employee was stacking 100# bags when he felt a pain in his right shoulder. he informed the lead man and they stopped packing, once stopped packing, they informed the plant foreman. they brought employee to get a drug screen and see the doctor. the doctor gave him 2 prescriptions. his report from the doctor states that he can return to work with out restrictions.

Employee operating bag spout valve up and down filling 4000# bags at tank #1. he reported his shoulder was bothering him. We sent him to the company doctor and the doctor said that he had a bone spur to take aleve and he was released for full duty work (it was an first aid only case). employee says still hurting and went to see his own doctor and now he is waiting to get an MRI.

employee was swapping bulk loading hose from tank 6 over to tank 7. He squatted down to close ears for connection, stood back up and felt a slight release followed by what felt like fluid filling in my lower back. Went to walk to pumper scale house and was starting to feel pain and stiffness with each step.

Employee was sitting down welding on a mill part (spider) when slag from the welding rod fell inside his laceup boot burning the inside of his left ankle and bottom of left foot.
While stacking 100# bags of barite on pallets employee's left hand got caught between moving rubber conveyor belt and non moving roller belt. Employee was wearing gloves and the emergency stop button did work stopping the belt. Hospital said employee strained muscles in hand & arm.
While stacking 40kg bags of barite on 42"x42" pallets employee's right foot entered the forklift slot in the pallet. When employee turned to put bag on pallet he lost his balance and his right foot became lodged in the pallet dislocating his big toe on his right foot.

WHILE CLEANING HARD MATERIAL FROM CHUTE WITH A T HANDLED SCRAPPER. EMPLOYEE SMASHED MIDDLE FINGER LEFT HAND BETWEEN T HANDLE AND A PIECE OF METAL SCREEN.
WHILE INSTALLING AN ADAPTER PLATE TO THE BOTTOM OF #5 MILL, THE INJURED WAS IN A SITTING POSITION, WHILE TRYING TO POSITION THE ADAPTER PLATE, THE INJURED STRAINED HIS UPPER RIGHT BACK. HE DID NOT WORK 6-9-95 FOR PERSONAL REASONS. RE-IN JURED HIS BACK ON 6-10-95 WHILE PULLING ON A PIPE WRENCH.
WHILES USING A METLA PRY BAR TO PRY FEED GATE OPEN ON #1 MILL THE EMPLOYEE FELT PAIN IN HIS BACK, EMPLOYEE RPORTED ACCIDENT BUT SAID HE WOULD BE OK AND CONTINUED WORKING. THE INJURY HAS BOTHE RE HIM ON AND OFF SINCE THEN BUT HE SAID HE WOULD BE ALRIGHT. ON 7-17-95 EE WENT OT THE DR HIS FIRST DAY OF WORK MISSED WAS 7-18-95.
