The injured employee and another employee were spinning the rotor. The injured employee bent down to pick up a dropped tape measure. While bent down, IE pulled on the hammer as part of the process. As this happened, the other EE was spinning the rotor. This caused the injured employee's right arm to be pinched between the hammer and the cheek plate.

The employee strained their back in February while lifting a tire. The employee received no medical treatment for the injury. While receiving a bone scan from a non-occ issue it was discovered that the employee had a fracture to a vertebra. The employee believes this is from the strain they suffered in February.

While changing an elevated belt that was on the ground in below freezing temperatures, EE stepped on the belt to manually pull the belt which caused the EE to slip. EE complained of left shoulder pain from the fall. EE was told prior to the fall to use a skid stir for the particular task.

The miner was welding hammers on the primary crusher. He took welding helmet off and using his hands brushed the dust off of himself. When he brushed the dust off the fan he was using to blow welding fumes out of crusher, blew a small piece of metal in his eye. The miner had on all of his safety equipment, including his safety glasses.

Employee was lifting a 1.50" wash down hose up from side of wash bin to wash out a classifier tank & sand screw, the hose had been lowered over the side of the bin to drain so it wouldn't freeze, and evidently some water remained in the hose & froze. The hose was heavier than normal.

THE PART BEING WIRE BRUSHED WAS NOT SECURED CAUSING THE WIRE TO FLING THE PIECE STRIKING HIM.
