While washing the pad under the crusher, the employee did not realize that they were close to the edge and slipped off. The edge of the pad is approximately 8 inches tall. When the employee fell they landed on their right elbow causing a dislocation.

According to the electrician, the wire came out of the lug and landed on the fuse holder causing the arc when the employee went to install the new fuse. The employee was not able to see the loose wire due to the plastic guard that keeps individuals from making contact with the live top. During the investigation it was discovered that the box was properly locked out.

While cutting legs free on a refeed hopper at the jaw, a piece of rust popped and went behind employee's PPE. Employee was seen by a physician and diagnosed with a right eye abrasion.

EE was helping check the finishing plant out during start of shift. EE grabbed a 2 inch water hose and when they bent over they told the foreman they hurt their lower back and could not stand up straight. The foreman brought EE to break room and asked if they need medical assistant. EE stated that they did, and then was taken to the company doctor.

We had about 1 inch of snow on the ground. The maintenance crew drug a piece of metal across the snow, which packed the snow down tight. When employee crossed the path where the metal had been dragged EE slipped and landed on elbow.

The injured employee was positioning an 8" hydraulic check valve onto the forks of a fork truck. The valve slipped on the forks and the employee attempted to catch it before it fell off of the forks. The Valve pinched his left pinky and ring finger between the fork and the valve injuring those fingers.

On 12/01/13 he reported to work and worked for several hours, then said his ankle was still bothering him, so he went to doctor the next day.

EE told a supervisor he felt a pull in groin. The details didn't appear to be something that could be work related so the supervisor told him he could leave. The ee told the supervisor later that he had pulled something in his groin while moving a screen. Information obtained from the doctor indicates this could be a recordable inj due to conflicting MD information.

MOVED THE DRILL OFF THE BLASTING SITE & WHILE WAITING FOR THE SHOOT, THE DRILLER WAS GOING TO CHANGE THE DRILL BIT. HE HAD PARKED UNDER A HIGH VOLTAGE LINE. ALSO, THERE WAS A GAS LINE ABOUT 10 ' BELOW GROUND IN SAME AREA. DRILL RIG BOOM HIT THE HIGH VOLTAGE LINE, GROUNDED AND CAUSED A GAS EXPLOSION. DRILLER SUSTAINED INJURIES TO ALL 4 EXTREMITIES.
