Dredge operator tripped on swing cable while stepping from dredge ladder to pontoon. Employee fell to the deck of the pontoon suffering a sprain/strain lower back.

Two miners were attempting to lift a conveyor belt scale that weighs approximately 60 pounds. One of the miners strained a muscle in lower back while lifting the scale.
Miner suffered a strain neck when ee was struck on the top of hard hat by a lump of dirt that fell from the top of an excavator bucket that was resting on the ground about to be greased. The injury was treated as first aid and after a week the injury did not get better. Miner sought medical treatment on 10/19/2016 and was placed on restricted duty starting 10/20/2016.

Employee stated he felt a pull and tightness in his left knee while climbing the stairs helping carry a metal strap. Did not want treatment until 20 days later, left knee sprain strain
Employee was removing a down spout from the chute under sand classifier when it slipped and pinched the tip of his left ring finger between the spout and the edge of the chute resulting in a laceration requiring stitches. Employee was wearing all PPE including gloves.

While helping guide a loader moving a 14 inch poly pipe the chain slipped over the loader bucket causing the pipe to fall and swept the EE feet and legs out from under him fracturing his left leg
8/13 EE was suspended further testing random drug test 8/20/12 HR called EE terminate for positive drug test. EE reported he might have carpal tunnel has not been treated or reported. 9/17 EE went to hand specialist 10/01/12 receive doctor report with diagnosis treatment plan would be no loss time but EE was terminated before reported injury
The Red River had been flooded for 2 weeks leaving mud along bank edge EE stepped of loader and sunk knee deep in the mud injured left knee twisting to get unstuck.

Employee, who is a truck driver, was walking near the scales toward the conveyor/ hopper when he tripped and fell to the ground. This caused him to hit his head resulting in a mild concussion and laceration to his head.
Employee was pulling a drive shaft bolt from under a loader, when his wrench slipped and hit him in the mouth causing a chip to his tooth.

Rigger Seven Injury was tying rigging onto catwalk from a manlift; Aother ee stepped on catwalk to ee's hand some rigging ; weld broke and catwalk hinged down hitting him in the right forearm, requiring 6 stitches.
EE was getting into on boat when his foot caught on the bottom of the boat causing him to fall forward, resulting in sprain of right ankle and dislocated carpal bone of right wrist.
Employee alleges pain in left arm from shoulder to wrist due to repetitive operation of controls on dozer.
Employee attempted to catch his fall after missing step on loader resulting in right arm strain. EE did not fall.

Employee was moving fuel stand when the pipe broke loose, pinching fingers between handle and pipe, resulting in crush injury to right middle and ring fingers.
EE was operating the dredge when he stood up to close the door, at which time the cutter head hit a log causing the dredge to bounce, slamming the door against employee's right hand.
