There were heavy rains all morning while we were working on equipment. After completing work on the D10R I was checking the fluid levels. When climbing off the dozer my foot missed the step on the push arm and began to slide. My feet went above my head and I fell 4-5 feet on my back.

Two mechanics were lifting center steering link. One mechanic slipped. As they were both letting go of center steering link, one employee held on too long and "strained" his back. Or appeared to strain his back. According to the witness he let go of steering link first. Mechanic strained back on 2/4/15. Did not notify supervisor until 2/7/15.
Mechanic was trying to pull steel of drill into place, so it could be put into rack. AS the operator swung the rack in, the mechanic's foot was caught in a pinch point.
Employee exited tractor trailer cab and stepped on ice resulting in a sprained right ankle.
Moving rock truck tires and wheel rings with #703 excavator, while unhooking chain from several wheel rings, one of the wheel rings slipped down and caught left ring finger between tow of the wheel rings.
Putting torque in D-8 dozer & twisted back. He is still working, this was reported to me on 9/26/05.

THERE WAS A LARGE ROCK ON TOP OF SHOT, WHEN IT SLID OFF IT DUG IN AND TURNED END OVER END HITTING RIGHT CORNER OF THE TRACK BED, CAUSING A WHIP LASH MOTION TO OPERATOR HURTING NECK.
THE EE ALLEGED THAT HE WAS BEING LOADED BY THE 92G AND THAT A ROCK ROLLED ACROSS THE BED AND STRUCK THE OFF SIDE OF THE BED, CAUSING INJURY TO LOWER BACK. INJURY REPORTED 3-19. EE WORKED 3-14 3-15. WAS OFF WORK 3-18 FOR DOC APPT. PRE SCHEDULED WORKED 3-19 REPORTED COMP 3-26.
