Employee was grinding a metal surface in preparation for welding. While wiping dust from his forehead, dust entered his right eye and caused an abrasion of the cornea.
Employee was changing the motor mount on an L-220D front end loader. While tightening the last bolt, employee slipped off the 3rd step of a ladder & struck the FEL battery box with his rib cage, fracturing ribs on his left side.

Employee was using a metal pipe to dislodge solids from the bottom of a sweeper truck refuse tank. The little finger on his right hand got caught in between the pipe and tank, was struck by the pipe, causing a laceration.
Employee was climbing down a ladder on a FEL. His left foot slipped off the bottom step, with his knee catching the step, causing his knee cap to push upward. Injury is inflammation of the left knee cap area.
Employee was pressure-washing a front-end loader. He slipped off the fender of the loader, falling onto the tire and loader lift arms. Injured left shoulder & right hand.
Employee was carrying a 5-gal bucket and bending over while passing under a belt conveyor structure. Suffered muscle strain to lower back and hip.
Employee was exiting from a front-end loader. While climbing down the ladder, his right foot slipped off the bottom step & his right knee cap struck the ladder step. The employee suffered a sprain to the right knee.

EE was using a utility knife to cut fiberglass pipe insulation. The wind carried the insulation into ee's face. Symptoms were not immediate. A few days later, ee's eyes became swollen and he experienced respiratory discomfort.

Employee was holding hand wrench while another employee was attempting to remove nut from a bolt. The hand wrench slipped & spun around, hitting the injured employee in his right hand, resulting in a sprain of the right hand.
Employee was lifting & pulling with heavy wrenches, resetting the die and rollers on Mill #3. Employee strained upper & lower back.
EMPLOYEE WAS ASSISTING A WELDER BY HOLDING THE METAL PIECE TO BE WELDED. ALTHOUGH ATTEMPTING TO LOOK AWAY DURING THE WELDING PROCESS, THE EMPLOYEE LOOKED AT THE WELD SEVERAL TIMES WITHOUT THE PROTECTION OF APPROPRIATE WELDING GOGGLES OR GLASSES. HIS EYES BEGAN TO STING/BURN SEVERAL HOURS LATER ("WELDER'S FLASH").

EMPLOYEE WAS HOLDING A BOX-END WRENCH WHILE REMOVING NUTS FROM BOLTS WITH AN IMPACT WRENCH. WRENCH SLIPPED ON AND OFF, CAUSING HANDS TO VIBRATE.
EMPLOYEE WAS USING A POCKET KNIFE TO REMOVE ELECTRICAL WIRES TO TAKE EQUIPMENT OUT OF SERVICE. INDEX FINGER ON RIGHT HAND WAS CUT.

EE WAS PERFORMING MAINTENANCE ON COAL CENTRIFUGE. TORQUE WRENCH SLIPPED OFF BOLT AND CAME DOWN ON THUMB, PINNING THUMB BETWEEN WRENCH AND MACHINE GEAR BOX, RESULTING IN DISLOCATION OF RIGHT TH UMB.
EE HAD TRANSPORTED ANOTHER EE FROM THE DREDGE TOTHE POND DOCK BY BOAT. DOCK ROLLED OVER AND EE JUMPED INTO THE BOAT. BOAT TIPPED FORCING EE INTO WATER. EE THEN GOT BACK ON SHORE AND FELT A THR OBBING PAIN IN LOWER BACK & TESTICLE AREA A FEW HOURS LATER.
EE SLIPPED ON WET FLOOR OUTSIDE THE ENTRANCE INTO THE ELECTRICAL ROOM, FELL ON HIS BACKSIDE AND STRAINED LOWER BACK AND HIP AREAS.

EE WAS USING A WRENCH TO OOSEN A NUT ON A CONDITIONER PADDLE. THE WRENCH SLIPPED OFF THE NUT AND STRUCK THE EMPLOYEE ABOVE THE RIGHT EYE, CAUSING A LACERATION ABOVE THE EYE AND OUTSIDE OF THE RIGHT EYE.

EE WAS CLIMBING UP LADDER ON SIDE OF COAL TRUCK BED TO GET COAL SAMPLE. AS HE WAS CLIMBING BACK DOWN THE LADDER, HIS RING ON HIS LEFT HAND CAUGHT ON ONE OF THE RUNGS, CUTTING HIS FINGER.
