IP and other employee where using a chain hoist to pick up an electric motor for the #2 Swing Mill into position for the mobile crane to hoist into proper placement. The IP claims the IP disregarded it in the beginning and finished the job but the days following the pain had increased.

EE tripped on a step and fell against handrail striking elbow and forearm. Something in left shoulder popped. Lost range of motion in left arm. EE was seen by Dr on 11/30 & 12/8. Continued to work until 12/14, when EE could not do their job. Going to get an MRI done on 12/26. The company decided to have EE off until the MRI results are completed. Lost time begins 12/15/2023.

Employee was adjusting the deflector shield on a belt at the East quarry. Walking, they tripped over their feet, fell, hurting their shoulder. Went to orthopedic doctor on 10/10/2023. Received steroid injection in left shoulder. No work missed - still working. Unknown if any further treatment at this time.

Employee was moving transite from one roof level to an upper level. EE would crouch to support the transite while another person pulled it up with a rope. EE's right foot slipped while crouching to boost the transite. EE dropped down to knee and the transite sheet hit EE's chest on the left side.

Working on the 7th floor of the Mill Bldg, re-wiring the tripper motor and checking proximity switches. Employee got overheated causing heat exhaustion. Employee was taken to ER where EE received IV fluids, and sent home. Work release to return to work the next day.

Customer truck driver fell from the lid closing station after failing to correctly align the truck at the closing station. We assert that the incident does not meet the definition of an accident and the injured party was not a miner, so 30 CFR 5a does not require submission of the report.

Employee was walking down stairs from the baghouse, EE missed a step or lost EE's footing, missing 3 steps when EE fell on the transition platform twisting left ankle, landing on EE's left knee, straining EE's neck and must have hit EE's head on the hand rail causing EE to have a mild concussion.

Employee was replacing decking at the feed shelf. When EE was stepping onto the ladder, EE's foot slipped on the ladder. EE grabbed a pipe causing EE to swing into the ladder. EE received three broken ribs and a strain to the left shoulder. Will be off work 3-4 weeks to allow the shoulder and ribs to heal.

Employee was changing valves at bulk silos on the roof. They stepped down off the roll around ladder and slipped on a small patch of ice. Employee fell, twisting left knee. Grade II MCL strain. Will be off work 3-6 weeks.

EE was walking up the stairs in coal handling, when EE was on the fourth stair, EE slipped & fell forward on right wrist. EE cracked a bone & dislocated right wrist. Surgery was performed on wrist on 2/9/22 & will be off work for an unknown amount of time.

While packing cement the employee was not aware the cement bag was not in the #2 cradle properly as it filled. The bag pushed EE's right foot under the #1 cradle, cutting the top of EE's right foot. Right foot required 6 sutures.

Employee was emptying out a railcar loaded with Gypsum rock. EE was removing an air powered vibrator (weighing approx. 45 lbs.) from the railcar. EE removed the vibrator and turned to lay it down on the ground, approximately 36 inches. When EE turned, EE felt a sharp pain in back.

Employee was throwing rocks off walkway when a rock smashed EE's finger against handrail. Employee smashed finger, tore nail off finger and out EE's finger. Received 6 sutures to EE's left hand ring finger.

Employee stepped away from a railcar and got right foot hung up on an air hose and fell on right shoulder. Employee immediately reported accident but did not want to receive any treatment at the time. No time loss or treatment beyond first aid until 12/9/2020.

Employee was operating the loader in the east quarry, cleaning up the remaining rock on the quarry floor when they ran over a 2 ft. diameter rock that fell out of the loader bucket. The loader front tire ran over the rock causing the loader to rock to the left, pushing the employee into the ROPS inside the cab, hitting their head, causing a cut to the head requiring stitches.

Employee was moving scrap metal in a wheelbarrow when they brushed against a wall, causing the scrap metal to slide into their right hand. This resulted in a 2 cm long laceration requiring 5 sutures.

EE was climbing up onto the 35 ton haul truck after doing the pre-shift inspection. EE slipped on a glaze of ice on the step and mud on EE's boots, causing EE to lose grip with hands. EE fell on left shoulder and fell about 2 feet. X-rays taken and later an MRI indicating rotator cuff tear. EE continued to work until surgery on March 18, 2019.

EE and another worker were removing an analyzer from the kiln feed shelf. As the analyzer was pulled out ee dropped end due to sudden pain in right shoulder. The weight of end was no more than 40 lbs. Results of MRI indicate possible rotator cuff tear.

The Monarch Cement Company · Absorption of radiations, caustics, toxic and noxious substances

Employee received a first degree burn to right cheek and chemical burn to eyes from kiln feed during an inspection. EE was not wearing proper PPE. EE was treated at the hospital and then later by an eye Dr and by another Dr. ee will be released to go back to work the evening of 8/7/17.

EE was carrying 40lb sump pump motor from work bench to the pickup, when the pump cord got tangled on the work bench. The EE turned to see what the cord was tangled on when foot slipped and EE fell onto the concrete floor holding onto the pump. Pump landed on left hand causing a comminuted fracture to left hand. EE will see ortho doctor on 1/9/17 for further evaluation.

Replacing a motor on K660 coal drag. While lowering the motor to the ground it got caught on a mounting bracket causing the motor to roll, resulting in side-loading the gate on the hook of the Tuggit. Failure of the gate caused the motor to fall. In the fall the motor struck the removed guard causing it to slide into the employee's left shin and right ankle.

EE was cutting a piece of metal on metal shear. EE was lining up piece to be cut when ee pushed down on pedal causing plunger to engage and drop down. When ee plunger dropped down left pinky finger was under plunger, smashing finger on tip and pulling off nail and breaking bone at tip. EE had fingertip skin sewn up and no lost time was involved.

Employee was coming out of the cab of the dozer, walking on the tracks to step down to the ground, when EE stepped into the area of a missing plate on the track. EE's right foot slipped into a missing track piece which is around 8" x 24" in diameter. Employee twisted right knee with a possible MCL strain. An MRI was done, waiting for results. This will be a lost time accident.

Employee was sandblasting and was climbing down to platform when EE's foot slipped allowing EE's right hand, that was holding onto a piece of angle iron, to slip off cutting the inside part of EE's right hand below EE's thumb. He will under go therapy/rehab on hip.

While installing the gasket between the 8" flanges the pry bar slipped and the flange closed together pinching the end of his index finger on his right hand. The end of the finger was cut off and surgery performed later that afternoon. Employee returned to the doctor on Thursday, 11/7 for evaluation. Could be off 2-4 weeks.

EE was flagging the crane operator while the forklift was being used to pull the crane into gear. The crane went into gear. A 3" nylon strap was hooked up to the crane and a 3/8" choker was hooked up to the forklift. The strap and choker were connected with a 7/8" shackle. Due to too much tension the choker broke. A slingshot effect occurred and the shackle hit employee's leg.
