DATE OF DEVELOPMENT 8-02. ROOF SUPPORT 6' FULLY GROUTED RESIN BOLTS WITH STRAPS. IN A 4 WAY INTERSECTION. THE FALL WILL NOT BE CLEANED UP.

Mostrando los 34EE was taking a belt pin out using two hammers. When he hit one hammer against the other a piece of hammer (steel) came off and went into his left hand above the thumb.
EE WAS MOVING CABLES. HIS RIGHT LEG GOT CAUGHT IN THE MUD. WHEN HE PULLED HIS LEG OUT OF THE MUD HE SPRAINED HIS RIGHT KNEE. ON 5/2/05 HE HAD TO HAVE SURGERY ON HIS RIGHT KNEE.
A PILLAR FALL OCCURRED WHILE BACKING THE CONTINUOUS MINER OUT OF THE SECOND PILLAR SPLIT OF THE NO.10 PILLAR BLOCK, COVERING UP THE HEAD OF THE MINER. THE LENGTH, WIDTH & THICKNESS COULD NOT BE DETERMINED. THE FALL WAS IN THE NO. 10 ENTRY, 3 BLOCKS INBY STATION NO.219.
EMPLOYEE WAS OPERATING THE FLETCHER ROOF BOLTER. WHILE BACKING UP TO GO INTO ANOTHER FACE, HE PINCHED HIS RIGHT LEG AND FOOT BETWEEN THE ROOF BOLTER AND THE JOY SHUTTLE CAR. HE WAS IN THE DECK OF THE BOLTER WITH HIS RIGHT LEG ON THE BUMPER.
A roof fall was found at 11:00 am on 6-9-04 in an idle entry 001 north mains. The fall will not be cleaned up. It was 18' wide 45' long 8' high. fully grouted resin bolts with 11' straps.
EMPLOYEE WAS WALKING BY THE POWER CENTER OF THE SECTION. HE SLIPPED ON THE BELT MAT AT THE POWER CENTER, SPRAINING HIS RIGHT KNEE. HE DID NOT GO TO THE DOCTOR UNTIL 6/11/04.
EE WAS STEPPING INTO THE SCOOP BUCKET TO HOOK UP A CHAIN WHEN HE PULLED HIS LEFT KNEE.
A ROOF FALL WAS FOUND ON #1 BELTLINE AT SPAD NO. 14 IN A 4-WAY INTERSECTION. IT WAS 18 FT WIDE, 26 FT LONG, 10 FT HIGH. THE ROOF SUPPORT IN THIS AREA WAS 6' FULLY GROUTED RESIN BOLTS WITH CROS SBARS. DATE DEVELOPED WAS MAY 2002.
EE WAS REPAIRING THE CONVEYOR CHAINS ON THE MINER, WHEN HE HIT HIS LIFT FOOT WITH A SLEDGE HAMMER.
EE BENT OVER TO MOVE THE MINER CABLE OUT OF THE SHUTTLE CAR ROADWAY, WHEN HE PICKED THE CABLE UP HE PULLED A MUSCLE IN HIS LOWER BACK.
EE WAS CHECKING A HYD. HOSE THAT HE HAD TIGHTENED ON THE 488 S&S SCOOP. HE HAD TO BEND OVER AND LOOK INSIDE TO SEE IF IT WAS LEAKING. WHEN HE TURNED THE SCOOP ON AND PUSHED THE LEVER A HYD. HO SE BESIDE IT BLEW UP IN HIS FACE, AND GOT DIRT AND METAL IN HIS LEFT AND RIGHT EYES.
EE WAS TYING A PLUM-BOB TO A ROOF SPAD. A PIECE OF ROCK 3' X 3' X 1' FELL FROM BETWEEN ROOF BOLTS. IT STRUCK THE EE ON THE BACK HIS HEAD & HIS BACK.
THERE WAS A ROOF FALL IN THE PITMOUTH OF THE #3 HEADING, WHICH IS THE FAN ENTRY, THE FALL WAS 15FOOT LONG, 1'WIDE & 6'HIGH. THIS AREA WAS MINED 3/2002, IT WAS SUPPORTED WITH 6'FULLY GROUTED RESIN BOLTS, AND CROSSBAR. THE FALL WAS CLEANED UP.
AT 1:30 P.M. ON 2/12/03 A ROCK SLID DOWN OFF THE HIGHWALL AND KNOCKED OUT THE CANOPY IN #4 ENTRY. THIS ROCK WILL BE CLEANED UP AND AN ARCH SET INSTALLED.
ROOF FALL IN THE #1 ENTRY AT SPAD NO. 57 IN THE EAST MAINS SECTION. FALL WAS 7' HIGH, 16' WIDE & 5' LONG. SUPPORTED WITH 6' FULLY GROUTED RESIN BOLTS WITH 11' STRAPS.
EMPLOYEE WAS OPERATING A SHUTTLE CAR ON THE MAIN SECTION WHEN HE GOT DUST IN HIS RIGHT EYE. HIS EYE WAS WASHED OUT THE SAME DAY AT THE HOSPITAL. ON 11/11/02 HE WENT BACK TO THE HOSPITAL. THEY SENT HIM TO A DOCTOR WHO TREATED HIS EYE INFECTION. HE DID NOT MISS WORK UNTIL 11/11/02.
AT 1:00 PM ON 10/13/02 A ROCK SLID DOWN OFF THE HIGHWALL AND KNOCKED OUT THE RIGHT WASS OF THE #4 ENTRY CANOPY. THE ROCK WAS CLEANED UP AND CANOPY REBUILT.
A ROOF FALL WAS FOUND ON THE FIREBOSS RUN IN A 4WAY INTERSECTION, 16' WIDE, 10' HIGH, 60' LONG. IT WAS BOLTED WITH 6' FULLY GROUTED RESIN BOLTS. NO DAMAGE TO REPORT.
EE ALLEGES STEPPED IN HOLE AND TWISTED RT. KNEE.
A ROOF FALL WAS FOUND ON 5-28-02 AT 5:33 AM BY THE FIREBOSS. IT IS NOT KNOWN WHEN THE FALL OCCURRED. THE FALL WAS 16' WIDE, 25' LONG, 18' HIGH. IN A 3WAY INTERSECTION. 6' & 7'3/4" RESIN BOLTS WERE INSTALLED IN THIS AREA. ALSO 3" X 8" X 16' CROSSBARS. THE FALL WILL BE CLEANED UP.
FAIRMOST ENERGY MINE #2 WAS STARTING. PITMOUTH UNDERGROUND. THE PITMOUTH FOR THE #3 ENTRY WAS IN THE HIGHWALL 15' AND 5-5' ROOF BOLTS HAD BEEN INSTALLED WITH 3-6" H BEAM, INSTALLEN. THE HIGHWA LL FELL IN TAKING OUT THE 5 ROOF BOLTS AND THE 3-6" H-BEAM.
EMPLOYEE WAS MOVING A SUPPLY CAR WITH A KERSEY TRACTOR. WHEN HE WAS PULLING THE PIN OUT OF THE TONGUE OF THE SUPPLY CAR HE SMASHED HIS FINGER ON HIS RIGHT HAND.
A ROOF FALL WAS FOUND AT 11:30 AM ON 07/27/01 ON 1 EAST MAIN 3 BLOCKS INBY SPAD 117 IN CROSSCUT BETWEEN 5 ENTRY AND 6 ENTRY. THIS AREA HAD BEEN CRIBBED OFF FOR 1 YR BECAUSE OF A BROW HANGING. THE AREA WAS MINED ON 8/16/99, THE FALL WAS 6'HIGH, 16'WIDE, 15'LONG. SUPPORTED BY 6' FULLY GROUNDED RESIN BOLTS, CRIBS, CROSSBAKS.
EMPLOYEE WAS SHOVELING OUT A "GOBED OUT" ROCK BELT UNDER THE RAW CAOL SCREEN. WHILE HE WAS STANDING UP ON THE BELT, THE BELT BROKE AND HE FELL INTO THE GOB, HITTING HIS HEAD ON A CONCRETE BARR IER.
EMPLOYEE WAS BOLTING ROOF AND CUT HIS RIGHT HAND BETWEEN HIS THUMB AND FINGER.
EMPLOYEE WAS OPERATING A TRACTOR WHEN STEERING WHEEL BALL CAME AROUND AND HIT HIM ON THE WRIST.
EMPLOYEE WAS ROOF BOLTING IN NO 2 ENTRY. AFTER BOLTING THE PLACE HE WAS CHANGING HIS DUST BOX AND A PIECE OF ROCK HIT HIM IN THE BACK, HEAD, ANDLEGS.
EE WAS OPERATING #2 SHUTTLE CAR WHEN THE WHEEL UNIT BOLTS BROKE OFF LETTING THE SHUTTLE CAR HIT BOTTOM, INJURING THE RIGHT SIDE OF HIS BACK. HE DID NOT MISS WORK OR REPORT PROBLEMS UNTIL 4/30.
EE WAS CUTTING A CONVER OFF WITH THE MINER, WHO A PEACE OF SULPHUR BALL CAME BACK AND HIT HIM INT HE RIGHT THIGH.
EE WAS OPERATING THE #3 SHUTTLE CAR ON THE 2 EAST SECTIONS. HE HAD LOADED HIS FIRST LOAD OF COAL AT THE MINER. HE WAS GOING TO THE FEEDER TO UNLOAD. HE WAS TRAMMING TO THE FEEDER, HE WAS AWARE THE SHUTTLE CAR WAS STICKY TO TRAM GOING TO THEFEEDER. HE GOT TO THE FEEDER AND UNLOADED THE LOAD OF COAL. AS HE WAS GETTING OUT TO TURN AROUND, THE SHUTTLE CAR JUMPED ACROSS THE FEEDER, PINC
EE WAS OPERATING #1 SHUTTLE CAR ON THE 2 EAST SECTION TO THE BELT ENTRY. THERE WAS BELT STRUCTURE STORED UP BY THE RIB. THE TIP OF THE SHUTTLE CAR HIT A PIECE OF STRUCTURE AND HIT EE ON THE RI GHT SHOULDER, BRUISING HIS SHOULDER. EE DID NOT MISS WORK UNTIL 3-6-01.
EE WAS OPERATING THE MINER AND CUT A XCUT THRU. WHEN THE RIPPERS CUTH THRU, EITHER THE TIP OFF OF A BIT OR A PIECE OFF A HEAD OF A BOLT, FLEW BACK AND HIT EE IN THE UPPER LIP, CUTTING THE LIP INSAIDE AND OUT.
EE WAS ROOF BOLTING ON 2 EAST SECTION RIGHT PANEL WHEN A ROCK FELL ON THE CANOPY THE ROCK WAS 2'X4'X6" WHEN IT HIT THE CANOPY IT BROKE INTO & HIT EE ON THE RIGHT LEG & ANKLE
