. They pushed the pan over and the base lift moved on the relay bar moving the base lift forward pinching his finger between the bolt and the base lift bracket.

The employee had a drill steel stuck in the roof. When he pulled on it to get it out the steel came down on his finger smashing it between the steel and the drill box. Resulting in stitches and a small fracture to his right little finger.

The employee was lifting a Kennedy panel off of the top of a damaged overcast. When he put his hand up and over the top of the panel, there was a metal shard on the top side of the Kennedy. He placed his hand on the piece of metal and it penetrated his leather glove and caused a deep puncture to the palm of his left hand below his thumb.

EE and his partner were putting up a metal mat. The mat was bent and they were trying to get it to stay on the TRS. The mat fell and struck EE in the right middle finger. Resulting in a 1" laceration. He was transported to Castle Dale Medical Clinic were he received 4 stitches. He was released to return to work on his next available shift.

Two employees were pulling a SC into the mine with a 25X. They had 2 3/4" chains between the machines. They went around a corner to backup. When they started forward the chains had slack in them an came apart. The SC rolled back and the hoses pulled the steering valve off and it hit the SC operator in the right forearm.

An employee was installing a compressor for a 25x shield hauler. He had the compressor in place an was preparing to mount it to the machine, when it rolled on it side and smashed his finger against the frame of the engine. Resulting in a laceration to the right index finger on the pad. Taken to medical clinic where they glued the laceration.

While setting a roof bolt in the roof, the employee was using a drill steel wrench to finish pushing it to the roof. With his hand on the wrench, he attempted to set the bolt. The wrench slipped off the bolt and kicked over smashing his hand between the wrench and the drill steel clamps, resulting in a Laceration which required 5 stitches.

While building a Kennedy stopping the employee had just raised one of the panels, he remembered that he needed a drain pipe under the panel. When lowering the jack the panel stayed up. While removing the jack the panel collapsed, falling against his left hand, cutting his left small finger. He was diagnosed to have received a Laceration which required 7 stitches.

Employee was attempting to change the wench on the #9 Wagner 25X in the surface shop. As he removed the last bolt, the wench slid back and smashed his left hand between the wench and the frame and cut his hand. 10 stitches were required to close the wound. EE was released to return to work.

while pulling the shuttle car slack in 20th East, Employee had secured a loop of shuttle car cable to the Mini Trac and walked away when he noticed that the nip was falling off of the machine. As he walked back to the machine, another employee signaled the operator to go, and the machine ran over employees right foot. EE was released with restricted duty thru 10/24/13.

Employee was initially injured on 10/5/13 at the 23rd West drive area when he was standing on a cinder block to operate a come-a-long and his foot slipped off of the block thus spraining his left ankle. EE was examined and released by Dr. on this date. On 10/14/13 he returned to the doctor and was given a modified work release.

Employee was attempting to set a simplex jack, he was holding the jack with his left hand & had slide the top section of the jack up to the roof. As he was jacking the jack upwards, the top section of the jack released & slide down inside the bottom section of the jack & smashed his left index finger between the top and bottom sections of the jack. Laceration required 9 sutures.

2 employees were changing a top roller in the tail piece in 22nd West. While attempting to install the new roller, the roller slipped and smashed injured miners right hand between the roller and the frame structure of the tailpiece, resulting in a fractured Scaphoid bone in the right wrist.

As employee was preparing to enter the operator cab of a Wagner 25X in 22nd West, rock and coal sloughed off from the roof/rib and hit his right shoulder area. EE went to the ER, x-ray were negative, diagnosed abrasion right shoulder, released for work. The next day his neck was stiff, went back to ER and Doctor has him off from work until 9/3/2013.

After drilling a hole, the employee placed a roofbolt on the drill head and as he attempted to push the bolt into the hole, the bolt missed the hole & hit the mine roof causing the roofbolt to bow out and smash his left index finger between the bolt and the drill steel clamps resuting in a laceration which required 7 sutures.

Employee was setting up the #3 face in 22nd West. He was preparing to install a ventilation tube (that was laying on the ground) into the existing ventilation tube that was hung from the mine roof. He noticed that the vent tube needed to be turned end for end and as he reached & lifted the tube and spun it around, he felt a sharp pain in his lower back. Released for modified work.

Employee strained his right shoulder while attempting to tighten a hydraulic fitting on the Eimco 975 in the surface shop. EE was taken to the Doctor, examined & a MRI was ordered & he was released to return to work, EE continued working until 7/23/2013, at which time he underwent surgery to repair torn muscles in his right shoulder.

Employee was building a metal cross over on the 2nd West Mains belt conveyor at cross cut #8. As he would tack weld the pieces together, he neglected to lower his welding hood and when he started to weld is when he would lower his welding hook. Employee sustained flash burns to both eyes.

Employee was taking the angle iron rails off of a Kennedy stopping at x-cut 25 in 20th West, so the stopping could be removed. As he went to throw a rail aside, the rail caught his right leg and cut his leg. 10 stitches were required to close the wound.

The employee and 2 other employees were attempting to place a monorail dolly onto the monorail system at x-cut 26 in 20th West. When the dolly slid into place, it smashed his right thumb between another dolly already installed. 6 sutures were required to close the laceration on the right thumb. 2 prescriptions were also prescribed.

Employee picked up a 4" aluminum pipe that was approximately 4 foot long. As he attempted to toss the pipe along the rib at x-ct 75 in the left mains, his glove snagged a sharp burr on the end of the pipe resulting in a laceration to his little finger on left hand. 8 sutures were required to close the wound.

Injured employee and another employee were carrying a 60" bottom belt roller on their shoulder, up the slope on the 2nd West belt entry at x-cut #2. As the employee stepped over a small roll in the mine floor, he felt a sharp pain in the middle of his back.

Employee was lifting a 5 gallon bucket of hydraulic oil into the bed of an Isuzu pickup truck when he felt a sharp pain in his lower back. He was initially examined by a doctor on 2/26/2013 and diagnosed with a back strain and released to return to work. He returned to the Dr. on 4/29/2013 for a follow-up and was placed on light duty pending an MRI.

Employee was operating a Fletcher double boom roof bolter in the face of the 21st West development section. As he went to drop his canopy, his right hand was placed on the material tray and as the collar on the canopy boom went past the cable tray it smashed his little finger on his right hand thus resulting in the last 1/4 inch of his little finger being cut off.

Employee was attempting to remove the cover from the exhaust filter housing on the #9 25X located at x-c 37 on #5 road when the cover slipped out of his grip and smashed his left thumb resulting in the thumb nail being removed, a laceration (4 stitches and a fracture to the bone in the tip of the thumb.

On 2/11/13, Employee strained his lower back as he was attempting to lift the end of a vent tube into place in the #2 entry of 21st West. EE reported the incident to his foreman and continued working all of his shifts until 2/25/13, at which time he went to the Dr. and was instructed to be off from work until a MRI could be conducted and evaluated.

As the employee exited the forklift on the surface, he slipped on the ice and twisted his left knee, EE was taken to the hospital and examined and released to return to work. EE had a follow-up visit on 2/20/2013 and a MRI was conducted on 2/26/2013. On 2/27/13 the EE was referred to Orthopedic surgeon and instructed to be off from work until after surgery.

Employee was repairing a damaged overcast when another employee working upwind of him dumped a bag of mortar mix and some of the mortar mix was suspended into the air current and blew into the employees left eye.

While retrieving vent tubes from the face area of the #1 entry in 21st West, the employee twisted his left ankle. EE was examined by a physician and the results of the x-ray were negative and the EE was released to return to work. The next day he return to the physician and was given a work release excusing him from work for that day.

On 1/11/13 the employee claims that as he was walking down the slope in 2nd West intake from the last open x=cut towards the transformed, he suddenly felt a sharp pain in his right groin. The employee continued working his scheduled shifts until 1/29/2013 at which time he had surgery to repair the hernia. This injury became a lost time injury as of 1/29/20123.

The employee and two others were installing 60" top roller clusters in the new belt line in the 1st West Mains section. As they lifted the cluster, it slipped from the injured employees hand and as he tried to catch the roller cluster, his right ring finger was smashed between the roller cluster and another roller cluster that was laying on the ground.

Employee was working on the 17-2 battery scoop in the 1st West Raises section. As he attempted to place the cover on top of the machine into place, when he lifted the cover and twisted to slide it into place, he felt a sharp pain in his left shoulder.

After the employee exited from the Isuzu mantrip # 23-22, as he slammed the door shut on the mantrip, he smashed his index finger on his left hand in the door resulting in a laceration to the finger which required 10 stitches and also fractured the tip of the bone on his index finger.

While conducting an on shift examination in the tailgate area of the longwall in 17th East, The examiner lost his balance while traveling thru an area where the shields had been pulled in and when he reached down with his right hand to catch himself, he felt a pop in his right wrist. X-rays indicated a hairline fracture to a small bone in the back of the wrist.

Employee was holding a shuttle car cable in his left hand while preparing the cable for a splice when his razor knife slipped and cut the palm of his left hand. 7 stitches were required to close the wound.
