Fell from 3-4 entry to #7 entry. 3-4 was bolted on 3x4 with straps in the intersection & 4 8' resin bolts. 5,6,7 entries were bolted on 4x4 with 6' torque tension bolts & 4 8' resin bolts in every intersection.
The EE was straightening a 6' torgue tensioned bolt and the bolt spun around, injuring the EE's left shoulder.
A roof fall was found 80' L x 19' W x undetermined H at 90' outby station 2513 in #2 entry along 8 belt. This fall does not impede ventilation or travel.

The employee pulled (injured) his lower back loading bolts from the scoop bucket to the bolting machine. His first lost time was November 5, 2007
On November 15, 2007 the employee completed an injury report for October 29, 2007 stating his knee hurt him at the end of his shift. He has been off work since 11-02-07. He had MRI read on Nov. 19 which indicated a medial meniscus tear and is scheduled for physical therapy for six weeks.
The employee slipped on a rock and twisted his right knee on 9/26/07. He continued working until 12/19/07 when he visited a surgeon that recommended time away from work.
When taking out drill steels from the roof, two 3" steels fell out striking the employee's hand against the bolter.

At 10:00 am on 12-6-05, a roof fall was found in an outby area of 9 mains, in #6 entry from station 2709 to station 2923. the roof fall was 20' wide x 80' long x unknown height.
Employee threw a 6' bolt on the roof bolter. The bolt bounced striking him in his upper lip. Required 5 stitches outside and 1 stitch inside.

The ee pulled down a loose piece of rock (24"x36"x4" thickin the (enter triangular with shape) when it hit the bottom the piece flopped over striking his left knee.
WHILE BOLTING, A PIECE OF COAL (12" X 16" X 3") FELL FROM THE ROOF AND HIT THE EMPLOYEE'S RIGHT SHOULDER.
