Amrize Stringtown Quarry Metal/Non-Metal

Maintenance employee was removing a tooth from the 992 loader. EE was striking the tooth with a hammer when a shard of the tooth flew off and struck EE on the inner thigh of EE's right leg resulting in the shard being embedded in EE's groin area.

A plant operator was removing a roller frame from CB-2B conveyor tail end. While pulling the roller out EE's grip slipped and resulted in dropping the roller and it landed on EE's foot approximately one inch above protective steel toe.

Water truck operator was cleaning the area around the secondary crusher with a water hose when a rock fell from above and struck the employees hard hat. The employee began to experience a headache and nausea. The employee was diagnosed with a concussion.

Maintenance worker was moving man lift from north side of plant to the south side of the plant. While traveling down a ramp in the lift a hydraulic hose failed causing the operator to lose control of the lift. The man lift fell over resulting in the operator suffering a laceration to the left shin and a strained shoulder.

An employee was in a man lift assisting with installing a new conveyor belt. As the new belt was being pulled onto the conveyor it began to slip off and the employee reached out to grab the belt to keep it from falling to the ground. The employee injured left bicep.

An employee was standing under the rail load out conveyor. A frozen piece of material fell from the frame of the conveyor and struck the employee on the hard hat knocking employee to the ground. The employee suffered serious neck and back injuries.

Plant operator walked into the office and told office workers they had a pain in shoulder. EE was going to get the shoulder checked out. Employee drove to local hospital. Medical staff told employee they were having a heart attack and rushed employee to hospital in the city.

Electrician was performing electrical portion of installing a new rock hammer at the primary crusher. As the electrician was removing the motor starter of the old rock hammer an arc flash occurred. The electrician suffered 1st and 2nd degree burns to face and arm.

Employee was operating the water truck when the front left tire blew out. The window was down when the tire blew. The sound of the blow out caused pain in employee's left ear.

Plant operator was using a hand held grinder to cut the bearing housing off the tail pulley shaft. The grinder kicked back and came into contact with the employees left foreman. The employee suffered a laceration that required 16 sutures to close.

Two employees lifted a metal gear box to discard it in a metal waste bin. One of the employees got their right middle finger pinched between the gear box and waste bin. Employee suffered a laceration that required 3 sutures.

Employee was adjusting the forks on a skytrack and pinched finger between the fork and mast of the skytrack.

Employee was helping with installing stairs on primary crusher. Employee was pulling on stairs to get them aligned so the stairs could be welded in place. While pulling on stairs employee felt a pop in the right shoulder. Employee suffered a sprain right shoulder.

Employee went to sit in a chair . The chair malfunction and the employee and chair fell backwards resulting in a strain to the right elbow

Employees were moving a 55 gallon drum of oil from containment area. The barrel slip from the ledge of the area causing the dolly to strike EE in left hip and straining his lower back. No treatment was sought until July 7/8/2015. EE was evaluated 7/24/2015 MRI results 08/04/15 prescription meds and physical therapy changed first aid to medical.

EE did his vehicle inspection went to climb in the loader when he reached up to grab the hand rail he felt a pop in right shoulder and a slight pain about and hour later it was slightly burning and sore. He reported the incident and refused medical treatment. He treated under his medical insurance until he needed an MRI.

Workers were using a crane and tag line to remove old conveyors when the structure holding the bottom section of conveyor fell striking the employee holding the tag line in the right shin. EE had laceration to right shin requiring 9 stitches

EE was attending annual refresher training stepped outside during break and was stung by a bee. EE allergic to bee stings

EE was on the crane by the spool and the cable broke causing him to lose his balance so he jumped to the ground instead of falling. He fractured his right heel

Employee was told by treating physician that she had a work related injury. The diagnosis was carpal tunnel to the left wrist. Medical attention was sought 12/2010 received diagnosis 05/10/2011. Reported to supervisor 05/13/2011.The result of repetitive operation of a Loader. She stated that she will need surgery in July or August.

Employee turned on the a/c unit on a piece of equipment when dust from the vents blew into his eye resulting in irritation and an abrasion.

On 2/9/10 a water valve broke causing water to push employee in a low spot in the ground resulting in a knee strain. At the time he denied medical treatment. On 10/15/10 employee requested to seek medical treatment and was taken off work.

Employee was hooking a clevis to a loader bucket when a rock fell from the loader striking the employee in the right eye resulting in a corneal abrasion.

Employee was using a pneumatic impact wrench to tighten a bolt. He used his left shoulder to lean against the wrench for more leverage. This resulted in pain to his shoulder and chest. After seeing a medical professional this injury was not MSHA recordable; however the employee wished to get a second opinion at which time it became reportable.

Employee was using a prybar to install a roller on a dozer when he smashed his thumb between the roller and the equipment. At the time, the employee did not seek medical attention. It was not until 10/23/09 when swelling was apparent in his thumb that the employee wished to see a medical professional.

Employee was assisting in an oil change on eqipment when a hose fell and the nozzell hit him on the right hand resulting in the dislocation of his 3rd knuckle.

Employee was working on a motor when a pulley slipped and landed on his left hand causing a laceration to his left index finger.

Employee was stepping off a piece of equipment when his foot landed improperly resulting in a sprain to left ankle

EE was operating a loader on 2/23/09 when she hit several potholes causing strain to her neck. On 2/27/09 while operating a loader, again she hit several potholes causing strain to her neck. EE did not report injury to supervisor until 3/4/09.

Employee burned his eyes due to a welding flash.

Employee alleges chest wall strain as a result of pulling himself up onto the Hitachi 1200 shovel.

After welding bed of haul unit, employee was descending ladder on back on unit when the ladder slipped causing employee to fall to concrete floor, injuring back.

Employee has pain in right hand/wrist and left shoulder due to repetitive operation of front-end loader.

Employee alleges lower back strain resulting from releasing hand brake on engine. EE did not report alleged injury until 9/13/07.

EE was using a hydraulic jack to place metal liner up against crusher wall when the jack slipped hitting employees left forearm.

While operating haul unit, rear-end came loose from frame of unit causing unit to suddenly stop, resulting in strain/contusion to employee's abdomen caused by impact of seat belt.

Employee was climbing down off of shovel and stepped on rock, resulting in sprain of left ankle.

EE was hooking up hoses in water pit which resulted in alleged back pain.

EE WAS ATTEMPTING TO MOVE ELECTRICAL PLUG WHEN HE BENT DOWN HITTING LEFT KNEE ON CORNER OF METAL RACK, RESULTING IN CONTUSION/ POSSIBLE FRACTURE OF KNEE.

WHILE POURING SECONDARY PIT, EMPLOYEE'S LEGS CAME INTO CONTACT WITH WET CONCRETE, RESULTING IN CONCRETE BURNS.

WHILE WALKING ON UNEVEN GROUND SURFACE EE TWISTED RIGHT KNEE.

EMPLOYEE ALLEGES NECK STRAIN AS A RESULT OF RUNNING LOADER INTO A ROCKPILE TOO HARD.

EMPLOYEE STRAINED LEFT GROIN AREA WHILE CLIMBING UP AND DOWN THE HAUL TRUCK.

EE WAS INVOLVED IN CONSTRUCTION OF NEW CONVEYOR WHEN HE SLIPPED & FELL APPROX 4' TO THE GROUND RESULTING IN FRACTURED LEFT WRIST.

EMPLOYEE STATES THAT CLIMBING ON/OFF THE CRANE TO REPAIR SHIFTING MECHANISM RESULTED IN STRAIN TO LEFT KNEE.

EE LIFTING SCREEN TO PLACE IN LOADER BUCKET WHENHE FELT PAIN IN LOWER BACK.

AS THE EMPLOYEE WAS CHANGING THE SCREENS ON A SHAKER, HE CUT THE PALM OF HIS RIGHT HAND ON THE EDGE OF THE SCREEN HE WAS PULLING OFF. NOTE: WORKGLOVES WERE BEING WORN AT THE TIME OF THE INCIDE NT.

EE STRAINED MID-BACK WHILE CARRYING METAL BAR UPTHE STAIRS.

EE WAS HAMMERING A RIPPER TOOTH ON DOZER WHEN A PIECE OF THE TOOTH (STEEL) BROKE OFF AND EMBEDDED RIGHT THIGH.

EMPLOYEE WAS OPERATING A MIG WELDER WHEN THE SLAG BURNED THROUGH HIS GLOVE CAUSING 2ND DEGREE BURNS TO LEFT HAND.

EMPLOYEE WAS ATTEMPTING TO INSTALL A TAILPULLEY WHEN IT SLIPPED, CAUSING RIGHT LITTLE FINGER TOBE CAUGHT BETWEEN SHAFT AND CONVEYER FRAME, RESULTING IN LACERATION OF FINGER.

EMPLOYEE WAS OPERATING CUTTING TORCH FRO AN EXTENDED PERIOD OF TIME RESULTING IN STRAIN TO HIS NECK AND LOWER BACK.

EE STEPPED DOWN OFF OF RAILCAR ONTO TRACKS AT WHICH TIME HE SPRAINED HIS LEFT ANKLE.

THE WELDING TRUCK WAS BROUGHT TO THE SHOP AREA AND PARKED. THE MAINTENENACE CREW WENT TO THE WATER PUMPS AND DRAINED FOR THE NIGHT. UPON RETURNING TO THE SHOP AREA EE ROUNDED THE CORNER OF THE BLDG AND DISCOVERED THE WELDING TRUCK WAS ON FIRE. THE CREW TRIED TO PUT THE FIRE OUT WHILE SUMMONING THE STRINGTOWN FIRE DEPT. ALL FLAMES WERE OUT WITHIN 25 MINUTES OF STARTING. FIRE BURNT

EMPLOYEE WAS CHANGING A ROLLER ON THE CONVEYER WHEN A DIRT PARTICLE EMBEDDED INTO LEFT EYE.

EE WAS WALKING DOWN STEPS ON SHAKER SCREEN WHEN HE STEPPED IN A HOLE AND FELL ON RIGHT HIP.

EMPLOYEE WAS CLIMBING INTO A TRUCK WHEN HIS RIGHT FOOT SLIPPED, RESULTING IN A CONTUSION OF THE RIGHT KNEE.

EE BENT OVER TO PLACE LID ON WATER CAN WHEN HE FELT PAIN IN BACK.

EE WAS CUTTING A PIECE OF BELT WITH A KNIFE WHENTHE KNIFE SLIPPED RESULTING IN LACERATION TO HIS RIGHT INDEX FINGER.

EE STEPPED ON A PIECE OF METAL AND HIS FOOT SLIPPED, TWISTING HIS RIGHT ANKLE.

EMPLOYEE WAS DRIVING TRUCK AND HIT CHUG HOLE CAUSING STEERING WHEEL TO SPIN, HITTING RIGHT HAND.

EE WAS CHANGING A ROLLER IN THE IDLER STAND ON CRUSHER BELT & HIT HIS THUMB WITH A HAMMER.

EE WAS STANDING ON THE BUMPER OF HIS GREASE TRUCK, WHEN HE STEPPED DOWN, HIS FOOT HUNG CAUSING HIM TO TWIST HIS KNEE.

EE WAS WORKING ON A TRANSMISSION FOR AN END DUMP. HE HAD APPLIED AIR PRESSURE TO SEE IF THE VENT BREATHER WAS OPEN. HE SAID NO MORE AIR WAS COMING OUT BUT WHEN HE UNCOVERED THE FILL PIPE IT BL EW HOT OIL ON HIM.

INJURED SLID HIS HAND DOWN THE GUARD OF THE TAILPULLEY WHEN HE STUMBLED, CAUSING A CUT TO HIS LEFT HAND, REQUIRING STITCHES.

EMPLOYEE WAS BITTEN BY A SPIDER & DEVELOPED AN INFECTION

EE CALLED IN AT NOON TO REPORT THE ACCIDENT HAD HAPPENED THAT MORNING. HE STATED HE WAS CHANGING A HYDRALIC HOSE ON A LOADER WHEN HE SLIPPED AND FELL. HE DID NOT GO TO THE DR. UNTIL ONE WEEK A FTER THE ACCIDENT WAS REPORTED.

EMPLOYEE WAS DISTRACTED BY A TRANSMISSION PROBLEM AND WAS NOT WATCHING THE ROAD. HE RAN HIS TRUCK UPON A BANK ON THE SIDE OF THE ROAD.

EMPLOYEE WAS REPLACING TRACK ROLLERS ON A DBL DOZER & STRAINED HIS BACK. WE DID NOT REPORT TO FOREMAN UNTIL SEVERAL DAYS AFTERWARD.

EE HAD REACHED DOWN INTO THE JAW CRUSHER TO FREE A ROCK THAT WAS JAMMED, WHILE SWINGING A SLEDGE HAMMER HE STRAINED HIS BACK.

EE SAYS HE SLIPPED OFF THE BACK OF HIS FUEL TRUCK FALLING TO THE GROUND, HURTING HIS BACK. NO LOSS TIME REPORTED AT THIS TIME BUT HAS BEEN PLACED ON RESTRICTED DUTY.

EMPLOYEE WAS USING A 2" WATER HOSE TO WASH MATERIAL OUT FROM UNDER A CONVEYOR WHEN HE LOST HIS BALANCE AND FELL BACKWARD AGAINST ANOTHER CONVEYOR. HE DID NOT REPORT THE ACCIDENT UNTIL THE NEX T WEEK WHEN HE WENT TO THE DR ON THE 24TH OF AUGUST. THE REPORT WAS A SPRAIN TO HIS LEFT HIP. THIS ACCIDENT IS UNDER INVESIGATION BY PROPER AUTHROITIES.

EMPLOYEE WAS STEPPING OFF A LOADER, WHE HE FELL LANDING ON HIS LEFT FOOT.

EMPLOYEE WAS SWINGING A SLEDGE HAMMER WHILE CHANGING THE THEETH ON A LOADER & STRAINED HIS BACK

EMPLOYEE WAS CHECKING GROUND AFTER A SHOT IN THE QUQRRY HE WALKED ONTO GROUND THAT APPEA RED TO BE SOLID BUT CAVED IN UNDER HIM ALLOWING HIM TO FALL INTO A CREVICE WRENCHING HIS BACK & SCRAPIN G HIS LEG (NO LOSS TIME AT THIS TIME)

EMPLOYEE WAS PUTTING HYDRAULIC OIL INTO COMPANY EQUIPMENT WHEN HOSE REEL ON GREASE TRUCK CAME UNLATCHED REEL IS SPRING LOADED & YANKED THE SLACK FROM THE HOSE VERY SUDDENLY CAUSING WHAT DR DIA GNOSED AS A STRAINED SHOULDER EMPLOYEE RETURNED TO WORK BUT AFTER NO RECOVERY BY THE 18TH OF SEPT SAW DR WHO DIAGNOSED IT TO BE A TORN ROTATOR CUFF

I.NAME WORKING ON A LOADER ENGINE STANDING ON A LOADER TO LOOSEN THE HEAD BOLT.WRENCH SLIPPED CAUSING LOSS OF BALANCE AND FALL 6' TO GROUND.LANDED ON AND BROKE LEFT HIP.

EE WAS CHANGING A SCREEN ON THE 6 X 16 SHAKER OF THE CRUSHER. HE WAS CLIMBING OUT OF THE DECK AND TWISTED WRONG CAUSING PAIN IN HIS BACK.

DESCENDING STEPS FROM BELT STEPPED ON STONE CAUSING HIM TO FALL IN STEPS BRUISE R LEG

HE AND ANOTHER EMPLOYEE HAD REPAIRED A WHEEL LOADER TIRE AND WAS REPLACING THE MOUNTED TIRE ON THE LOADER. HE INCURRED A BACK STRAIN AT THIS TIME. EMPLOYEE NOTIFIED THE COMPANY ON 11-2-90.

GOING TO SERVICE DRILL L FRONT TIRE OF TRUCK STRUCK ROCK, TRUCK TURNED TO HIGH WALL HE JUMPED OUT OF TRUCK

EMPLOYEE WAS ENTERING THE CAB OF THE TAMROCK DRILL, AND ACCIDENTALLY SLAMMED THE DOOR ON HIS THUMB.

EMPLOYEE WAS STANDING BESIDE E.NAME, WHO WAS DRIVING A BRARING OFF OF THE SHAFT ON THE EXHAUST FAN, WHEN A PIECE OF STEEL FLAKED OFF THE HAMMER, STRIKING EMPLOYEE ON THE RIGHT FOREARM.

I,NAME WAS INSIDE THE BIN UNDER THE SCREEN, HOLDING THE NUTS FOR THE CENTER STRAP TENSION BOLTS. THERE IS APPROX. 8 FT CLEARANCE, SO EMPLOYEE WAS SITTING ON A ROD ABOUT 5 FT. FROM THE FLOOR, F OR EASE OF REACHING THE NUTS. HE LOST HIS BALANCE AND FELL, BREAKING HIS FALL WITH HIS LEFT ARM.

I.NAME WORKING ON CRANE STEPPED DOWN OFF MACHINE ON 3" DIA SHAFT THEY HAD JUST REMOVED.

EMPLOYEE FELL DOWN ON STEP OF ASPHALT PLANT.

EMPLOYEE WAS USING A DISK GRINDER, GRINDING ON AN ALTERNATOR BRACKET THAT HE HAD CLAMPED IN THE VISE, IN THE SHOP, AT ABOUT CHEST LEVEL. THE GRINDER KICKED BACK STRIKING EMPLOYEE ON THE LEFT W RIST.

THE INJURED AND ANOTHER EMPLOYEE WERE IN THE PROCESS OF SPLITTING WOOD WITH A HAMMER AND WEDGE.THE OTHER EMPLOYEE STARTED THE WEDGE IN A BLOCK OF WOOD WITH THE HAMMER,IT CAME OFF STRIKING THE INJURED ON THE FOREHEAD.LACERATION ON HIS FOREHEAD,ABOVE HIS LEFT EYE.

EMPL WAS REPAIRING THE RAILROAD TRACK AND WHEN HE STRUCK A SPIKE TO DRIVE IT INTO THE TIE IT RICODHETED OFF THE RAIL STRIKING HIM IN THE MOUTH

EMPLOYEE WAS HELPING CLEAN UP AROUND THE CRUSHER AND PULLED A MUSCLE IN HIS BACK WHILE LIFTING AN OLD SCREEN.

EMPLOYEE WAS HELPING INSTALL A 6 IN.WATER LINE WHENHE STEPPED OVER A MOUND OF DIRT & STUMBLED TWISTING HIS KNEE

EMPLOYEE WAS WORKING UNDER A JAW CRUSHER WHEN A ROCK DISLODGED STRIKING THE EMPLOYEE ON THE FOREHEAD.

EMPL OPERATING 7AMROCK DRILL MOVING IT FROM ONE HOLE TO ANOTHER.DRILL MOVED OVER MOUND OF ROCK IT PITCHED FORWARD,HE JUMPED OFF PLATFORM AND WRENCHED HIS BACK.

EMPLOYEE BECAME3OVERBALANCED IN A SQUATTING POSITION, FALLING AND REINJURYING RIGHT HIP JOINT.

EMPLOYEE WAS DRIVING AN END DUMP TRUCK, TWISTED HIS KNEE WHEN HE CAME DOWN THE LADDER

TRUCK DOOR HANDLE BROKE EMPLOYEE FELL FROM TRUCK, SPRAINING HIS BACK
