EE reported on 7/9/07 that his right hand/fingers were going numb. He states it's from operating the front end loader. He indicated it started on 6/1/07. He did not report it until 7/9/07. Dr. issued him a wrist splint. He was allowed to go back to work immediately with no restrictions or lost work time. He has a follow-up appointment on 7/20/07.

Employee was getting out of his vehicle in the parking lot. As he placed his right foot on the ground, it slipped out from under him. He caught the door of his vehicle preventing a fall, but felt a pain in his back. The parking lot was covered with hard-packed snow.

EE REPORTED HIS WRISTS WERE HURTING & HE WAS HAVING NUMBNESS IN HIS HANDS & LOWER ARMS. I WAS INFORMED ON 1/9/02 THAT HE HAS CARPAL TUNNEL SYNDROME IN BOTH WRISTS WHICH MAY RESULT IN LOST TIME IN THE FUTURE.
THE OPERATOR DESCENDED FROM THE CAB OF A 769C HAUL TRUCK ON THE PASSENGER SIDE. HE STEPPED ON A FAULTY STEP AND FELL, WHILE HOLDING THE LADDER WITH HIS HANDS, TO THE GROUND. THE INCIDENT RESUL TED INA STRAINED KNEE.

THE EE WAS WORKING IN CINDY CONDITIONS PERFORMING NORMAL ASPECTS OF HIS JOB. AT SOME POINT (UNDETERMINED) A FOREIGN BODY ENTERED HIS RIGHT EYE.
EE WAS WORKING ON A NEWLY INSTALLED AND NOT YET OPERATIONAL STACKING CONVEYOR STRINGING ENSTEP CORD. HE WAS APPROX 24' IN THE AIR BY THE HEAD PULLEY OF THE CONVEYOR WHEN THE ENTIRE STACKER COL LAPSED. HE RECEIVED MULTIPLE FRACTURES TO HIS LOWER RIGHT LEF AND LIGAMENT DAMAGE TO THE SAME AREA.
EE WAS CHANGING A DRILL BIT FROM A GYPSUM SET-UP TO A LIMESTONE SET-UP. IN LIFTING THE BIT TO THE SHAFT, HE STRAINED HIS LOWER BACK.

EE'S CUSTOMARILY HEAD CARS WITH GYPSUM IN THEM TO LOOSEN THE GYPSUM. THE HEATED METAL ALLOWED RUST OR SOME OTHER ARTICLE TO ENTER EE'S EYE.

THE EE WAS LIFTING A METAL GUARD WITH TWO OTHER EE WHEN THE WEIGHT OF THE GUARD SHIFTED AND FELL ONTO HIS HAND THUMB AREA, CAUSING A BRUISE TO HIS HAND.
THE EMPLOYEE STATES HE WAS REMOVING A CHAIN PULLER (COME-A-LONG) FROM ITS POSITION ON A HORIZONTAL BEAM ABOVE THE SHAKER SCREENS IN THE QUARRY SCREENHOUSE. AS HE UNHOOKED THE CHAIN PULLER AND TURNED TO HIS SIDE, TO PULL IT DOWN, HE FELT A PAIN IN HIS BACK.
EMPLOYEE STANDING AND TALKING WITH FELLOW EMPLOYEE AND COLLAPSED INTO FELLOW EMPLOYEE'S ARMS. TRY TO REVIVE HIM WITH CPR, DID NOT SUCCEED. EMT'S ARRIVED AND TAKEN TO TAWAS ST. JOSEPH HOSPITAL. AT 3:20 PM. DR. CAME OUT AND STATED VICTIM HAD DIED.

EE STATES HE WAS WALKING AROUND THE BACK OF OUR QUARRY PRIMARY PANFEEDER IN THE CRUSHER BUILDING WHEN HIS FOOT SLIPPED ON THE MUDDY FLOOR CAUSING HIS LEFT TORSO SIDE TO STRIKE AGAINST AN I-BEA M SUPPORTING THE PANFEEDER.
EMPLOYEE STATES HE CLIMBED OFF THE DRAGLINE TO GO TO THE BATHROOM. HE WALKED BACK TO THE DRAGLINE & CLIMBED UP THE LADDER TO THE DRAGLINE SHOE, HE THEN SLIPPED ON THE ICE & SNOW & FELL APPROX 3-4' TO THE FROZEN GROUND. HIS L ARM EXTENDED;HIS RIB CAGE EXPOSED; THUS THIS IS THE AREA WHERE 4 RIBS RECEIVED A SLIGHT FRACTURE. HE LANDED ON THE SNOW & ICE ON THE GROUND.

HE DID NOT ACTU ALLY KNOW ANYTHING WAS WRONG UNTIL LATER ON THATDAY. HE THINKS THE PIECE OF METAL MAY HAVE BEEN STUCK TO HIS FOREHEAD OR HAIR AND FELL INTO HIS EYE WHILE CLEANING UP OR SHOWERING.

THE EE STATES HE WAS WELDING ON THE STACKING CONVEYOR, WHEN HE WAS STANDING ON THE WALKWAY AND HIS FOOT SLIPPED THUS CAUSING ONE FOOT TO DROP LOWER THAN THE OTHER, THUS RESULTING IN LOWER BACK PAIN.
THE EMPLOYEE STATES HE WAS SHOVELING BEHIND THE PRIMARY PANFEEDER. DURING THIS ACTIVITY, HE SLAMMED HIS LEFT ELBOW INTO A VERTICAL I-BEAM AGAINST THE BACK WALL, THUS CAUSING A SEVERE BRUISE T O HIS LEFT ELBOW AND SOME INFLAMMATION.
THE EMPLOYEE STATES HE WAS DRIVING UP TO THE DRAGLINE IN THE QUARRY PICKUP (EMPLOYEE WAS RIDING AS A PASSENGER), WHEN THEY HIT A BUMP CAUSING HIS HEAD TO HIT THE FRAMEWORK OF THE PASSENGER DOO R. THIS RESULTED IN A CUT TO HIS HEAD REQUIRING FOUR STITCHES. (THE EMPLOYEE STATES HE WAS WEARING HIS SEATBELT).
