While removing a drilling hose from the flood shaft hole, the hose pressurized due to being capped off. When the cap was removed and pressure relieved, the cap flew off and struck a nearby employee in the ankle. The employee had a small abrasion wound in the ankle and ankle soreness the following day.

Two Production Miners were loading shot in a face. One miner started to feel weak and was unable to catch breath. The loading process was stopped and the ill miner was taken to the Production office where attempts to cool the employee were started.

Employee attempting to open a slide on an air lock door. The employee pinched EE's right hand, at the base of EE's thumb, between the handle of the slide door and the door.

Employee complained of nerve pain in arms after using air and electric chipper for months doing shaft work. An occupation doctor visit triggered a nerve study. The nerve study was positive for carpal Tunnel Syndrome. This was considered compensable due to the nature of the work. This became a recordable injury on the date of surgery.

Employee was driving a bolt with an impact wrench. Brake clean solvent was in the hole and when compressed, combusted and blew the bolt out of the hole. This drove the impact upwards. The employees left hand was on top of the gun. EE's left index finger was mashed in between the gun and the equipment breaking it, requiring surgery.

Louisiana Cat Techs were installing the tilt cylinder on the rear axle. While trying to line up the bores of the frame and the rod of the cylinder, the bearing on the frame was not lined up and was caught. While lining up EE's hand was on the bottom of the cylinder, when cylinder lined up it dropped 3" onto EE's finger. EE had gloves on at the time.

The employee was removing a guard. EE was holding the guard with left hand and struck the guard with a hammer with right hand. The guard dropped down & pinched the small finger on left hand between a block and the handle on the guard. The employee broke small finger on left hand.

EE was removing a drill rod from a drill. EE was holding the drill rod on the very end instead of the middle. When the threads finished the rod fell to the ground jolting EE's left elbow. This inflamed tendonitis in EE's elbow. EE was shown stretches and released to full duty until the injection on 6-13-17.

Employee/contractor was working on a troubleshooting task for approximately 45 minutes to 1 hour on computer in the clearlane building with mine employee/witness. EE passed out/fainted in the clearlane room. The room temperature was warm at an estimated 100-105 degrees. Heat and dehydration were determined to have contributed to the incident.

Employee was cutting belt line with a retractable knife. Employee put themselves in the line of fire by cutting towards wrist and cut themselves needing sutures.

Injured employee was front seat passenger in a four seat UTV personnel vehicle. The driver turned too wide and tipped the UTV over on the driver's side. Injured employee reached across chest with left hand to brace for impact, injuring left shoulder. Did not become recordable accident until 8/23/16

Initial injury occurred on 7/14/15 while employee was stacking 50lb salt blocks on a pallet. After visiting the doctor it was determined that it was just a strain and employee was sent back to work on full duty. On 5/31/16 Employee was shoveling salt at the screen plant. After taking EE to the doctor it was determined that EE has a hernia and was put on light duty.

An employee was using a retractable blade safety knife to cut a tie wrap. EE had the safety knife in right hand. EE was holding a pipe and hose in left hand. The pipe moved and the knife stuck the employee's left thumb causing an approximate 2" laceration on the inside portion. The employee was taken in for medical evaluation, received sutures and released to full duty.

Employee plugged in a golf cart. He tripped over the wire and grabbed onto the top of the golf cart to prevent his stumble. Employee injured right shoulder in the process. At first it was not reportable. He was only given home exercises and told to put heat on it. On 2-16-2016 it became reportable when injections were given and physical therapy was prescribed.

Employee was cutting the banding off of troughing idlers. When the final band was cut the first row of idlers fell off of the pallet striking the employee on the right foot, penetrating the boot behind the steel toe and injuring right big toe. The employee fell to the ground between the stack of idlers and a material handler which was parked aproximately 4' away.

The employee was running the hammer. He was going down steps when his right heel got hooked and he fell forward to the cement and stuck his left hand out to break his fall. His left middle finger was jammed and he received what is referred to as mallet finger that needed to be surgically repaired through outpatient surgery.

1597 Scaling rig ruptured a hydraulic hose which caused a fire. The suppression system shut off the rig and went off as designed. The employee was able to get safely off of the rig and noticed a fire that was burning uncontrollably. Everyone was evacuated safely and the scaler burnt to the ground. The scaling rig was a total loss and the mine was shut down for two days.

An employee was cutting a 1" flex pipe with a safety knife. The knife slipped and cut through the black dot glove and the inside of his middle joint on the middle finger of his left hand. The employee needed sutures to repair the laceration. He was also given a prescription for the pain. The employee was released to full duty on his next shift with no restrictions.

An employee was removing a sling from a fork while standing adjacent to a undercutter bar. The outrigger for the material handler that was holding the bar on a set of blocks was raised. The bar slid off of the blocks landing on the left foot of the employee.

The employee was attempting to close the width of the forks on a 4 wheel drive Gradall Material Handler. He placed his left hand into the pinch point behind the fork and his right hand slipped off the fork. His left middle finger tip was crushed in between the fork and the mast.

Employee was descending a service loader with a filter assembly. The assembly got stuck on the steps and the employees right middle and ring finger tips were snipped off by a pinch point created between the handle and the housing.

Employee was using a 2" ball valve to try and drive out a pin to change out forks on a Gradall Material Handler. The employee missed his target and sliced his left pinky finger open requiring 3 stitches.

Employee was working on a water heater. His right foot got caught on a copper tubing and he tripped. He fell, landing on his right knee, followed by his face and then his hands. He received surface abrasions on his face and right knee and had a stiff neck and back. He was released to full duty. But on January 31,2012 he was taken out of work pending MRI and treatment.

Employee was using a lift assist to stack 50# salt blocks on a pallet. The assist failed and a block was dropped onto the ground. The employee bent at the waist to pick up the block and then pivoted to place the block onto the pallet. While pivoting he felt a pain in his right lumbar. He did not report this until the next day when the pain persisted.

Employee was tramming the 980 loader to the work area and reached down to adjust the seat. He over-reached and his finger went inside the seat suspension. Hitting another bump, the weight of the seat along with his body weight, collapsed the suspension causing it to pinch the middle finger on his right hand.

Employee was changing a hose on a drill without stopping for breaks or drinking any water. The employee had to be brought to the hospital by ambulance. Test were run which all came back negative and IV's were administered with no prescriptions, just to hydrate. Employee was diagnosed with heat stroke, near syncope and released to full Duty with no restrictions.

Employee was negotiating a corner in a John Deere Gator on a slope underground and the right tires climbed the berm guarding the right rib after turning too short. The Gator turned over on it's side and the employee's left leg was injured in the accident.

While hoisting salt, the South skip dumped the load of salt and one of the pedestals for the dump arms broke. The dump gate opened while it traveled down the shaft and got wedged in the shaft. The damage caused the skip to be changed out.

Employee was walking up entry decline #20 near the #1 crosscut to retrieve survey materials. He slipped on the floor and fell, fracturing both bones in his lower right leg.

Employee was holding two boards (for alignment purposes) to be nailed together with air nailer. Hands were approximately 18" from point to be nailed. Air nailer double shot and the second nail struck employee in the left index finger between the hand and first joint in finger. Nail was removed from finger by the doctor and employee returned to work. Nail did not contact bone.

Crew was gathering tools and materials to start job. EE was returning to tool trailer for additional tools when he walked to close to an elevator cover which had been previously removed and placed on the ground. As ee passed by the cover, the flange on the cover caught on his pants, cutting pants and right leg.

Employee had no particular point during his shift where he felt any pull or pain. He noted that he was pulling up on a wrench all night while changing clips in the production shaft. He went straight to sleep after leaving work and woke with the pain. He has been diagnosed with a incarserated hernia that will be surgically repaired on Thursday January 21st.

Employee ran a John Deer Gator XUV into a MTI truck. Employee's face struck the ROPS on the Gator and sustained lacerations above and below right eye, busted teeth out of mouth, punctured right cheek and fractured portions of his upper jaw.

EE was bitten by a spider that came out of a pallet on Monday 11/02/2009. He did not want any medical care. It progressively got infected and he finally agreed on Thursday 11/05/09 to be seen by a physician. The Doctor prescribed a oral antibiotic and released him to full duty with no restrictions.

Employee was replacing bolts in gallery structure. Driving old 3/4" bolts out of wood structure using new 3/4" bolts as a punch. As he was striking bolt with sledge hammer, his left elbow struck an existing bolt. Bruised left elbow.

employee and partner were changing the oil on a speed reducer for belt drive # 204. After completing task, they were cleaning up the work area. They attempted to lift a 50 lb Jabsco pump into the back of a Gator, as EE bent down and gripped the handle, he felt something pop in his lower back.

Employee opened door to check weather. He leaned on a removeable handrail that was not seated properly and it gave way, causing him to fall 4' to the ground, face first.

New, first day of labor. Employee was working with 2 miners loading a face with Anfo. Work started at 9:30am. After running the hose for the first heading, employee complained of chest pains, sweating, nausea. He was removed from work, given oxygen, removed from the mine, taken to the hospital, treated for dehydration and checked for cardio-vascular issues. None found.

At 8:30 pm power was lost to the mine. Evacuation called. MSHA was notified. After evacuation completed, electrician and supervisor entered the mine and discovered the 4160 volt cable had been damaged in an abandoned area of the mine, by a roof fall. The cable was disconnected, re-routed and repaired. This took place on the 500' level.

Employee had been standing on a ladder welding the frame of a CAT loader of which he had also been welding on at different times the week prior. Around the time of the illness, he complained of dizziness and nausea. The determined result was that employee had suffered a heat stroke due to dehydration and elevated temperatures.

The employee returned to work with restrictions.
