Employee was helping powder and stepped on rock rolling an ankle and falling on their buttocks and left hip area. Employee didn't feel they needed medical evaluation. Two weeks later employee started feeling sharp pain down left leg. This became reportable on Oct. 27, 2017 when employee was taken to Chiropractor for a bulging disk in back. Received treatment and realignment.

Employee was loading a truck from a bin. The gate stuck on the chute he was using so he used a pipe wrench to help close the gate. The pipe wrench slipped and smashed his index finger on left hand between the wrench and guard.

Employee was hauling rock with a Cline truck out of underground mine. When he got to the portal he hit a section of rough road. This caused the truck to bounce and jarred his back.

HE CLAIMS THAT ON THE MORNING OF 12-3-94, HIM AND I.NAME WERE LOWERING A SIZE 4AWG CABLE INTO THE MINE THROUGH A PVC PIPE LINED AND GROUTED BOREHOLE. HE CLAIMS THAT ALTHOUGH THE CABLE WAS TIED OFF WITH A ROPE, IT STILL SLIPPED AND STARTED GOING DOWN THE BOREHOLE. WHEN IT DID SO, HE CLAIMS IT PULLED ON HIS RIGHT ARM AND CAUSED HIM TO FALL, DRAGGING HIM ALONG THE GROUND. I.NAME JUMPE

EMPLOYEE WAS POWDERING A DRILL HOLE IN THE MINE FACE IN THE NORMAL MANNER FROM THE PLATFORM ON THE POWDER TRUCK. WHEN THE AIR WAS APPLIED, THE ANFO HOSE KICKED BACKWARDS. THIS SUDDEN AND FORCE FULL MOVEMENT OF THE HOSE CAUSED THE EMPLOYEE TO LOSE HIS BALANCE AND FELL TO THE MINE FLOOR.
EE WAS ASCENDING THE LADDER OF THE LOADER WHEN HE FELT A SHARP PAIN IN HIS LEFT KNEE. HE BACKED DOWN TO THE GROUND, AND FOUND THE PAIN IN HIS KNEE SEVERE ENOUGH TO PRECLUDE HIM BEARING WEIGHT ON THE LEG.
