EE went up the small henatite ramp to get material to feed the hopper and then fill the bucket of the front end loader and started down the ramp in reverse. He came to close to the edge to the ramp and flipped the front loader on its left side.

EE was tightening a bolt plug on a mill gear reducer when the plug stripped out and the wrench. He was using came off the bolt plug causing his left arm. He was using came off the bolt plug causing his left arm to strike against the corner of a protective guard causing a small puncture and bruise to his left upper arm above the elbow.

EE HAD AN ACCIDENT WHILE TRYING TO DISLODGE A ROCK WHICH HAD BEEN LODGED IN THE FEEDER ROLLER. ACCORDING TO EE HE HAD BEEN HAVING PROBLEMS WITH LARGE ROCKS GETTING STUCK AT THE FEEDER ROLLER. HE TRIED TO DISLODGE THESE ROCKS W/ A METAL ROD. DOING SO, THE METAL ROD GOT WEDGED BETWEEN THE ROCK & THE SIDE OF THE FEEDER ROLLER. HE LOOKED INSIDE THE FEEDER & SAW THAT THERE WAS A CLEARAN

HE WAS OPERATING BAGGING MACHINE. HE TRIED TO RAISE THE POWER DISCHARGE BAG CHAIN. HE HAD BEEN INSTRUCTED NOT TO MAKE ANY ADJUSTMENTS, TO STOP MACHINE AND GO GET A MAINTENANCE MAN TO MAKE REPA IR.
