EE was exiting the 980loader in the shop,maintaining 3-points of contact,EE's L foot was on the ground & EEs R foot got stuck in the bottom step.This threw the EE off balance and EE fell backwards and EEs L side landed on a pallet with a cylinder on it.The pallet was approx 10'away from the loader.EE broke 4 ribs on EEs L side from the fall onto the pallet with the cylinder on it.

Associate was shoveling rock off of our 30" silo belt. EE was going to go to break and started walking down the belt. As EE was walking down the belt, EE slipped and fell onto a metal chute. The associate landed on the right side rib cage, breaking four ribs. A contributing factor was that it had started to rain, which caused the belt to become slippery.

Associate was attempting to dispose of an air filter, from a 988 FE Loader, when ee went to lift it into the dumpster ee heard shoulder pop. Upon examination from a doctor it was determined that the injury was a torn rotator cuff requiring surgery to repair.

WHILE STEPPING UP INTO A TRACTOR THE EMPLOYEE'S LEFT HAND SLIPPED OFF THE HANDLE AND HE FELL BACKWARDS TO THE SHOP FLOOR. WHILE ATTEMPTING TO CATCH HIMSELF HE INJURED HIS LEFT WRIST.

EE WAS OPERATING A 16G CAT MOTORGRADER AND WAS HIT HEAD-ON BY ANOTHER EE OPERATING A D400E ADT (OFF THE ROAD) DUMP TRUCK. THE IMPACT CVAUSED FIRST EE TO HAVE TINGLING IN HIS UPPER EXTREMITIES FROM JARRING MOVEMENT OF IMPACT.

THE EE WAS INSIDE A CRUSHER HOPPER TRYING TO MOVE SOME ROCKS AROUND SO THE CRUSHER WOULD RUN. WHILE MOVING THE ROCKS, HE GOT HIS LEFT HAND RING FINGER IN BETWEEN THE ROCK AND THE HOPPER WALL. HE BROKE THE TIP AND HAD A SMALL CUT ON THE FINGER.

THE EE WAS TIGHTENING A NUT WITH A RACHET AND SOCKET WITH ANOTHER EE. THE SOCKET SLIPPED OFF THE NUT AND PINCHED THE EE'S LEFT HAND RING FINGER BETWEEN THE RACHET HANDLE AND A HAND RAIL. THE TIP OF THE FINGER WAS FRACTURED AND THE FINGER NAIL WAS TORN OFF.

THE EMPLOYEE HAS SEEN MD\S & CHIROPRACTORS IN THE PAST FOR HIS BACK PROBLEMS & HAS A LONG HISTORY OF BACK PROBLEMS.
