An employee was using a chisel and hammer to remove a bearing. The chisel splintered and a metal fragment hit employee on the forearm causing a small laceration. A month later, employee started feeling discomfort when moving finger. An X-ray was done on 9/16/24 showing a foreign body embedded in their arm. Surgery will be required to remove it.

Employee was spraying down the material build up on the rerun stacker belt. Water splashed up onto EE's face and something went into EE's eye.

Employee was connecting 16" pipe flange. While putting a nut on the bolt on the left side of flange, when the pipe shifted causing the middle finger on left hand to be pinched between the bolt and pipe.

Employees were using a winch to pick up an impeller when the chain got hung up on a bolt stud. The chain popped off of where it was hung at the same time as the employee put his hand onto the pipe to brace himself. At that moment the chain hit the pipe where the employee hand was resulting in lacerations to the left index finger.

During the process of replacing a guard on a conveyor, due to the weight and awkwardness of the guard, it slipped through the employee's hands causing lacerations to 2 fingers and a bone break to one finger.
Material was located on the step of the Cat 725 haul truck resulting in the employee slipping and spraining his right ankle.

Emp. was operating a Skid Steer moving material near the belts when the skid steer went forward into a hole. The equip went forward into the hole & then righted itself up with jerks until it stopped & rested correctly on the ground. Everything was fine until the employee got off the piece of equip 20 minutes later when he began to experience pain in his back.
Employee was tugging on a pry bar when he felt a burning in his lower back.

Employee was on the work boat with 2 other employees working on repairing the left swing dredge cable. During the course of the repair, the boat turned and the cable struck the employee in the face and knocked him to the deck of the work boat.
A Contracted electrician was installing a junction box to the wall and in his attempt to remove a cardboard box from underneath the existing junction box he was installing, he hit his right middle finger against the box cover causing a laceration requiring 10 stitches. He was wearing gloves at the time of the incident.
An Orkin Pest Control Technician had sprayed around the office trailer for bugs & was in the parking lot to leave the mine site. He slipped on a 1' x 5" mud section disguised by grass. When he fell he landed and made some noise. He was able to stand & walk lightly. He declined medical care & left. He ended up going to the Dr. X-rays showed a broken left leg. Surgery is scheduled.

Driver drove to the dump off pile, got out of his truck wearing PPE but no gloves, walked to the rear of the truck, opened the tailgate of the trailer with his hands & tried to get 200 lbs. of sand out of the trailer bed. When driver was attempting to ensure sand was not lodged between the tailgate & the bed, the tailgate latches closed on his right thumb crushing it.

The EE working in direct sun light without water or taking frequent breaks adding a joint to the dredge line. EE failed to follow instructions to come off the pond. He went home without properly notifing Supervisor and later that evening went to ER and was diagnosed with heat exhaustion.

EMPLOYEE WAS MOWING FIELD ON TRACTOR, HE BECAME VERY DIZZY & A NUMB SENSATION WAS FELT IN HIS EXTREMITIES. HE STOPPED & CALLED FOR HELP, AS HELP ARRIVED HE COULD NOT STAND & WAS HAVING TROUBLE SEEING. HIS SUPERVISOR TOOK HIM TO THE HOSPITAL, HE WAS DEHYDRATED.
EE WAS HELPING PLANT OPERATOR REMOVE BELT GUARD ON SECONDARY SHAKER. WHILE LIFTING GUARD OUT OF PLACE, EE'S FOOT SLIPPED ON WET SURFACE CAUSING HIS ANKLE TO ROLL, SPRAINING HIS ANKLE.

Ee was wrapping up & putting away his tools & equipment. He stated that he was extremely sweaty to the point it was running into his eyes causing burning & poor visibility. He lifted his safety glasses & wiped his eyes with his shirt sleeve & got something in his eye. He didn't realize it till next morning.
Ee was replacing return roller on gravel stacker. He was lying on the channel iron underneath the belting trying to loosen the bracket bolts with wrench. He gave it a sudden jolt to break loose the bolts and felt a pain in his upper right arm.
EE WAS ASSISTING ANOTHER EE REMOVE PULLING PUMP PAKCING WHEN HE PULLED A MUSCLE IN HIS UPPER LEFT ARM.

EMPLOYEE WAS BUSH HOGGING. BUSH HOG THREW AN OBJECT INTO RHE AIR FROM UNDER THE BLADE, HITTING THE EMPLOYEE'S LEFT LITTLE FINGER, BUSTED THE FINGER OPEN.
EMPLOYEE WAS CHANGING A SCREEN AT PLT 2 IN THE SHAKER. THE SCREEN BECAME WEDGED OR STUCK & EMPLOYEE USED HIS RIGHT HAND TO JERK SCREEN LOOSE. LOST USE OF HIS RIGHT ARM & HAND; AS THE RADIAL NE RVE QUIT WORKING. EMPLOYEE UNABLE TO LIFT HIS HAND AT THE WRIST.
EE WAS DIGGING SAND FROM AROUND THE PIPELINE TO REPAIR A HOSE THAT WAS IN THE PIPELINE. HE CUT HIS FINGER ON A SHARP EDGE ON THE PIPLELINE.
EE WAS CHANGING A HYDRAULIC HOSE ON A LOADER & PULLED A MUSCLE IN HIS NECK.

EE WAS GOING INTO 3412 ENGINE AREA TO IDLE THE ENGINE UP. STEPPED DOWN INTO THE HOLE & STEPPED ONTO A 2" ROUND DISCHARGE HOSE. THE HOSE ROLLED CAUSING EE TO TWIST LT. ANKLE.
EE MOVED A BOARD PROPPED AGAINST HIS TRUCK. HE SLID HIS HAND UP THE BOARD TO GET A BETTER GRIP. HE GOT A SPLINTER, APPROX. 2" LONG STUCK INTO HIS HAND, BETWEEN HIS THUMB & INDEX FINGER.
EE WAS SWINGING A HAMMER & HIT HIS LEFT HAND INDEX FINGER. THEN THE EE WAS ADJUSTING A SWINGLINE, PUCTURING THE SAME FINGER - LEFT HAND INDEX FINGER, IMBEDDING A PIECE OF STEEL CABLE INTO HIS FINGER.

EE & COWORKERS WERE TRYING TO PUT PIPELINE BACK TOGETHER. EE CRANKED OUTBOARD MOTOR TO MOVE PIPELINE. MOTOR JUMPED INTO GEAR. WHILE EE WAS TRYING TO GAIN CONTROL OF MOTOR, HE GOT HIS HAND CAUG HT BETWEEN PIPELING AND MOTOR, CAUSING A CUT TO HIS LEFT HAND THAT REQUIRED STITCHES.
WHILE LIFTING AN OUTBOARD MOTOR TO REPLACE IT, EMPLOYEE FELT HIS BACK CRAMP UP. EMPLOYEE REMOVEDMOTOR FROM WORKBOAT AND PLACED IT ON THE GROUND.

EE WAS PULLING PIPELINE INTO SLEEVE WITH THE WORK THE BOAT MOTOR WAS NOT LOCKED DOWN AND THE COVER WAS WHEN THIS MOTOR WAS ACCULTED THE MOTOR RECKLED AND STRUCK THE EE ON THE LEFT .
SKIRTING AROUND IMPACT BED ON STACKER WAS TOO CLOSE TO ALLOW EASY INSTALLATION OF A NEW TROUGHING IDLER. MAN WAS FORCING IDLER INTO POSITION WHEN IT FELL OVER SMASHING HIS RIGHT RING FINGER.
EE WAS REPOSITIONING THE SAMPLE SHAKER WHICH WEIGHS ABOUT 61 POUNDS WHEN HE FELT A POP IN HIS LEFT SHOULDER FOLLOWED BY HIS FINGERS BECOMING NUMB.
EE WAS REMOVING A SCREEN FROM A PILE OF SCREENS. IN DOING SO, HIS HAND SLIPPED CAUSING A CUT TO HIS LEFT THUMB.
