While assisting in barge movement on the lower dock, an employee was struck by a 2" Three Strand twisted polypropylene rope snapback after the rope failed under tension. The force of the line propelled the employee's arm into their chest and mouth, resulting in a fractured tooth requiring a crown, along with contusions and to the lower arm and hand.

While offloading material from a haul truck, a hydraulic cylinder broke, causing the bed to fall unexpectedly. The employee suffered a strained neck and was initially treated with First Aid. After five days, they sought medical attention, were diagnosed with an irritated nerve, and returned to work with restrictions.
Employee was removing screens (est. weight of approximately 70 lbs each) from the manlift when the screens began to slide. While attempting to prevent them from falling, the screens slid across the employee's wrist, causing a laceration that required 8 stitches. The employee was returned to work with medical restrictions
While dismounting from the driver's side of a water truck, the employee missed the bottom step and fell backward, landing on their backside. Initial injuries included a scrape to the arm and leg and back soreness (first aid). On 2/11, they reported neck stiffness, received medication, and were recommended physical therapy starting 2/21. Work status: No restrictions.

Employee was riding in a side by side utility vehicle that flipped over on its side and EE sustained an injury to their right knee. EE was wearing the seat belt at the time of the incident
Employee struck their left thumb with a hammer while tightening a bolt with a slug wrench.
The employee was washing at the plant station with a 1.5" water hose when the employee's left forearm began to swell. Due to a pre-existing personal injury, they had recently been released to full duty.
Employee was helping a customer hook up a water hose on the water truck and their shoulder began hurting. Employee did first aid. Employee was then seen at a clinic. F/U to clinic employee was given orders to start physical therapy on 7/30/24.

A 777 haul truck was getting loaded by a 992 loader when a rock hit the bed hard enough to make the driver bite their tongue and pop their back.

Employee alleges to being bent over using water hose to clean beneath CV46 when felt sharp pain to lower back.
Employee was working on One Jaw Crusher when the left hand was pinched by the shims. This resulted in two broken fingers and required surgery.
(First aid) Employee misplaced their footing and fell while reaching above their head for bolts. 4/21/2021 Became medical due to starting prescription medication.

While loading haul truck boulder caused rocking motion of truck, resulting in restricted duty injury to the haul truck driver.
Employee was walking in the plant and twisted the left knee.
Lifting tool box (40 pounds) - modified work days injury to back
Employee struck chin against handrail resulting in a laceration to the chin.
EE was changing out roller cans on CV-23. EE bent over to be able to get between the belt and the roller and was extended to the other side of the conveyor frame. When trying to grab the roller can it was still stuck. When it finally broke loose and the weight of the can transferred to the injured employee's arms, pain was felt in the groin area.

As injured employee was walking EE wasn't watching EE was going and EE's knee bumped the sledge hammer handle that EE had previously placed on the floor adjacent to the track mobile. The injured employee grabbed for the hammer handle to prevent it from falling to the floor and the same time the right ankle turned inward.
Employee was removing rock from a spill from a plugged chute when chute was free a rock rolled off from the chute hitting employee in the top of the hard hat. It became a lost time on Aug 16, 2016
EE was working outside using a 20lb sledge hammer to loosen material that was stuck inside the chute. EE went in to cool off. About 10 minutes after EE tried calling to get ahold of someone because EE stated was cramping up and skin was hot clammy and EE was shivering.
Employee put boots on in morning noticed bug bite at the end of the day at home.

Employee was underneath a truck making repairs and his hard hat fell off - when employee reached to retrieve his hard hat he bumped his head, causing a small laceration to his head.
Employee using a 2" water hose - Pulled hose to remove a kink from it and the released energy from the hose knocked employee off balance causing her to fall.

An Employee was attempting to remove a 36lb submersible pump from a hatch on the upper dock. As the employee was pulling the pump up, it hung on a ladder rung. The employee pulled on the pump a couple of times to try and force the pump loose, which caused the employee to have a back strain.

An Employee was starting mobile equipment on the ready line. After starting the first 992G loader he suffered a heart attack.
An Employee was repairing a hydraulic pump. Once it was repaired, the employee picked up the hydraulic pump to put it in place. Two days later the employee reported his upper back was hurting from lifting the pump.

Employee was prying snap deck panel loose from the screen with a screw driver. The screw driver slipped and struck him just above the left eyelid, making a small cut above left eye. The employee was wearing the required eye protection at the time of the incident.
Employee was operating a man-lift when he drove over a rock causing the employee to loose his balance. The employee hit his shoulder on the handrail resulting in a contusion. Reported on 8/21/2009, became reportable on 4/9/2010 when the doctor took him off of work.

Employee was standing under conveyor attaching a nylon sling to the belt that was tangled around the framework of the take up pulley. A piece of material fell from the framework of the conveyor and struck them in the back of the hardhat.
Employee was loading rollers into the back of a pick up. While loading he struck the tail gate post that locks the tail gate in the closed position with his left pinkie finger causing a laceration.
Employee was walking down 5-dock ramp to boat, when he stepped on a patch of ice, causing him to fall onto his right hand and forearm.

Employee was walking between MCC5 and LPT system water tanks when he stepped into a hole full of slurry. At the time of the incident he did not think he had an injury. Reported to company as injury on 10/23/07, doctor released to full duty at that time, became reportable when doctor took him off work on 11/13/07.
Employee was standing on rope stand platform, throwing a swing line rope to another employee. His left foot slipped off the platform, onto the first step, twisting his back.
Employee was climbing into grader. He placed his right hand on the door frame and was inspecting the grease on the table when the wind blew the door shut on his right hand.
Employee was attemting to clean Ultrasonic at the 7 ft. cone crusher. His right middle finger was caugh between a bolt and the motion sensor, causing a laceration.
Employee was climbing ladder to haul truck and felt a sharp pain in his lower back. Delay in reporting due to misdiagnosis by doctor. Doctor stated the pain was caused by a fractured vertebrae from a fall. The employee had not fallen and went for a second opinion where a pinched nerve consistent with the activity was diagnosed making the injury reportable.
Employee was unhooking the haul cable from a loaded barge, when the cable was unhooked and the winch operator began to raise the cable the cable hung under the cavel. When the cable became tight it sprang from under the cavel and struck the employee in the left thigh.

Moving pipe threader, felt pain in knee.
Carrying load down steps, when knee popped, causing pain in right knee. This was 1st aid until 5-16-06 when EE wanted to have DR. check it. This company was notified of this on 5-16-06.
EE was checking work at area #46 conveyor hand rails and walk ways when a section of grating measuring 24 1/2 x 67' fell through after apparently slipping out of place. This resulted in a fall of 15'3" to ground.
Employee fell from scaffolding during dismantling process.
Slipped on welding blanket in screen, extended body to catch fall, struck back of neck on beam above.
Co-worker pushed concrete chute striking employee on the opposite side of chute causing him to fall off 4' high pier. Employee worked until Jan. 16, 2006. Began missing work on Jan. 17, 2006.

Employee was pulling extension cord while grinding and tripped over the cord and fell.
While using handheld rock drill, at chest height, the bit broke. Drill fell to the ground & EE hit his right elbow on top of broken drill steel in the rock. The skin was not broken & EE only reported his elbow was "sore". EE worked without interruption for more than 2 months. Later the EE had swelling around the area and went to Doctor's office.
Changing screen cloth on the middle deck of the station 3 Diester. Employee laid a chipping hammer on the deck in front of him. Another employee then slid the screen cloth down to him so it could be put in place. The screen hit the chipping hammer and it popped up hitting employee in the forehead.
Two men, Other ee and EE were picking up rebar to stand up vertical got out of baance and fell back.
Employee was working on a screen deck in the tower of the #1 plant. His right hand started to itch, he thought it may have been a mosquito bite but it later began to swell.
Employee was being lowered in manbasket. Basket stopped suddenly. Employee was standing with legs crossed lighting a cigarette when basket stopped causing him to fall in squatted position. Knee struck basket interior causing injury.
Employee was using a utility knife to cut the end of a fire hose and the knife slipped cutting his right index finger.
Employee was checking a flex conduit at 2 jaw. He had his hand on a beam and a rock fell from above. The rock hit his left index finger causings a cut.
Employee was climbing access ladder on a WA800 front end loader when he fell to the ground. Employee right foot slipped on second rung from cab platform and he lost his grip with right hand. He swung out to the left while left foot and left hand still on loader when he lost his grip and fell an estimated 8.5 feet to the ground landing on his right side.
Employee was turning the water on at the water stand pipe so he could wash the bed of the truck bed. When he bent over to turn the water valve on there was an aluminum valve wrench hanging over head approximately six foot off the ground. The wrench fell striking the employee's hardhat and back of his head.

Employee was loading scrap metal on trailer. Had just loaded a 10" steel pipe with boom truck and disconnected lifting cable. Employee was concerned that pipe might shift during transport and roll off trailer. Employee repositioned pipe and mashed his right ring finger.
Ee was performing work on the crane on the lower dock. He and a co-worker were changing over from the claw to the bucket. After attaching the tagline to the bucket, ee tugged on the tagline to tension it up with the boom. The resulting tension sucked his hand into the cable guide (bell), and cut his hand.
Employee was replacing the drill steel in the drill and the end of one drill steel rolled over the top of his foot.
Employee was stepping over a water line that was approximately 4" off the ground. Employee's right foot caught on the water line and he stumbled and while trying to catch himself with his left foot, he sprained his left ankle.

EMPLOYEE WAS OPERATING A WATER TRUCK AND WHILE DRIVING THE TRUCK OVER A PERIOD OF TIMME HIS LOWER BACK AND LEG BEGAN TO HURT. THIS DID NOT BECOME REPORTABLE UNTIL AUGUST 4, 2003.
EE WAS USING A STEAM CLEANER TO CLEAN DRILL. HE TURNED TO THE LEFT TO PULL SOME SLACK IN THE HOSE. WHEN HE TURNED AROUND, THE END OF THE NOZZLE PASSED OVER THE TOP OF HIS FOOT. THE WATER PRESS URE CUT A HOLE IN THE TOP OF HIS BOOT, & THE HOT WATER BURNED THE TOP OF HIS FOOT.
EE WAS ASSISTING THE MECHANIC INSTALL A DRIVELINE IN A 100 TON HAUL TRUCK. WHILE INSTALLING THE DRIVELINE EE STRAINED HIS BACK.

EE WAS WASHING DOWN A CRUSHING PLANT & HIS LEG STARTED BURNING & HURTING. WHEN HE GOT HOME HIS LEG WAS RED & HE HAD A BROWN SPOT ON IT. HE WENT TO THE EMERGENCY ROOM & WAS TOLD THAT HE HAD BEE N BITTEN BY A SPIDER.
EE WAS WASHING WITH A 1 1/2" WATER HOSE, AND HITA PILE OF ROCK WITH THE STREAM OF WATER, AND THEOVERSPRAY WASHED DEBRIS OVER HIS SAFETY GLASSES AND INTO HIS EYE.
EE WAS REMOVING THE AUGER @ LIME MILL. THE BOOM TRUCK WAS ATTACHED TO AUGER TO LIFT WITH. THEY ATTACHED A COMEALONG TO ONE END OF AUGER TO PULL WITH. EE REACHED TO MOVE THE CHAIN ON COMEALONG & AUGER SLIPPED STRIKING HIS ARM.
EE WAS DRIVING A TRUCK & THE BEADED SEAT COVER KEPT PRESSING AGAINST THE KILL SWITCH BEHIND THESEAT. HE REACHED BEHIND THE SEAT TO CLEAR THE SWITCH & ACCIDENTLY STUCK MIDDLE FINGER ON RT. HAND IN FAN RESULTING IN 12 STITCHES TO FINGER. PUT ON ANTIBIOTICS FOR INFECTION & PAIN RELIEVER(UNABLE TO WORK FOR 2 WEEKS).
EE WAS PUTTING A HYDRAULIC PUMP ON MICHIGAN LOADER WHEN HIS WRENCH SLIPPED OFF BOLT, RESULTING IN HITTING HIS LEFT ELBOW, BRUISING MUSCLE. DR. PUT ARM IN SLING FOR 2 DAYS AND GAVE AND ANTI-INF LAMMATORY DRUG.
