Irving Materials, Inc. · Fall from machine

The EE involved arrived 15 minutes before start time and went directly to his haul truck and started the truck. While in the shop the EE reported to another EE that he had slipped and fallen from his truck after it was started. He said he missed the bottom step and fell against the tire. He slid down the tire, striking his head on the concrete. He died on 11/27/2014.

THE DUST IRRITATED HIS EYE AND THAT EVENING WHILE RU BBING THE EYE. HE SCRATCHED HIS EYEBALL. TWELVE HOURS LATER HE WENT TO THE DUKES MEMORIAL HOSP EMERGENCY RM IN PERU. IND. THEY ADVISED HIM TO SEE A SPCEICALIST WITHIN 12 HOURS IF NOT BETTER.

THE INJURED MOVED BETWEEN THE BLOCKED BUCKET & THE HI-LIFT HOLDING THE BLOCKED BUCKET IN ORDER TO SPEED UP THE PROCESS OF UNHOOKING THE CHAINS TO MOVE THE BLOCKED BUCKET.
