Pendleton Quarry: 112 MSHA citations Metal/Non-Metal

Employee was working from truck bed to remove ramp wall off of portable plant. When removing bolt put in for transport, the wall began to fall towards them. As wall was falling employee fell in the bed and injured left ankle. Wall came to rest on truck bed walls.

Employee was performing a pre operational inspection of a piece of equipment and slipped while in the engine compartment. This led to the employee spraining their right knee.

Employee states that over the past 2 years of operating a loader they have developed a cyst on their left thumb from repetitive motion. Employee will have surgery to remove the cyst.

Employee told manager that they were having bilateral wrist pain from operator the joysticks in the loader. Employee was treated on and off for 20 months before receiving Reportable level treatment.

Employee was tempting to put antifreeze in Cat740. Employee set container on step, then climbed up the steps, once up on the platform, lost balance forcing employee to jump off the platform. Employee landed on feet but fell/rolled onto left shoulder on the ground. Employee will Have to have surgery on left shoulder.

The Mechanic helper was working on a piece of equipment in a tight spot. He had his leg folded under his thigh for a period of time and his leg went numb. He went home and the feeling never returned, so he went to the hospital. Doctor said it was a pinched nerve and may take a few days for feeling to return 100%.

Employee was driving water truck with window down, particles blowing around in cab got in his left eye.

Employee was working with hand tools disassembling commercial crusher. Employee while using a pry bar, pulled/ strained back muscle.

Employee was cleaning rocks from a plugged screen.

Employee picking mud from conveyor line, S & G plant left shoulder became sore. Reported to supervisor but no medical was immediately sought. On 08/20/2012 the employee requested to see doctor, shoulder was sore. First doctor's appointment 08/22/2012. Shoulder previously injured. There has been no lost time and employee continues regular job. Surgery is being considered.

Two employees were loading two hydraulic cylinders onto a forklift, when one cylinder slid into the other smashing my left little finger.

EE was unpluging split master on 6x16 with a bar slipped and hurt his back.

EMPLOYEE WAS CLEANING HOPPER OUT ON STONE PLANT AND SLIPPED ON ICE BRUISING HIS RIGHT KNEE. (CONTUSION)

EE IN GETTING UNDER LOADER FOR INSPECTION AND REPAIR HEARD HIS BACK POP, THEN REALIZED THE FOLLOWING MORNING THAT HE NEEDED HIS BACK LOOKED AT.

HE WAS CUTTING AND GRINDING METAL FOR HOPPER. A PIECE OF MATERIAL GOT BEHIND HIS SAFETY GLASSES.

EE WAS WASHING MATERIAL AWAY FROM CONVEYOR BELTS, WHEN HE PULLED THE HOSE TO REPOSITION IT, HE BUMPED HIS HEAD ON THE PLANT.

EMPLOYEE WAS PULLING WITH ARMS OVER HIS HEAD INSTALLING A NEW TAIL PULLEY WHEN HE INJURED HIS LEFT SHOULDER.

EE WAS IN PROCESS OF SWITCHING A GATE TO MAKE #2STONE WHEN HE STRAINED HIS LEFT SHOULDER

HE INJURED HIS ANKLE HELPING WITH PIPE PROJECT, STEPPED WRONG.

HURT BACK SHOVELING IN TUNNEL.

EMPLOYEE WAS HELPING ANOTHER EMPLOYEE DO SOME REPAIRS, WHEN HE STEPPED ON A HOSE TWISTING HIS RIGHT ANKLE.

EE WALKING THE PLANT TO BEGIN HIS SHIFT, STEPS ON AN UNEVEN PART OF CATWALK,W HICH CAUSE DINJURY TO HIS LEFT KNEE. EE'S KNEE BENT OUT SLIGHTLY.

EE WAS WORKING NIGHTS AT PLANT AND NOTICED WHILE WORKING THAT HIS LEFT THUMB WAS HURTING. HE CONTINUED TO WORK WENT HOME TO BED AND WHEN HE AWOKE HE HAD SWELLING ON LEFT WRIST. THE INFORMATION I GOT FROM EE WAS THE DR THINKS HE MAY HAVE SOME SORT OF BONE DISEASE THAT MAY BE CAUSING A DETENARATION OF BONE.

EE WAS GOING UP LADDER ON 980G WHEN HE SLIPPED WITH HIS LEFT MISSING ITS GRIP, LEFT HIM HOLDING ON WITH HIS RIGHT HURTING HIS RIGHT SHOULDER. PS THIS WAS NOT A LOST TIME ACCIDENT UNTIL 1/5/200 0, THE DAY OF SURGERY.

EE WAS WORKING IN THE SHOP ON THE SPLITMASTER FOR THE STONE PLANT, WHEN HE STRAINED HIS LOWER BACK MOVING PLATE STEEL

CUT LEFT HAND CUTTING RIDLE RUBBER FOR SCREEN

EE WAS RUNNING THE SCRAPER DOWN SOME FILL, RAN OVER A ROCK THAT HE DIDN'T SEE. AT THAT TIME THE BACK OF HIS HEAD SLAMMED INTO THE CAB. THIS CUT HIS HEAD CAUSING HIM TO GET 5 STITCHES.

INJURY WAS FROM LIFTING A BIG ROCK OFF TRUCK DUMP.

HIGH WIND DUST BLEW IN EYE.

MOVING SHOR TPIECES OF I BEAMS PICKED UP ONE END AND HURT BACK.

MEN WORKING ON JAW CRUSHER USED A COMALONG TO PULL JAW BACK FROMCLEARANCE TO WORK ON TOGGLE BEARING WHEN WELD ON EYELET BROKE ALLOWING JAW TO SWING FREE CRUSHING EE FOOT
