REPORT SUGGESTS POSSIBLE DEGENERATIVE ARTICULATIONS IN T HE LOWER LUMBAR REGIONS AND TOLD HIM THAT HIS CONDITION COULD BE WORK RELATED AND HE NEEDED TO SEE A SPECIALIST.

PRESSURE WAS BEING APP LIED TO THE PIN FROM THE OTHER SIDE BY A PORTABLE HYDRAULIC PRESS. ONE OF THE STRIKES W/ HAMMER HIT CENTER PIN, CAUSING A PIECE TO FLY OFF & HITEE ON CALF AREA. METAL CHIP REMOVED W/3 STITCHE

USING A 10LB SLEDGE HAMMER WITH BOTH HANDS SGRIPPING THE HANDLE, HE WOULD SWING AND STRIKE THE SHIM, DRIVING IT IN FURTHER, REMOVING THE PLA NT IN THE SUPPROT LEG. DURING HIS LAST STRIKE, HIS RIGHT HAND THUMBNAIL CAUGHT THE EDGE OF A TIRE, WHICH IS PART OF THE PLANT. WHEN THIS HAPPEN, IT CAUSED HIS THUMBNAIL TO BE PULLED BACK OFF
