At roughly 12:30 am on Saturday 7/11/26 employee was overcome by clean coal while shoveling spillage out from bottom of Clean Coal Silo. Employee is currently in hospital. Vitals are stable but final outcome still TBD. ***EE passed away 07/20/2026***

Employee was setting a post at 24xcut in 7 South #6 track entry, to barricade off an area of concern. While tightening up a post a piece of rock dislodged from the upper portion of the rib and fell striking the employee. The employee suffered fatal injuries as a result of this event.

Employee was in the process of setting or had set a roof jack, on the left inby corner or #60 Crosscut between the No 2 and No. 1 entry in the 4 west miner section. A piece of stone fell out of the roof and struck the employee, pinning him to the ground.

AT THE BUNKER TAIL SHAFT AREA, MECHANICS WERE GETTING READY TO TAKE SLACK OUT OF "B" CHAIN TO PULL SLACK WHEN A WELD THAT HELD THE TENSION ARM IN PLACE BROKE. THE RELEASED TENSION ARM CAME UP THROUGH THE STEPS WHERE THE VICTIM WAS STANDING CAUSING SEVERE LACERATION TO FEMORAL ARTERY IN HIS GROIN AREA.

17B LONGWALL WAS IDLE FOR MAINTENENCE & REPAIRS. A RAM JACK WAS BEING TRANSPORTED THROUGH THE STAGELOADER ON THE CHAIN CONVEYOR. THE EMPLOYEE STEPPED ONTO THE STAGELOADER CHAIN TO CHECK THE PO SITION OF THE JACK. THE STAGELOADER CHAIN STARTED & HE WAS PULLED INTO THE STAGELOADER CAUSING THE CRUSHING INJURIES.

Located at 6 South 100 Wall the employee was in the process of grabbing a rock prop, one rolled off the pile onto their hand.

Employee was replacing a blown hose on the angle bolter. When they were getting down from the bolter, they placed their weight on to left leg and left knee gave out. Employee finished their shift. Was seen on the 22nd of July and diagnosed and taken off work with a possible tear to their ACL in their left knee.

IE was on a crew changing rollers on the beltline. While crossing a designated belt cross-over, IE bumped head against a roof bolt, resulting in a headache. Injury was not the result of a training or compliance issue.

Employee was side bolting off of the miner in the 15L Tailgate section. EE was stepping down from the miner after grabbing a strap from the strap tray. While stepping down EE felt a pain in EE's left knee. Employee was taken off of work until EE receives a MRI on August 4 to examine EE's left knee.

An unintentional roof call occurred at previously mined area at 101 crosscut 3-4 entry. This was a continuation from a previous fall that occurred at 101 crosscut 5-6 entry. The approximate measurement of the fall is 110' by 16' by 10'. This fall was caused by adverse roof conditions in the form of deteriorating laminated shale.

Employee was in the process of picking up a bundle of 6 foot roof bolts from the scoop bucket. While picking them up to place them on their right shoulder, they felt a pain in their right shoulder. Employee worked all scheduled shift until they were taken off on the 22nd for July after being diagnosed with torn rotator cuff and scheduled for surgery to repair.

An unintentional roof fall occurred along the 4 South track. The fall was in the 70th crosscut from the #6 to #7 entry. The roof in this area previously identified as an area concern and was supported and dangered off. The area proceeded to fall within the supports previously set. The fall was approx 60' by 16' by 8' high.

A shaft employee was closing the concrete chute on an empty bucket and missed the latch. By missing the latch it caused the chute to fall catching EE's right middle finger in a pinch point.

While using a grinder to cut a piece of metal guarding, the grinder kicked back and struck EE's left index finger causing a laceration. The lacerations was medically closed with 4 stitches. Employee did not miss any scheduled work days

Employee was standing on the pan-line chain when a rock fell from the roof, at the tip of the shields, and flopped toward the employee. The rock struck the employee and pinned them against the panline structure. The rock was approx. 9 ft long by 5ft wide by 16inch thick. This incident resulted in fractured legs.

Employee was using a pry bar to move a roller. The bar slipped out from under the roller causing EE's hand/finger to be caught between the bar and the belt structure causing injury.

There was an unplanned roof fall in 6 South section #8 entry 104 crosscut. Adverse geological conditions caused the roof to separate and break. It was measured 12' high x 60' long x 16' wide. "The conditions contributing to the fall in 6 South mains area was Adverse Geological conditions in the form of a fault line possibly caused by a stream valley"
