EMPLOYEE WHILE WALKING IN WALKWAY ALONG M8 BELT SLIPPED AND STRUCK HIS HEAD ON OVERHEAD BEAM CAUSING PAIN AND STIFFNESS IN HEAD AND NECK.
EE WAS ATTEMPTING TO PUT SUPPLY CAR ON TRACK W/ AN AIR BAG. WHILE BLOWING UP THE AIRBAG A PIECE OF WOOD FLEW OUT STRIKING HIM ON THE LEFT SIDE OF THE FACE. RESULTING IN CUT ABOVE THE L. EYE & BONE FRACTURE OF LEFT SIDE OF FACE.
INTERSECTION AT 223 XCUT ON MAIN 9 BELTLINE BROKE LOOSE AND FELL AT APPROX. 0900, STOPPING OPERATION OF MAIN 9 BELT. FALL SIZE WAS APPROX 20' X 30' (ENTIRE INTERSECTION) MINED 9-4-86. SPAD 24+ 40.
EMPLOYEE WAS SHOVELING AROUND THE BELT AND THROWING MATERIAL ONTO BELT WHICH WAS OVER HEAD. WHILE THROWING A SHOVEL FULL OF MATERIAL ONT0 THE BELT A ROCK CAUGHT THE SIDE OF THE BELT AND IT FLEW BACK CUTTING HIS LEFT EAR.
EE WAS RUNNING 15 TON MOTOR PULLING TRACK CLEANER AND THE TROLLEY POLE CAME OFF THE TROLLEY WIRE, EE REACHED FOR THE POLE AND RAISED UP AND HIS HEAD HIT AGAINST OVER HEAD CANOPY OR STEEL BEAM CAUSING HIS HEAD TO GO DOWNWARD HITTING SOME PART OF THE MOTOR DECH CAUSING CUT TO BACK OF HEAD AND OVER LEFT EYE.
TOP BROKE LOOSE CAUSING FALL FROM 208 1/2 X-CUT TO 209 1/2 X-CUT. APPROX. 100' LONG.
TWO WORKERS TOOK THE COVER OFF THE BATTERY MOTORTO CHANGE FUSE FOR COMPRESSOR MOTOR. AFTER CHANGING FUSE THEY RESET BREAKER AND COMPRESSOR DIDN'T RUN SO THEY PUT OLD FUSE BACK IN AND SET THE BREAKER. EMPLOYEE WAS ON OPPOSITE SIDE OF MOTOR & CAME IN CONTACT WITH COVER OVER BATTERY LEADS.THE RESULTING ARK MELTED COVER CAUSING A CUT TO RIGHT ARM & FLASH BURNS TO ARMS, FACE, ABDOMEN.
TOP FELL PULLING BOLTS OUT IN AN AREA 20' BY 18' BETWEEN 207-208 XC ON MAIN 9 BELT.
154 XC WHICH WAS SUPPORTED WITH 6' RESIN AND 12' COMP BOLTS AND STEEL SETS ON 4' CENTERS BROKE LOOSE AND SAT DOWN ON STEELS SET WHICH IN TURN PUSH STEEL SETS TO BELT LEVEL. LOCATION 10 & 04 #3 ENTRY EAST MAINS MAIN 8 BELT.
