THIS PARTICULAR DAY THE PLANT WAS SHUT DOWN FOR MAINTENANCE AND REPAIR AND WAS NOT DUE TO BE OPERATED THE WORKER REMOVED THE GUARDS ON THE TAIL PULLEY SECTION OF THE CONVEYOR THREW THE MAIN S WITCH TO THE PLANT AND STARTED THE CONVEYOR WITH THE CONVEYOR ON HE ATTEMPTED TO USE A WIRE BRUSH ON THE TAIL PULLEY THE BRUSH AND HIS HAND BECAME TANGLED RESULTING IN THE LOSE OF HIS LEFT A
